This should allow the tailored targeting of the different pain components, thus potentially optimising clinical outcomes.&lt;/p&gt;\u0000&lt;p&gt;We agree with the authors' conceptual framework [&lt;span&gt;2, 3&lt;/span&gt;] but, as accurately discriminating between the above pain phenotypes may not be straightforward, we propose five “simple rules” to guide inexperienced clinicians. Moreover, a comprehensive multimodal therapeutic approach is resource intensive, as physiotherapists, pain specialists, psychotherapists, gastroenterologists, urologists, and dieticians may be involved. Selected interventions, appropriately prescribed for patients with specific pain mechanisms, could avoid undue burden on already overstretched public health systems.&lt;/p&gt;\u0000&lt;p&gt;First, suspect early-onset endometriosis in adolescents complaining of painful periods and abdominopelvic pain (primarily nociceptive pain), as its prevalence in severely symptomatic teenagers is high even when ultrasound is negative [&lt;span&gt;2, 4&lt;/span&gt;]. Diagnostic delay promotes afferent hypersensitivity and fosters the transition from peripheral to central sensitisation [&lt;span&gt;2&lt;/span&gt;].&lt;/p&gt;\u0000&lt;p&gt;Second, at diagnosis, suppress recurrent ovulatory cycles with continuous bioequivalent oestradiol–progestogen combinations or progestogen monotherapies. Generally, ethinyl-oestradiol is pro-inflammatory, whereas progestogens are anti-inflammatory [&lt;span&gt;4&lt;/span&gt;]. Prompt secondary prevention could avoid disease progression, symptom worsening, and the transition from localised pelvic pain to widespread nociplastic pain.&lt;/p&gt;\u0000&lt;p&gt;Third, along with potentially different neurobiological mechanisms, evaluate symptom type. Menstrual pain should be relieved by establishing amenorrhoea; deep dyspareunia may benefit from the excision of deep lesions and treatment of pelvic floor hypertonicity.&lt;/p&gt;\u0000&lt;p&gt;Fourth, try a three-month course of a GnRH agonist or antagonist without ‘add-back’ therapy in non-responders to first-line medical treatment to differentiate between hormone-dependent (presumably mostly nociceptive) and hormone-independent (presumably mostly nociplastic) pain [&lt;span&gt;4&lt;/span&gt;]. This pharmacological test is practicable in any clinical setting, overcomes the potentially confusing uncertainties regarding progesterone resistance, and identifies the best candidates for additional interventions such as aerobic exercise, cognitive behavioural therapy, anti-inflammatory diet, pelvic floor rehabilitation, acupuncture, and non-hormonal pharmacological therapies. This study examines the association between adolescent dysmenorrhea and psychological symptoms.DESIGNCross-sectional study.SETTING116 schools in Stockholm, Sweden.SAMPLE1054 postmenarchal school girls (mean age 14.1 ± 0.7) randomly sampled from a population of 10 299 lower-secondary pupils in a school-based project.METHODSA self-report health survey assessed psychological symptoms using validated instruments. A multiple-choice item identified dysmenorrhea (menstrual pain affecting everyday life) and severe dysmenorrhea (dysmenorrhea that is hard to cope with).MAIN OUTCOME MEASURESPrevalence of dysmenorrhea and severe dysmenorrhea in girls with and without symptoms of depression (Beck's Depression Inventory-II score ≥ 20), anxiety (Generalised Anxiety Disorder 7-Item Scale score ≥ 10), self-injury (≥ 3 instances on a modified Deliberate Self-harm Inventory), and suicide ideation (recent serious thoughts/plans on the Paykel Suicide Scale).RESULTSOverall, 55.1% reported dysmenorrhea while 11.7% reported severe dysmenorrhea. Prevalence was 29%-34% higher among girls with psychological symptoms compared to those without. Severe dysmenorrhea was significantly more frequent among girls with any symptom (prevalence ratio [PR] 2.25; 95% CI 1.61, 3.13), depression (PR 2.60; 95% CI 1.86, 3.63), anxiety (PR 2.89; 95% CI 2.09, 4.00), self-injury (PR 1.87; 95% CI 1.29, 2.71), and suicide ideation (PR 1.75; 95% CI 1.18, 2.58) compared to girls without the same manifestations.
