We present the case of a 32-yearold female patient, without any particular history, who consulted the emergency room for a red right eye with major chemosis, palpebral edema, and decreased visual acuity. There was also a crusty rash in the V1 territory, suggesting herpes zoster. Visual acuity was 2/10 in the right eye and 10/10 left eye (Figure 1 and 2). Examination at the slit lamp revealed a non-hypertensive granulomatous anterior kerato-uveitis of the right eye. We report a clinical case of a 16 years old aged male, who presented in the emergency room with a painful red eye, severe photophobia, and tearing right eye. The slit lamp examination was showing unusual anterior uveitis with hypopyon. The visual acuity was 2/10 in the right eye. The slit lamp examination of the left eye was unremarkable. Described for the first time by Friedrich Best, It is due to the accumulation of lipofuscin in the macular pigment epithelium. We report a case of a 13 years old female with no medical history. Slit lamp examination revealed unremarkable findings in the anterior segment of both eyes. Posterior segment exam, at the macula, showed a well circumscribed yellow material similar to the egg yolk structure, in both eyes (Figure 1). Figure 1: Retinography with bilateral egg yolk-like image on macula. The Optical Coherence Tomography (OCT) macula revealed Pigmentary Epithelial Detachment (PED) and LE atrophic scar. The systemic examination had no remarkable findings. The name of the 2 corneal pathways, and 2 iris bites will rely on the name of the corresponding filament end that passes through, just before the loop is externalized: Sp/SP: short-end puncture/paracentesis, Lp/LP: long-end puncture/paracentesis, SIB: short-end iris bite, LIB: long-end iris bite. When the loop is externalized, the “intermediary segment”: (I) is created from L. The limits of (I) have been adjusted: instead of generally admitted limits from the middistance between SIB and LIB, to the middle of the externalized loop, I suggest that the S/(I) limit be situated at 1mm after the exit point of the loop from SP, and the (I)/L limit be situated at the re-entry point of the loop in SP. Consult for a decrease of vision on his right eye that has progressed for 3 weeks. Ophthalmic examination found visual acuity: 2/10 P10 in the right eye, the left eye presents enophthalmos with an opacified cornea; the anterior segment was normal in the right eye, fundus examination shows hyalitis graded 1+, papillary edema, peripapillary hemorrhages and stellar macular exudates, cottony nodules, copper arteries with vascular sheathing of the superior temporal branch and areas of retinal ischemia (Figure 1). The inflammatory and serological assessment is negative. Fluorescein retinal angiography shows papillary diffusion of fluorescein and vasculitis (Figure 2). We find serous retinal detachment on OCT (Figure 3). The general examination found bipolar aphthosis. The diagnosis of Behçet’s disease is retained. {"title":"Pneumatic Displacement with Intravitreal Plasminogen Activator (PA) versus Vitrectomy with Subretinal PA for Submacular Haemorrhage","authors":"A. Chandra","doi":"10.26420/austinjclinopthalmol.2019.1099","DOIUrl":"https://doi.org/10.26420/austinjclinopthalmol.2019.1099","url":null,"abstract":"Introduction: To compare the efficacy of pneumatic displacement with intravitreal recombinant tissue plasminogen activator (rTPA) [Group 1] versus vitrectomy with subretinal injection of rTPA with/without anti-VEGF [Group 2] for submacular haemorrhage (SMH) in patients with neovascular age-related macular degeneration (nAMD) in two tertiary referral centres. Methods: Retrospective analysis of thirty consecutive patients presenting with SMH and treated with the aforementioned regimens in two surgical units between 2012 to 2016. Primary outcome measure was SMH displacement. Secondary outcomes included best-corrected visual acuity (BCVA) change post-operatively, SMH height, SMH area, and surgical complications. Optical coherence tomography (OCT) images and clinical data used to analyse outcomes. Results: Eleven patients included in Group 1 and 19 in Group 2. Haemorrhagic displacement was complete in 9 (82.8%) out of 11 and 18 (94.7%) out of 19 patients in Groups 1 and 2, demonstrating no difference between them (p=0.536). BCVA improved by -0.50±0.74 (p=0.045) and -0.72±0.93 (p=0.004) compared to baseline at 6 months in Groups 1 and 2, with no difference between groups (p=0.155). Subfoveal haemorrhage height reduced (Group 1:-900.57μm, p=0.007; Group 2:-607.27μm, p<0.001), without difference between groups (p=0.582). SMH area reduced significantly in Group 2 but not 1 (Group 1:-44.18μm, p=0.078; Group 2:-30.28μm, p<0.001), without difference between groups (p=0.913). Conclusion: Intravitreal treatment and vitrectomy were equally effective at subfoveal haemorrhagic displacement. BCVA gains did not differ significantly between techniques. OCT data demonstrated similar efficacy in both techniques.
