Mohammad","doi":"10.3329/bjn.v30i2.57389","DOIUrl":"https://doi.org/10.3329/bjn.v30i2.57389","url":null,"abstract":"Methodology: This retrospective cross sectional observational study was carried out in the Department of Neurology and Department of Medicine of Dhaka Medical College Hospital. (DMCH), during March, 2010 to February, 2011 in patients admitted with a history of first ever stroke. The duration of the study was one year .For this purpose, a total number of 140 patients, of which 70 were included in ischemic stroke and 70 in intracerebral haemorrhage group. CT scan of head of each patient was done at least 6 hours after the onset of the event .CT diagnosed cases of infarct and ICH were included in this study and subarachnoid haemorrhage cases were excluded. CT negative cases were further investigated by MRI brain when the clinical suspicions of stroke were strong. \u0000Results: The mean age of the patients having features of ischemic stroke and intracerebral haemorrhage were 59.81±11.08 and 57.21±10.09 respectively. Male female ratio was 1.92:1 and 1.69:1 in IS group and ICH group respectively. Regarding the risk factor hypertension and smoking were observed most common risk factors in the study patients in both groups. However, diabetes mellitus, arrhythmia, ischemic heart disease and dyslipidemia were observed more common in patients of ischemic stroke than ICH patients. Among the studied patients, ischemic stroke was observed more common in parietal region (20% of patients), internal capsule/ capsular region (17.14% of patients) and caudate nucleus region (10% of patients). ICH was observed most frequently 32.86% (n=23), 15.71% (n=11) and 5.71% (n=4) in lentiform nucleus/putamen, thalamus and combined gangliothalamic region respectively. In both group left sided lesion prevailed more than right side. In case of ischemic strokes, left side lesion was 61.43% .In intracerebral haemorrhage, left side lesion was 58.57%. It was also observed that deep intracerebral haemorrhage is the most common location (74.29%) than the lobar region (25.71%). \u0000Conclusion: Site of predilection of lesions and their distribution pattern differ in ischemic stroke and intracerebral haemorrhage. This study revealed that infarcts were more common in parietal and capsular region and haemorrhage were more common in putamen, thalamus and combined gangliothalamic regions. It is difficult to approach for its complication and vital structures. \u0000Objective: To see the outcome of surgery of cerebellopontine angle tumors at the Bangabandhu Sheikh Mujib Medical University. \u0000Results: Among the 24 patients 16(66.66%) were males. It was evident that 14(58.33%) belonged to the age group 41-60 years. It was documented that the majority 19(79.16%) of patients had vestibular schwannoma. Among the clinical signs and symptoms the highest group had hearing loss 20(80%). Among 24 patients the tumor was removed completely in 15(62.50%). Among the post operative complications 4(16.66%) developed facial palsy. 2(8.33%) of patients had lower cranial nerve palsy. After operation 1(4.16%) patient died due to aspiration pneumonia. \u0000Conclusion: Cerebellopontine angle tumor surgery is a very difficulty operation. The number of people with Alzheimer’s Disease and other dementias is increasing every year because of the steady growth in the older population and stable increment in life expectancy and it is expected to increase two-fold by 2030 and three-fold by 2050.In addition to Alzheimer’s disease there are so many reversible and irreversible causes of dementia. This study was aimed to explore the different etiological factors related to dementia patients. Risk factors for dementia, co-morbid conditions were also included. \u0000Methods: This cross sectional study was carried out from 2009 to 2014 at dementia clinic (OPD), department of Neurology, Bangabandhu Sheikh Mujib Medical University (BSMMU). A total number of 166 dementia patients, as diagnosed by Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) and confirmed by Mini Mental State Examination(MMSE) score were recruited in this study. Diagnosis of specific type of dementia was made on the basis of established criteria. \u0000Results: Alzheimer’s disease(32.5%) and Vascular dementia(31.9%) were the most common etiological factor followed by Mixed dementia(19.9%), PD with dementia(8.4%) and others(7.2%) like hypothyroidism, head injury, epilepsy etc. Increasing age, hypertension, diabetes mellitus, dyslipidemia, IHD, smoking are potential risk factors for dementia. \u0000Conclusion: This study concludes Alzheimer’s disease and Vascular dementia are almost equally occurring dementia. We report a case of Gradenigo’s syndrome, which was 50 yrs old man who presented with 6 months history of left-sided headache, facial pain, diplopia and dropping of left eyelid. Examination demonstrated a left eye lateral gaze palsy, diplopia, and dropping of left eyelid, otoscopy revealed a congested left tympanic membrane. X-ray mastoid Townes view shows mastoid air cell are reduced on left side. The association between severity of carotid atherosclerosis and high factor viii activity in ischemic stroke is still not clear. \u0000Objective: The present study was conducted to find out the association between carotid atherosclerosis and high factor viii activity in ischemic stroke. \u0000Methodology: This was a cross sectional analytical study carried out in the department of neurology,BSMMU during the period of july’2009 to june’2011.A total 60 subjects with ischemic stroke were included in this study and data were collected purposively. Chi square test was done and probability value <0.05 were considered as level of significance and 95% confidence limit were taken. \u0000Result: In patients with e”50% carotid stenosis, 7(23.33%) had high factor VIII activity and 23(76.67%) had normal factor VIII activity. In patients with <50% carotid stenosis, 2(6.67%) had high factor VIII activity and 28(93.33%) had normal factor VIII activity. Need of caregivers training to address the discharged patients for their home care management is often overlooked. Early discharge, lack of post-acute management facilities including home or community based services are other important predictors of poor functional outcome. Most of the patients are discharged home directly from hospital neurology unit after acute care management for inability to pay hospital costs. Rich patients who reside in the city can afford to receive treatment from trained nurse or physiotherapist at home but poor patients go home without any preparation for home care management of the patients. Hospital based rehabilitation management of stroke survivors are scarcely available in our country but thousands of inadequately treated patients are discharged home without any training to the caregivers. They come back to the outpatients department for next visit with multiple complications. ECG changes are common in patients with ischemic stroke. Objective: The objective of this study was to see ECG findings among ischemic stroke patients. \u0000Materials and Methods: This study was carried out in the Department of Neurology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka from July 2006 to October 2008. A total of 36 patients with acute Ischemic stroke were selected by purposive sampling method and diagnosed by history, clinical findings ard was confirmed by CT scan of head. The clinical details, investigations of the respondents were reviewed. Data were recorded in a pre-designed data collection sheet. \u0000Result: Majority of the subjects were in 7th decade 12(33.3%) and 6th decade 9(25%) with the male to female ratio was 1.25:1. MPS cases have been reported throughout the world. MPS patient typically appear normal at birth, but clinical features appear between two to four years of age. We report a case of 12-years-old boy presented with progressive deformity of multiple joints for eight years duration and gradual decline the cognitive function for the same period. On examination, his head was large, short stature, a coarse facial feature with depressed nasal bridge and stubby finger with flexion of distal interphalangeal joint. There was severe mental retardation. We diagnose the patients as Hunter Syndrome, on the basis of clinical findings, radiological features and positive for MPS screening test in urine. Although the golden standard for diagnosing the type of MPS is enzyme analysis. Rana","doi":"10.3329/bjn.v30i1.57363","DOIUrl":"https://doi.org/10.3329/bjn.v30i1.57363","url":null,"abstract":"Objectives: To evaluate the association of dementia in ischemic stroke. \u0000Methodology: This case control study was carried out in the department of Neurology at BSMMU, Dhaka from 1st January 2010 to 31st December 2011 for duration of two years to evaluate the association of dementia in ischemic stroke. The target population for this study include all patients presented with ischemic stroke at the range of 3 to 6 months after stroke with the age group of 40 to 70 years are included in this study and patients of dementia other than ischemic stroke like Alzhiemer’s disease, vit-B12 deficiency, thyroid dysfunction were excluded from this study. A total number of 120 respondents were included in this study. Age & sex matched 60 patients of ischemic stroke were selected as cases and rest 60 people were taken as control group. Informed written consent was taken from each patient or his/ her attendant. All information regarding history and physical findings; and other risk factors for dementia were collected to fill up the preformed questionnaire. Relevant physical examinations like nervous system examination, selected general and systemic examination were recorded. \u0000Result & Observation: Dementia was present in case and control group 18(30.0%) and 2(3.3%) respectively. The difference was statistically significant (p=0.001). Present smoking habit was more in case (45.0%) than in control group (16.7%) which was statistically significant (p=0.001) with a OR of 4.07 with a 95% CI of 1.89-8.75. Past smoking habit was more in case (16.7%) than control group (11.7%). Non-smoker was more in control (71.7%) than case group (38.3%). Diabetes mellitus was more common in case group (38.3%) than control group 5(8.3%) which was statistically significant (p=0.001) with a 6.84 OR and 95% CI of 2.39-19.6. \u0000Conclusion: The study permit to conclude that dementia is directly associated with ischemic stroke.
