Besides acting as building blocks during tissue synthesis, amino acids or proteins act specifically by upregulating defense systems or by stimulating key sites in metabolic pathways. Following premature birth, the neonatologist is responsible for delivering the right amount and quality of nutrients to the neonate, while exact requirements are largely unknown. However, nutrition matters, both in quantity as well in quality, especially during the first few weeks and months of life. Cancer results from multiple genetic and epigenetic events over time, so demonstrating a specific effect of nutrients or other bioactive food components in human cancer is challenging. Epidemiological evidence consistently suggests that milk intake is protective against colorectal cancer. Calcium supplements have been shown to reduce risk for recurrence of adenomatous polyps. Calcium supplementation has not been observed to reduce risk for colon cancer, although long latency and baseline calcium intake affect interpretation of these results. High calcium intake from both food and supplements is associated with increased risk for advanced or fatal prostate cancer. Results from epidemiological studies examining the relationship between intake of dairy foods and breast or ovarian cancer risk are not consistent. Animal studies have suggested that galactose may be toxic to ovarian cells, but results from epidemiological studies that have examined ovarian cancer risk and milk and/or lactose intakes are mixed. Dietary guidelines for cancer prevention encourage meeting recommended levels of calcium intake primarily through food choices rather than supplements, and choosing low-fat or nonfat dairy foods.</p>","PeriodicalId":87412,"journal":{"name":"Nestle Nutrition workshop series. There has been a striking reduction in dental caries and periodontitis in industrialized countries, although the proportion with severe disease has remained at 10-15%, and the prevalence increases in less developed countries. If left untreated, these diseases may lead to pain, and impaired quality of life and nutritional status. Prevention and treatment need, besides traditional implementation of proper oral hygiene, sugar restriction and use of fluoride, newer cost-effective strategies. Non-sweetened dairy products, which are proven non-cariogenic, or specific bioactive components from alike sources might prove to be part of such strategies. Thus, milk proteins, such as bovine and human caseins and lactoferrin, inhibit initial attachment of cariogenic mutans streptococci to hydroxyapatite coated with saliva or purified saliva host ligands. In contrast, both bovine and human milk coated on hydroxyapatite promotes attachment of commensal Actinomyces naeslundii and other streptococci in vitro, and phosphorylated milk-derived peptides promote maintenance of tooth minerals, as shown for the β-casein-derived caseino-phosphate peptide. Observational studies are promising, but randomized clinical trials are needed to reveal if dairy products could be a complementary treatment for oral health.</p>","PeriodicalId":87412,"journal":{"name":"Nestle Nutrition workshop series. Rapid early childhood weight gain is associated with increased likelihood of being overweight or obese later in childhood and of having risk factors for the development of chronic disease such insulin resistance and elevated blood pressure. This has led to concerns about the wisdom of promoting catch-up growth in infants born small for gestational age or in children with poor growth after birth. In considering the costs and benefits of promoting catch-up growth, we must not lose sight of the immediate health threats to children in resource-poor environments in developing countries where child morbidity and mortality remain high. The literature on short-term consequences of growth is limited by its focus on attained size as an indicator of prior nutritional status, but generally shows that children with evidence of poor prior growth are at greater risk of morbidity and mortality from common infectious diseases, including lower respiratory infections and diarrhea. In 2009, there are at least twelve published randomized controlled trials (RCT) assessing the effects of LCPUFA supplementation of infant formula for preterm infants and seventeen RCTs involving formula-fed term infants. In addition, at least five RCTs have investigated the effect of DHA supplementation during pregnancy and/or lactation on infant and early child development. Collectively, the published literature has demonstrated no harm of dietary LCPUFA for infants regardless of whether they are born preterm or at term. However, developmental benefit is more consistently observed in infants born preterm. This may be explained by the fact that DHA accretion to neural tissues peaks during the fetal brain growth spurt in the last trimester of pregnancy. Infants born preterm are denied the full gestation period to accumulate DHA and are at risk of incomplete DHA accumulation. First, it clarifies the methods that were used to create the charts as it has become apparent that the smoothing techniques have been somewhat misunderstood. The techniques included smoothing-selected percentiles between and including the 3rd and 97th percentiles and then approximating these smoothed curves using a procedure to provide the transformation parameters, lambda, mu, and sigma. Only the selected percentiles were used in this process due to small sample sizes beyond these percentiles. Second, given the concern that the infant charts were created with relatively few data points in the first few months of life, it compares the original observed percentiles with percentiles that include newly available US national data for the first few months of life. Third, it discusses the issues that arise if a 99th percentile is extrapolated based on the lambda, mu, and sigma parameters. The 99th percentile of the body mass index-for-age chart has been recommended to identify extremely obese children, yet the 97th percentile is the highest available percentile on the Centers for Disease Control and Prevention growth charts.</p>","PeriodicalId":87412,"journal":{"name":"Nestle Nutrition workshop series. Recent findings have expanded our knowledge in this area. Several growth factors and cytokines are present in breast milk and their capacity to persist in the infant gut and exert their activities is likely to affect maturation of immune function, possibly affecting the development of oral tolerance. A proper balance of polyunsaturated fatty acids (n-3/n-6 ratio) may also be of significance for allergy prevention in children, emphasizing the need for the mother to achieve a balance of these fatty acids in her diet. The recent findings that specific strains of bacteria are present in breast milk and act as probiotics in the early colonization of the infant gut and that human milk oligosaccharides are specific substrates for these probiotic strains may not only affect the defense against pathogens, but also affect energy utilization and development of obesity. Previously neglected milk fat globule membranes contain several components involved in protection against infection and may be an additional arm in the multifaceted shield that breastfed infants have developed against bacterial and viral antagonists. While nutrition science continues to define the composition and functionality of human milk as a reference, food ingredient, formulation and processing technologies facilitate the design and delivery of nutritional and functional concepts to infant products. Expanding knowledge of both nutritive and non-nutritive components of human milk and their functionality guides selection and development of novel ingredient, formulation and processing methods to generate enhanced infant products targeting benefits including healthy growth, development as well as protection of health through the life cycle. In this chapter, identification and application of select novel ingredients/technologies will be discussed in the context of how these technological advancements have stimulated innovation in infant foods. Pediatric formulae have been designed for different age groups, and for children with certain diseases; examples are special formulations for regurgitating infants, metabolic diseases, cow's milk or multiple food allergies, intestinal, pancreatic, renal, and hepatic insufficiency. Exclusive enteral nutrition is a therapeutic concept to induce remission in children and adolescents with active Crohn's disease. A new area of nutritional research in pediatrics is potential immunonutrition in critically ill children. Formulae are enriched with single components or a combination of key substrates that might play a crucial role during intermediary metabolism in sepsis, inflammation, tissue healing, and growth. For pharmaconutrition, single components are investigated in a scientific stepwise procedure in order to identify effective disease-dedicated nutrition therapy. Any new formula needs to be evaluated, if possible in comparison to a normal diet or the reference formulation to demonstrate its safety and efficacy (equal or superior to standard formula).</p>","PeriodicalId":87412,"journal":{"name":"Nestle Nutrition workshop series. Animal experiments suggest that the early postnatal period may be critical to development of healthful energy homeostasis and thus prevention of obesity. In humans, observational studies and follow-up of randomized feeding trials show that rapid weight gain in the first half of infancy predicts later obesity and higher blood pressure. Despite the mounting consistency of results, several questions remain to be answered before clinical or public health implications are clear. These include the need for body composition data in infancy and data from the developing world to identify modifiable determinants of gain in adiposity in the early weeks of life, to mount interventions to modify these determinants, to examine tradeoffs of more vs. less rapid weight gain for different outcomes, and to incorporate any interventions that prove to be efficacious into clinical and public health practice in a cost-effective manner.</p>","PeriodicalId":87412,"journal":{"name":"Nestle Nutrition workshop series.
