The Shoulder Pain and Disability Index (SPADI) is considered a highly specific questionnaire for assessing shoulder pain and functions. The objective of the study was to assess the validity and reliability of the Bangla version of SPADI (B-SPADI) among the Bangla-speaking patients.\u0000Materials and methods: The B-SPADI was developed using a designed process that included forward translation and backward translation, authentication, response, and final rectification. Validity and reliability were conducted in 48 patients with shoulder pain. The reliability was appraised by executing internal consistency and test-retest analyses. Its validity was assessed by comparing the SPADI with the SF- 36, Disabilities of the Arm, Shoulder, and Hand (DASH), and American Shoulder and Elbow Surgeons (ASES) questionnaire.\u0000Results: The cross-cultural adaptation method was flawlessly aligned with the content or language. Internal consistency (Cronbach’s alpha) was 0.97 for the total SPADI score. Intraclass Correlation Coefficient (ICC) was 0.95 for the total SPADI with 0.92 for pain subtotal and 0.96 for disability subtotal. The SPADI total score showed a correlation of 0.63–0.71 with the SF-36 physical scales, of 0.89 with the DASH and of 0.94 with the ASES. This disease caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) began in Wuhan, China, in December 2019, and since then has been spreading globally. Objectives: To find out the sociodemographic, clinical characteristics and in-hospital outcome of patients of acute COVID-19 confirmed cases in a tertiary care hospital. Methodology: It was a hospital based observational study. Consecutive samples were taken from RT-PCR positive cases (ranging from mild to severe form) admitted in COVID-19 unit of Shaheed Suhrawardy Medical College Hospital, Dhaka during the period of July 1st to August 31st 2020. Socio-demographic and clinical data were collected by using a pretested structured questionnaire. The severity of the COVID- cases was assessed based on the WHO interim guideline. Analysis was done with SPSS (Statistical Package for Social Science) version-23.\u0000Results: Total 77 cases were found valid for study according to inclusion and exclusion criteria. Male patients were predominant which was 72.7%. Mean age was found 53.32±13.48 years. The mean age was significantly higher (58.82±11.74 years) in severe group. Fever, shortness of breath (SOB), cough and body ache were the most common presenting symptoms. Body ache was significantly higher in mild & moderate group than severe group. SOB and sore throat were significantly higher in severe group (53.5% vs 79.4%) and (9.3% vs 26.5%) respectively. Diabetes 29(37.7%), hypertension 18(23.4%), CKD 16(20.8%) and hypothyroidism 16(20.8%) were most frequent co-morbidities among the patients. Diabetes, CKD, hypothyroidism and COPD were significantly higher in severe group than mild & moderate group. The mean neutrophil lymphocyte ratio (NLR) was found 1.3±0.3 in mild & moderate group and 2.3±1.0 in severe group. Mean C-reactive protein was found 6.2±2.3 mg/L in mild & moderate group and 8.5±3.9 mg/L in severe group. The mean sodium was found 143.6±7.9 mEq/L in mild & moderate group and 136.5±9.8 mEq/L in severe group. The mean potassium was found 4.7±0.7 mEq/ L in mild & moderate group and 3.9±1.2 mEq/L in severe group. Where X-ray chest (CXR) could reveal abnormalities only in 8 cases (10.4%); HRCT-chest was able to find out abnormalities in 56 cases (72.7%). Abnormal HRCT chest was found in 56 patients among them 48(87.3%) showed normal finding on chest x-ray. The difference was statistically significant between two groups. In HRCT chest abnormalities ground glass opacities (GGOs) was the most frequent observation in 33(58.93%) patients. GGOs was found 12(50.0%) in mild & moderate group and 21(65.63%) in severe group. Major complications were pneumonia 39(50.6%) and severe pneumonia 28(36.4%) respectively. Pneumonia was significant higher in mild & moderate group than severe group (76.7% vs 17.6%). Severe pneumonia was observed 27(79.4%) in severe group. The active, sincere and dedicated efforts of Prime Minister’s Office, Cabinet Division, Ministry of Health and Family Welfare and other different ministries have proved this COVID battle to be a winning game for our country. All the doctors, nurses and health workers played the pivotal role along with administration, police and all others. Among the frontline doctors, the fellows and members of all specialties of Bangladesh College of Physicians and Surgeons (BCPS) had a significant participation. They remain to be appraised. This review article probes to reveal the enormous contributions of all fellows and members of BCPS in various sectors of the COVID-19 battle in Bangladesh. This paper analyzed all activities of BCPS fellows and members by collecting data from the COVID-19 dedicated hospitals, Directorate General of Health Services, Bangladesh and directly from fellows of BCPS. A systematic search in the net was also performed using specific search words to extract relevant data and articles published on COVID-19 from Bangladesh. A non-probability sampling technique was used without any structured questionnaire. Data were directly included in MS excel spread sheet. Simple description analysis was performed and results were expressed in tables and charts. We found that fellows and members of BCPS took a significant leadership and provided supreme dedicated services in almost all fields of COVID-19 crisis; e.g. frontline patientcare, education and training, national policy making, research and publication and nationwide guidance and awareness build-up. Among the doctors of government COVID-19 dedicated hospitals in Dhaka, about 650 fellows participated in treating about 1.5 lacs admitted patients. First interim guideline on COVID-19 management in Bangladesh was published by fellows in executive committee of Bangladesh Society of Medicine. It was later transformed into national guideline by DGHS. Fellows were the leaders at national level. The National Technical Advisor Committee and Directorate General of Health Services of Bangladesh are the two main bodies who lead the war in the frontline and both are headed by fellows of BCPS. Journal of BCPS was the first in Bangladesh to publish COVID-19 supplement issue on July 31, 2020 with 12 original articles. So far 128 COVID related articles from BCPS fellows have been published in national and international journals. Fellows fought in the frontline in every sector, shoulder to shoulder with all doctors and health workers of Bangladesh. The PMR Faculty at Bangladesh College of Physicians and Surgeons (BCPS) was established about 30 years ago. As a medical specialist of 21st century, at this point of time, it is important to know and evaluate the spectrum of research activities performed by the trainees of the faculty. Objectives of the study was to make an in-depth analysis of the dissertations completed by the PMR fellow trainees of BCPS since the inception of the Faculty. It was also aimed to discuss the strengths and insights of shortcoming of the research areas and topics with directives for further improvement of the future trainees and faculties ultimately to initiate a dialogue among the stake holders and other specialties. This was an observational mixed method study. Information was gathered from the BCPS webpage, Faculty office and published materials, which was further strengthened with a small group discussion, online literature search and a telephone interview of the PMR fellows to learn their experience that they gained during the dissertation works. A total of 167 (n=167) dissertation topics were available for statistical analysis submitted by the trainee fellows from 1992 to 2021, a period of 30yrs. More than half (50.9%) of the submitted titles were on effectiveness or comparative study of rehabilitation therapy modalities which include therapeutic -electrophysical -agents(TEA), exercises, occupational therapy or orthotics. Musculoskeletal (MSK) and rheumatological disorders constituted (17.4%), neurological disorder 7.1%), risk factor assessment or etiological pattern of MSK conditions (7.2%), disability and functional outcome assessment (5.4%), interventions for regional pain management (4.8%) and other category (7.2%). The initial decade trend was on MSK, which later shifted to TEA and on interventions respectively. Overall, TEA modalities were the predominating titles in all 3 decades. Considering the other category of titles, there were interesting topics which include COVID-19, variation in body mass composition, assessment of drug effects, diagnostics and effectiveness of educational booklet. Of the 09 dissertations approved during the most recent year (2021), 34% were with interventional pain management. In the process of doing a dissertation, a number of opportunities and challenges were identified by the fellows for fellow trainee researchers. There was a clear trend of dissertation titles on MSK pain management and interventions with less attention to rehabilitation procedure and appliances. Identified opportunities include cooperative attitude of the trainers and mentors in the department and there were wide research areas with vast sample population. Time allocation and funding were the biggest barriers for research works during training period. After birth, 25% of craniopagus twins survive and can be considered for surgical separation. It is a complex multistage, multidisciplinary team involvement exercise needing detailed evaluation, planning and preparation beforehand. A harmonious collaboration from the clinical, administrative as well as logistic point of view is mandatory for successful management of such complex condition. The honorable prime minister of Bangladesh picked up “Rabeya and Rukaiya”, a craniopagus twins, the first ever case in the history of Bangladesh and handed them over to an expert team from Hungary and Bangladesh for successful management. The noble joint exercise known as “Operation Freedom” took place in home and abroad through multiple procedures/interventions in different institutions by a group of experienced panels of medical and non-medical professionals.
