In an effort to redesign delivery of health care to priority populations, University Health System, a major safety net hospital system located in South Central Texas, opened the first school-based health center in Bexar County, Texas. This article describes the planning process with emphasis on the community assessment as part of a strategy to promote community engagement and guide development of health services. Wodika","doi":"10.47779/ajhs.2014.217","DOIUrl":"https://doi.org/10.47779/ajhs.2014.217","url":null,"abstract":"The purpose of the study was to determine if disparities existed in the non-emergency medical transportation (NEMT) system in the lower 34 counties of southern Illinois. NEMT is described as the transfer of individuals from a pre-hospital care setting to care facilities, and ongoing care transfer of individuals from pre-hospital care settings. Several research themes developed during the study. These themes included an underserved rural population by non-emergency medical transportation, negative health consequences, and an ineffective use of medical staff’s time. Two types of data collection were used in the collection of information about the non-urgent medical transportation system. Qualitative research consisted of three focus groups, made-up of various stakeholders within the medical community. The quantitative research consisted of the collection of 250 surveys from various participating healthcare professionals. The results of the data collected indicated that the rural population of southern Illinois is inadequately served by the NEMT system. The data were collected from a sample of 333 college students using a self-administered online survey tool. The questionnaire consisted of multi-item scales measuring parental attachment, life satisfaction, and social tanning behaviors (tanning bed use vs. sun tanning) along with demographic questions. The majority of the participants were female (80.2%) and Caucasian (76.9%), with an average age of 19.8 years. Results suggest that there is an association between having a high parental attachment and engaging in “social sun tanning.” College students who are highly satisfied with their life are less likely to use tanning beds with friends and families. We identified non-medical use of prescription drugs (NMUPD) as a main area of concern. All participants were asked to complete three standardized assessments: the International Physical Activity Questionnaire, the Schutte Self-Report Emotional Intelligence Test, and the Kessler Psychological Distress Scale. Results showed that participants who reported higher levels of PA scored significantly higher in Global EI (p = 0.031) and mental health (p = 0.049) compared to participants with moderate and low levels of PA. A total of 238 African-American women completed a 50-item valid and reliable questionnaire assessing their knowledge, expectations, self-efficacy, self-efficacy in overcoming barriers and self-control in initiation and duration of breastfeeding. Regression results indicated that self-efficacy for initiation and self-efficacy in overcoming barriers for initiation were significant predictors for initiation of breastfeeding in African-American women. Those in positions of privilege may have greater access to quality care, even in light of the ACA. A nutrition education session was provided once a week for eight weeks at a church, with sessions composed of lecture, discussion, and hands-on activities. Matched pre- and post-program survey data for 38 women showed that participants decreased negative dietary behaviors (emotional eating, snacking on sweets, haphazard meal planning, meal skipping, cultural factors) and increased low-fat eating(p < .01). Knehans","doi":"10.47779/ajhs.2015.178","DOIUrl":"https://doi.org/10.47779/ajhs.2015.178","url":null,"abstract":"The purpose of this study was to assess the association between maternal participation in the Supplemental Nutrition Assistance Program (SNAP) and the use of food labels to purchase various types of beverages, and whether home availability of beverages differed by SNAP participation. Low-income mothers (n=250) of preschool children were surveyed. Food label use to purchase beverages and home availability of beverages were analyzed using adjusted multivariate logistic regression and iteratively reweighted with least squares regression.  SNAP participants were approximately 50% less likely to use the food label to purchase beverages for the home than non-participants. SNAP homes also had significantly higher availability of fruit juice (192 oz vs. 128 oz), juice drinks (192 oz vs. 128 oz), regular soda (156 oz vs. 135 oz), and low-calorie beverages (469 oz vs. 243 oz) than non-SNAP homes (p < 0.05). However, most primary care systems that integrate physical and behavioral health struggle with financial start-up and sustainability. The growth and development of mechanisms for supporting grass roots integration highlight the creativity and sheer will of communities, states, and healthcare systems to innovate their way to health. And now, for the first time, national-level funding initiatives have begun to support integration. This article highlights funding mechanisms at the state, foundation, payor, and federal levels that have been used to champion innovation.
