Although some etiologies are suggested, the specific cause might not be identified in many cases. Many patients are the women in peri-partum or childbearing age with no significant atherosclerotic risk factors and who will undergo conservative or interventional treatment with close follow-up. Case Report: We report a case of SCAD patient who was diagnosed and treated in university hospital of Sina, Tehran, Iran. She had dissection in the middle and distal part of Left Anterior Descending artery (LAD) which was managed invasively with Percutaneous Coronary Intervention (PCI). Conclusion: Clinicians should have reasonable suspicion of SCAD, especially in young female patients with no apparent atherosclerotic risk factors who complain of Chest pain with signs of cardiac ischemia. Interventional cardiologists are to know SCAD diagnosis, management and follow up.","PeriodicalId":89940,"journal":{"name":"International journal of cardiovascular research","volume":" ","pages":"1-8"},"PeriodicalIF":0.0,"publicationDate":"2020-07-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48859079","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Maximal oxygen uptake (VO2max), comprehensive index of cardiac, respiratory, and skeletal muscle functions, is one of the most important measurements obtained in CPET. Anaerobic threshold, the exercise intensity at which lactate starts to accumulate in the blood, is widely used, as it represents an exercise level that is not too strenuous, but not too mild. The Minute Ventilation/Carbon Dioxide Output relationship (VE/VCO2 slope) is often used as a prognostic marker of cardiac failure. The Oxygen Uptake Efficiency Slope (OUES), a submaximal index of cardiorespiratory functional reserve, correlates strongly with VO2max and is utilized as a prognostic tool for various heart diseases including congenital heart defects.","PeriodicalId":89940,"journal":{"name":"International journal of cardiovascular research","volume":"2020 1","pages":"1-4"},"PeriodicalIF":0.0,"publicationDate":"2020-07-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"42750744","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Moderate alcohol consumption is generally considered to be in the range of 1-3 drinks/day. Objective: The present narrative review was aimed to disclose the putative mechanisms of cardioprotective role of moderate alcohol consumption. Design: Narrative review of published studies. Evidence acquisition: This review was conducted by collecting clinical, epidemiological and translational research evidences from PubMed, Google scholar and Cochrane databases. Results: Several clinical and epidemiological studies outlined that drinking regularly of moderate amount of alcohol might decrease the incidence of debilitating cardiovascular diseases. Moderate Alcohol consumption increases high-density lipoproteins and reduces the oxidation of low density lipoprotein. It has also a capacity to induce the expression of endothelial nitric oxide synthase and synthesis of nitric oxide. In addition, moderate alcohol consumption induces the expression of putative cardio-protective proteins in the heart during ischemic conditions. Furthermore, it reduces unnecessary platelet aggregation and blood coagulation, facilitates smooth muscle cell proliferation during vascular insult, enhances insulin sensitivity and probably plays its cardioprotective role by reducing acute and chronic psychosocial stress which exerts allostatic load not only on cardiovascular system but also on the body. Conclusion: Cardiovascular diseases, Ethanol, Moderate ethanol consumption.","PeriodicalId":89940,"journal":{"name":"International journal of cardiovascular research","volume":" ","pages":"1-4"},"PeriodicalIF":0.0,"publicationDate":"2020-07-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48739140","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objective: To report a case of a child who had febrile illness with epistaxis, then 2 months after developed rheumatic valvular heart disease. Case Report: A 10-year old girl with one-week history of high-grade fever, A day history of bleeding through the nostrils. She did not have sore throat, joint pains nor past history of epistaxis. She was febrile (38.70 C), moderately pale and anicteric. She was transfused within 24 hours, treated for sepsis and malaria. She had features of cardiac decompensation on 4th day of admission despite adequate blood transfusion. Results of complete blood count, peripheral blood film and clotting profile were not suggestive of bleeding abnormalities. Fever resolved after several antibiotics and antimalarials. She defaulted follow up for 2 months then presented with progressive easy fatiguability, cough and fast breathing. No prior fever, joint pains or sore throat. She was in respiratory distress and tachycardic. She had displaced apex beat and apical pansystolic murmur. Echocardiography revealed RHD with mitral and aortic regurgitation. She is presently on follow up in Paediatric cardiac clinic. Conclusion: The patient had febrile illness with epistaxis and heart failure some weeks before diagnosis of RHD supporting the possibility of the initial presentation to be probably ARF with epistaxis. At risk children presenting with fever, epistaxis and cardiac decompensation should be evaluated for ARF.","PeriodicalId":89940,"journal":{"name":"International journal of cardiovascular research","volume":"2020 1","pages":"1-3"},"PeriodicalIF":0.0,"publicationDate":"2020-05-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"41767500","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: Transthoracic Echocardiography (TTE) examinations were performed routinely in a cohort of 2756 patients admitted to the internal medicine ward. Demographic and clinical variables were collected retrospectively. Results: A total of 2756 patients (48.1%; n=1201 females) with a mean age of 76.3 ± 4.3 years were enrolled in the study. Almost half of the patients (41.8%; n=1153) showed significant heart pathology: Left Ventricular Ejection Fraction (LVEF) 36 mmHg, or pericardial effusion, which required further diagnosis and/or therapy. LVEF 50 mmHg. RVSP >50 mmHg was associated with lung disease (49.2%, n =87), left heart disease (29.6%, n=56), and pulmonary embolism (7.3%, n=13). In the examined group, 28.3% (n=781) of patients required specialist consultation and 10.7% (n=295) were considered as candidates for surgery. Conclusion: TTE is an essential non-invasive tool to evaluate heart structure and function. Routinely performed echocardiography has a significant impact on the decision-making process or treatment in almost half of the patients admitted to the internal medicine ward.","PeriodicalId":89940,"journal":{"name":"International journal of cardiovascular research","volume":" ","pages":"1-7"},"PeriodicalIF":0.0,"publicationDate":"2020-05-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48185129","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, published studies on PCI induced cardiac damage have yielded conflicting results. Aims: To determine whether RIPC reduces reperfusion injury and cardiac damage in Indian patients undergoing elective PCI. Materials and Methods: This prospective, randomized, control study was conducted among Acute Coronary Syndrome (ACS) patients undergoing PCI from July 2017 to October 2018 in Pt. Jawaharlal Nehru Memorial Medical College, Raipur and Dr. Bhimrao Ambedkar Memorial Hospital, Chhattisgarh. One group received RIPC (three 5-minute inflations and deflations of a standard blood-pressure cuff on the upper arm) at 1 hour before PCI. The other group which did not received RIPC served as control group. Cardiac biomarker release (Troponin-I and Creatine Kinase- MB), Electrocardiographic (ECG) and echocardiographic changes were measured in all patients before and after PCI at different time intervals. Results: A total of 52 patients were randomised and equally distributed into control and RIPC group (26 each). Post PCI, there was 77.29% reduction in mean Area Under Curve (AUC) for Troponin-I in RIPC group (8.84 ± 9.72) compared to control group (38.93 ± 79.11). Also, 64.82% reduction was found in mean AUC of CKMB in RIPC group (179.95 ± 120.7) compared to control group (511.65 ± 701.0). Conclusion: RIPC of the upper arm before primary PCI in patients with ACS could provide protection against cardiac damage and ischemia-reperfusion injury.","PeriodicalId":89940,"journal":{"name":"International journal of cardiovascular research","volume":"2020 1","pages":"1-6"},"PeriodicalIF":0.0,"publicationDate":"2020-05-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46834933","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In 20-45% of atrial fibrillation-related strokes, the arrhythmia is not documented and the patient is asymptomatic from a cardiovascular standpoint, prior to the stroke. Subclinical Atrial Fibrillation (SCAF) is defined as asymptomatic or clinically silent atrial fibrillation. Patients with SCAF exhibit no symptoms during the episode of atrial fibrillation. SCAF is frequently detected by continuous electrocardiographic monitoring in patients without history of atrial fibrillation. Several studies have shown that atrial tachyarrhythmias incidentally detected by implantable cardiac devices such as Implantable Cardiac Defibrillator (ICD) and pacemaker, are associated with a two-fold increase in the risk of death or stroke. While there is strong evidence for the benefit of Oral Anticoagulation (OAC) therapy in reducing stroke risk in patients with clinically diagnosed atrial fibrillation, information is lacking regarding benefit in those with subclinical atrial fibrillation. The aim of this current review is to present the prevalence and predictors of SCAF and to assess the impact of anticoagulation on all-cause mortality and thromboembolic events in patients with implantable devices.","PeriodicalId":89940,"journal":{"name":"International journal of cardiovascular research","volume":"2020 1","pages":"1-9"},"PeriodicalIF":0.0,"publicationDate":"2020-05-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"47850345","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
