But these guidelines remain largely under used due to multiple reasons, including a lack of patient specific information, and for lack of integration with the clinician's workflow. Here we implement a novel approach to show the use of a guideline as a reference map to present a visual overview of the patient's treatment profile. We also demonstrate the use of the reference map approach to present electronic medical record data in the context of a comparative patient population. Further, we show the utility of this approach for integration within the clinician's workflow to facilitate their decision making process at the point of care.","PeriodicalId":90523,"journal":{"name":"IHI ... : proceedings of the ... There are many factors (such as time, volume of information and technical ability) that can potentially compromise the quality of decisions made for patients. In this work we demonstrate and evaluate a new medical decision making support system, called OMeD, which automatically answers medical queries in real time, by collecting and processing medical information. OMeD utilizes a natural-language-like user interface (for querying) and semantic web techniques (for knowledge representation and reasoning) to answer queries. We compare OMeD to a set of standard machine learning techniques across a series of benchmarks based on simulated patient data. The conventional techniques attempt to learn the answer to a query by analyzing simulated patient records. The sparsity of the simulated data leads conventional techniques to frequently misidentify the relationships between medical concepts. In contrast, OMeD is able to reliably provide correct answers to queries. Unlike conventional automated decision support systems, OMeD also generates independently verifiable proofs for its answers, providing healthcare workers with confidence in the system's recommendations.","PeriodicalId":90523,"journal":{"name":"IHI ... : proceedings of the ... Due to the complexity of patient information and the restrictions on sharing real world problems that arise from real world information, creating and implementing information systems for clinical support is difficult. Lessons from human-computer interaction design can mitigate time and cost while ameliorating the functionality and understandability of the system.\u0000 This paper describes the design of the problem list currently in development at the Regenstrief Institute for Health Care Informatics. In many situations, laws and regulation rules require the acquisition of patients' consent before one can access the patients' health data. However, in practice, patients oftentimes have difficulties determining whether they should permit or deny a certain access request. In this article, we propose an analytical approach to assist patients in the consent management of their medical information. Our consent management system employs a statistical learning method that evaluates the benefits and risks associated with access requests, so as to make personalized recommendation on consent decisions. Multiple factors are considered in the assessment process, including the importance of the request, the sensitivity of the requested information, and correlation information. We have implemented a prototype of our solution and performed evaluation with large-scale medical records.","PeriodicalId":90523,"journal":{"name":"IHI ... : proceedings of the ... In past decades, many preventive strategies have been recommended to reduce the risk of CVD. However, current CVD risk prediction schemes are not targeted to personalized and optimized recommendations. The goal of this study was to better identify individuals at high risk of a CVD event, and recommend an optimal set of risk factor changes that could reduce the risk of long-term CVD events. We identified 100 demographic, lab, lifestyle, and medication variables for 12907 individuals who participated to the ARIC study and had no CVD events at baseline. We examined the prognostic performance of these features in isolation and ranked them based on mutual information. Then we combined those features to build predictive models using k-nearest neighbor prediction to estimate the 10-year CVD risk for each individual. Our feature-ranking method agreed with traditional risk factors identified by a domain expert. Our approach was successful in identifying cases with high risk and performed as well as traditional methods. Then we applied inverse classification to find the personalized optimal changes to reduce 10-year CVD risk. We also created a personalized package of five optimal changes for each individual to reduce their 10-year CVD risk. This approach can be applied to other chronic disease risk prediction and personalized recommendations, and may be useful to both health care providers and patients in making personalized health care recommendations and decisions.","PeriodicalId":90523,"journal":{"name":"IHI ... : proceedings of the ... However, these models present several obstacles to widespread adoption. They are complex entities and impose a high development and maintenance cost. Current tools can be opaque, requiring multidisciplinary collaboration between a modeler and an expert programmer, and another round of translation when communicating with domain experts. In this paper, we describe the use of functional reactive programming (FRP), a programming paradigm created by imbuing a functional programming language with an intrinsic sense of time, to represent agent-based models in a concise and transparent way. We document the conversion of several agent-based models developed in the popular hybrid modeling tool AnyLogic to a representation in FRP. We also introduce Frabjous, a programming framework and domain-specific language for computational modeling. Frabjous generates human-readable and modifiable FRP code from a model specification, allowing modelers to have two transparent representations in which to program: a high-level model specification, and a full functional programming language with an agent-based modeling framework.","PeriodicalId":90523,"journal":{"name":"IHI ... : proceedings of the ... Currently, the first arriving rescue units perform the triage algorithm on paper instead of a mobile device. By using mobile devices, the patients' triage state and position can be instantly shared through a network. We implemented a map application to visualize this data on a rugged tablet PC that is intended to be used by the Ambulant Incident Officer (AIO). Even though, using a mobile device offers more benefits, it also requires some mental efforts from the user. The goal of the SpeedUp project1 is to ensure the speed-up of the rescue process. It is crucial to carefully develop, introduce and evaluate the User Interface (UI) iteratively close to the target group and adapt it to an MCI situation. Thus, multiple UI concepts have been developed to compare, rate and optimize them. This paper represents a follow up study and focuses on approaches to select patients on a digital map displayed on a heavy rugged tablet PC. An evaluation is performed to estimate how intuitive, efficient, and ergonomic the UI is without the need of special training for the target group and to increase the acceptance of new devices.","PeriodicalId":90523,"journal":{"name":"IHI ... : proceedings of the ... According to a national survey, about 44% of students engage in high-risk drinking activities. This paper presents a machine learning approach to a secondary analysis of data collected in a college drinking study at the University of Connecticut Alcohol Research Center sponsored by the National Institute on Alcohol Abuse and Alcoholism. Existing alcohol studies are deductive where data are collected to investigate a psychological/behavioral hypothesis and statistical analysis is applied to the data to confirm the hypothesis. However, the collected data often carries information beyond the original hypothesis. Our approach aims to discover knowledge from multivariate data collected at a major university campus, which may or may not confirm the original hypothesis and lead to potentially new insights. The proposed machine learning approach can effectively identify risk and/or protective factors for high-risk drinking that can be used to help detect and address the early developmental signs of alcohol abuse and dependence within college-aged students. We demonstrate the use of a statistical feature selection method, 1-norm support vector machine (SVM), to help classify college students as either heavy or low-risk drinkers and simultaneously select the risk factors for heavy drinkers. Results of our experiments are evaluated by several psychologists to delineate risk or protective factors and the interaction among these factors for college drinking behaviors.","PeriodicalId":90523,"journal":{"name":"IHI ... : proceedings of the ... However, it is written in English in some non-English native countries and becomes a barrier for a patient to read. In this paper we propose a framework for rapid acquisition of bilingual medical summaries using machine translation (MT) techniques. We describe a medical summary corpus and some terminological databases prepared for the framework. We then touch on the challenging issues of MT adapted from generic to specific domains, and propose a pattern translation scheme to achieve domain adaptation based on a background statistical MT system. We identify the significant patterns to capture the specific writing styles in a medical summary. The patterns are then translated with the involvements of doctors. Our major concern is to reduce the cost of translation and better allocate the efforts made by the domain experts. The experimental results show the proposed methods are effective in terms of the significance and diversity of the patterns. The approaches to integrate the mined patterns into background MT are also discussed.","PeriodicalId":90523,"journal":{"name":"IHI ... : proceedings of the ...
