Jordan","doi":"10.3844/IJRNSP.2019.1.9","DOIUrl":"https://doi.org/10.3844/IJRNSP.2019.1.9","url":null,"abstract":"The Hamilton Anxiety Rating Scale is an internationally used assessment tool for anxiety level however its acceptable factor structure remains fluid. The aim of this paper is to examine the psychometric properties and establish normative anxiety prevalence rates with a Persian randomized sample. A cross sectional survey was distributed to a randomised sample of adults (n = 1260) in the Middle East. A two-factor model was deemed the most statistically and theoretically appropriate measurement model. This represented a split between cognitive and physiological symptoms. Overall, 15.2% of respondents reported at least mild anxiety, with 2.3% experiencing severe anxiety. Lower anxiety scores on the scale were reported among males, the married, younger and higher educated people. The Hamilton Anxiety Scale continues to be a useful tool in the measurement of anxiety. The two factor model separates anxiety into two dimensions, cognitive and physiological aspects and both are highly correlated with each other suggesting a single overarching construct of anxiety. The two-factor model may help provide a better understanding of the effectiveness of different therapeutic techniques. Negative media reports and the stigma associated with long-term care perpetuates the belief that community care is associated with a better quality of life than nursing or residential home care. The aim of this paper is to investigate mental health, quality of life and medication use among a sample of care home residents and community dwelling older people. This cross-sectional study comprised of structured interviews with 75 care home residents and 852 community dwelling older adults in a large health and social care trust within the U.K. Data were collected using an abridged version of the World Mental Health-Composite International Diagnostic Interview (WMH-CIDI). Health-related quality of life was assessed using the SF-6D, derived from responses to the SF-12, which was embedded within the screening section of the WMH-CIDI. Results showed a significantly lower level of mental disorders among older people in the care home sample. Health-related quality of life results indicated that although care home participants were significantly more limited in their physical functioning, they reported better mental health and social functioning than older people living in the community. These results were statistically significant. Prescription rates for mental health medication were higher in the care home sample. This study provides tentative findings about mental health and quality of life in care homes, which should be further explored in future research. The study addresses the negative public perception of quality of life in care homes while contributing to the debate around the health care implications of ‘ageing in place’ for older people. Additionally, many pain-associated diseases are accompanied by the concomitant decline in cognitive and motor performance. The complex interplay and balance between diverse signal transduction mediators, genetic background and environmental factors may ultimately determine the outcome of nociceptive progression in various disorders. Pain is a subjective feeling that is difficult to standardize and parameterize in a traditional fashion for scientific analysis. The causes of visceral pain are often not very clear, as there are many symptoms of the reproductive, gastrointestinal, musculoskeletal, neurological, psychological systems and urinary tract that often cooccur in the same patient. VIscero-somatic and visceroviseral hyperalgesia and allodynia Cause the perception of pain to spread for an initial area, to adjacent visceral sites (Pan and Malykhina, 2014). Often times, there is not a clear relationship between the severity. There is often no clear relationship between the severity of the visceral pain and pathology in the viscera, including the reproductive tract, urinary bladder and colon. The clinician treating this pain is often times tempted to adopt a unidimensional approach, focusing on one organ system, ignoring the psychological and behavioral manifestations of the visceral pain. Therefore, studies of the nervous system in individuals with visceral pain associated with reproduction such as Chronic Pelvic Pain (CPP) syndrome, urinary system such as Painful Bladder Syndrome (PBS) and bowel disorders such as Irritable Bowel Syndrome (IBS), suggest a model in which alteration in the central stress circuits in predisposed individuals may trigger and then maintain, the pain and pathophysiological changes in the viscera (Mayer, 2011). These patients have significantly more depression, psychological and somatic complains and more often give a history of physical, sexual or emotional abuse, or trauma. Chronic visceral pain results in adverse affects not only one’s mood, but also their professional and social lives, as well as general well-being;; the quality of life issues can affect the severity of pain, degree of impairment resulting from a painful condition and success of treatment modalities in alleviating pain. \u0000 \u0000Pain accounts for a majority of all primary health care visits. For the past decade, medical literature has carefully documented the under-treatment of all types of pain by physicians. Pain is a complex and individual experience that is often difficult for patients to fully describe using a conventional clinical assessment (Meltzak, 2001). This is especially true for federally funded exploratory research centers, which tend to be more complex due to the involvement of multiple, interdisciplinary investigators. This study provides an overview of the approach used to develop an evaluation strategy and reports the lessons learned during the initial development of the Center for Ohana Self-Management of Chronic Illness (COSMCI) at the University of Hawai'i at Mānoa School of Nursing and Dental Hygiene. The COSMCI is composed of an interdisciplinary team of researchers and practitioners and aims to advance knowledge in the field of self management of chronic disease in the community setting. APPROACH: A systematic approach was utilized that included formative and summative strategies for ongoing evaluation. The problem was solved by addressing five key concerns: (1) development of research structure, (2) observing the process of the research pilot projects, (3) scholarly activity of COSMCI faculty, (4) dissemination and translation and (5) sustainability prospects. The method of research included formulating process strategies and determine if the plans for developing the Center were followed and whether these plans were effective. Interviews were also conducted at year one and at mid-point though the project. RESULTS: Themes that emerged from our evaluation included inclusion, timelines, realistic expectations, ongoing evaluation and preparing for changes in the team. This provided timely recognition of successes and challenges and facilitated a rapid response for interventions especially during the early development stage of the center. CONCLUSION/RECOMMENDATIONS: Effective development of a successful Center is highly dependent upon having a strong evaluation process in place that can inform ongoing development.
