Physical activity improves health, decreases the incidence of several diseases and may influence weight loss outcome after bariatric surgery. Knowledge regarding how patients experience physical activity after bariatric surgery could provide us with better tools to enhance physically active. Methods: The aim of the present study was to describe how patients experience physical activity one year after bariatric surgery. An inductive qualitative content analysis was used to analyse semi-structured interviews with twenty-four patients one year after bariatric surgery. Results: The main findings emerged in four overarching themes: \"Hindrances of physical activity\", \"Physical activity with a less obese body implies achievement\", \"Coming to an understanding of the benefits of physical activity\" and \"Need of support\". Conclusion: According to our findings patients experience both achievements and hinders with regard to physical activity one year after surgery. The aim of our study is to explore serum myostatin levels in overweight patients and its association with metabolic-related characteristics. Methods: 40 overweight patients and 40 normal weight controls were recruited, and serum myostatin were measured by ELISA methods and the relationships between myostatin and metabolic-related parameters were investigated. Results: Serum myostatin concentrations were significantly increased in overweight patients compared with normal weight controls (10.99±1.99 vs. 9.75±0.96 ng/ml, P=0.001) and positively correlated with body weight (r = 0.272, P=0.015), BMI (r = 0.263, P=0.018), waist circumference (r = 0.291, P=0.009), hip circumference (r = 0.336, P=0.002) and TNF-� (r = 0.611, P<0.001) in all subjects. Contrarily, serum adiponectin levels were negatively associated with body weight (r = -0.270, P=0.018), waist-to-hip ratio (WHR) (r = -0.253, P=0.027), TG (r = -0.235, P=0.040), ALT (r = -0.268, P=0.019), uric acid (UA) (r = -0.262, P=0.021) and positively correlated with HDL (r = 0.454, P<0.001). Adult subjects aged 18-60 yrs scheduled to undergo CT examination of the abdominal region were recruited for this study, stratified across BMI categories. Liver volume, fat content, and lean liver volume were determined using CT methods. A total of 100 subjects were recruited, including 30 normal weight, 31 overweight, and 39 obese. Total liver volume increased with BMI, with mean values of 1138 ± 277, 1374 ± 331, and 1766 ± 389 cm 3 for the normal, overweight, and obese, respectively (P < 0.001), which was due to an increase in both liver fat content and lean liver volume with BMI. Some obese subjects had no or minimal hepatic fat content. The prevalence of mild fatty liver in this study of 100 subjects was overestimated for all BMI categories using a range of qualitative diagnostic measures, with predicted prevalence of fatty liver in obese subjects ranging from 76.9% for liver-to-spleen ratio � 1.1 to 89.7% for liver attenuation index (liver HU - spleen HU) � 40, compared to 66.7% by quantification of fat content. Results show that total liver volume increases with BMI, however, not all obese subjects display fatty infiltration of the liver. {"title":"Effectiveness of Financial Incentives in a Worksite Diabetes Prevention Program.","authors":"Pouran D Faghri, Rui Li","doi":"10.2174/1876823720140107001","DOIUrl":"10.2174/1876823720140107001","url":null,"abstract":"<p><strong>Purpose: </strong>To evaluate the effect of financial incentive in a diabetes prevention weight loss program at worksites.</p><p><strong>Design: </strong>Group-level randomized intervention study.</p><p><strong>Setting: </strong>Four long-term care facilities, randomly assigned to \"incentive-IG\" or \"non incentive-NIG\" groups.</p><p><strong>Participants: </strong>Ninety-nine employees, all overweight or obese (BMI= mean 34.8±7.4 kg/m<sup>2</sup>) and at risk for type 2 diabetes.</p><p><strong>Intervention: </strong>A 16 week weight loss program (diabetes prevention program) with a 3 month follow up. IG could either choose a \"standard incentive\" to receive cash award when achieving the projected weight loss or to participate in a \"standard plus deposit incentive\" to get additional money matched with their deposit for projected weight loss. All of the participants received a one-hour consultation for a healthy weight loss at the beginning.</p><p><strong>Measures: </strong>Weight-loss, diabetes risk score (DRS), and cardiovascular risk outcomes.</p><p><strong>Analyses: </strong>Linear and logistic regressions for completed cases with adjustments for clustering effect at group level.</p><p><strong>Results: </strong>IG lost on average more pounds (p=0.027), reduced BMI (p=0.04), and reduced in DRS (p=0.011) compared to NIG at week 16. Using the 2008 Ghana Demographic and Health Survey data, this study explored the association between biological, behavioural and socio-cultural factors and overweight and obesity among women in Ghana. The bivariate descriptive analysis and the binary logistic regression estimation technique were used to analyse the data. From the results, it was observed that there were significant associations between some of the factors and overweight and obesity. For instance, age was found to have a positive significant association with overweight and obesity for all categories while having four or more children was significant but negatively associated with overweight and obesity. The introduction of socio-cultural factors in the model did not affect the direction of association between age and overweight and obesity even though the strength of association reduced. Amongst the socio-cultural factors introduced in the model, all categories of education and wealth status had a positive significant association with overweight and obesity. The introduction of behavioural factors in the model did not change the nature of predictors of overweight and obesity. Alcohol consumption, contraceptive use and exposure to television were positively associated with overweight and obesity. Chemerin is a novel adipokine that has been activated by the coagulation process and suggested to play an important role in the pathogenesis of metabolic syndrome. We aimed to identify the possible relationship between chemerin levels and haemostatic changes in the high fat diet (HFD) fed rats. A total number of 20 adult male albino rats were divided into two groups of 10 rats each. Group I, rats served as controls; Group II: rats received high fat diet (58% fat) for 8 weeks. In both groups, serum levels of glucose, insulin, chemerin, total cholesterol (TC), triglycerides (TG), HDL, LDL were measured. As well as the bleeding time (BT), whole blood clotting time (WBCT), Prothrombin time (PT), Activated partial thromboplastin time (aPTT), fibrinogen level and C- reactive protein (CRP) were measured. Moreover HOMA-IR was calculated for both groups. The results of the current study revealed that chemerine level increased significantly in the HFD- fed rats (p< 0.001). In addition, a significant positive correlation was detected for the serum chemerin levels with body weight, insulin levels, HOMA-IR, the levels of TC, TG, LDL and CRP, while a significant negative correlation was found between its levels and serum levels of HDL. Moreover chemerin was correlated negatively with the BT, WBCT, PT, aPTT and positively correlated with plasma fibrinogen, while there was insignificant correlation with platelet count. The purpose of this study was to determine if a modified postoperative analgesic regimen and a dedicated postoperative bariatric team reduced pain scores, length of stay and postoperative complications. Materials and Methods: We performed a retrospective analysis of morbidly obese patients admitted to our medical center for laparoscopic Roux-en-Y gastric bypass (LRYGB) and laparoscopic adjustable gastric banding (LAGB). Our previous postoperative pain regimen was ketorolac (30 mg IV plus 15 mg q6h) and patient controlled analgesia (PCA) morphine and was converted to ketorolac (30 mg IV and 30 mg i.m, plus 15 mg q6h), and IV PCA hydromorphone. Visual analog scale (VAS) pain scores from the post-operative care unit were collected retrospectively. The bariatric team was led by a nurse practitioner consisted of a psychologist, exercise physiologist, and nutritionist. Results: Eighty-five patients underwent bariatric operations in the year prior to implementation of the revised postoperative pain management regimen and 372 patients underwent bariatric surgery in the 2 years following implementation. Patient age, gender and BMI were evenly distributed for both groups. Mean VAS scores on postoperative days 1 through 5 were significantly lower after implanting our modified pain regimen (p<0.0001). Pain scores were significantly higher for patients that underwent LRYGB compared to LAGB (p<0.0001). Overall, length of hospital stay was unaffected by the new pain regimen, however a significant reduction was found in patients that underwent LAGB (0.8 days less; p=0.0001). It has been recognized to be a complication of bariatric surgery. A high BMI and a prolonged operative time are the main risk factors associated to the development of RML. The aim of this study is to define the incidence and the main features of RML in a cohort of obese patients undergoing bariatric surgery. Materials and Methods: a retrospective observational analysis was carried out on 100 patients undergone bariatric surgery. The bariatric operations were open or laparoscopic Roux-en-Y gastric bypass (RYGB) and laparoscopic sleeve gastrectomy (SG); they were performed at the university hospital Nuovo S. Chiara in 2011. Results: 6 of 100 patients enrolled developed RML. Three of these also showed acute kidney injury due to RML (50%). A significant correlation between post-operative increased creatine phosphokinase (CPK) levels and BMI was found (r=0.369; r 2 =0,137; p=0.005) as well as a significant correlation between the increased levels of creatine phosphokinase, myoglobin and operative time (increased operative time - highest CPK: r=0.550; r 2 =0.302; p<0.0001; increased operative time - highest myoglobin: r=0.553; r 2 =0.305; p<0.0001). Moreover, hypertension and prolonged operative time were found to be variables associated with RML in bivariate analysis. Conclusions: The incidence of RML following bariatric surgery was 6%. Since rhabdomyolysis is the most important complication of bariatric surgery, appropriate precautions should be taken during surgery. Besides the genetic aspects, which represent one of the motivating factors in the progress of obesity, the shift in the energetic balance also plays a significant role in the development of obesity. TV viewing and the high-calorie snack foods consumption have come under more scrutiny as potential causes of increased obesity in children and adolescents. The aim of the present work was to develop an instrument that allows to estimate the Chilean children's brand awareness. We developed the IBAI (International Brand Awareness Instrument), an age-appropriate instrument that uses twelve sheets with pictures of food logos to test children's recall and recognition. The IBAI was presented to a sample of 80 children aged from 3 to 10 years, enrolled in the city of Santiago de Chile. The instrument allowed distinguishing different kinds of children's brand awareness. Considering the total score reached by each child, the majority of the children for both genders showed a medium-low brand awareness (32.5%) followed by medium-high brand awareness (31.3%), high brand awareness (23.8%) and low brand awareness (12.5%).
