They were divided into 4 equal homogenous groups. Study groups (A, B, and C) received 3 successive injections of BT-A at 3 to 4 months interval. The injection sites were as follows: gastrocnemius for group A; adductors for group B; gastrocnemius and adductors for group C. Group D received no injections and was considered as a control group. Muscle tone and gait evaluation were conducted for every child in the BT-A groups at his entry and 6 months after the last injection dose. Also, the same evaluation protocol was conducted for every child in the control group at his entry and after 6 month's period.

Results: When BT-A groups were compared to the control group, results showed significant improvement in muscle tone following injection in all BT-A groups. A statistically significant increase of the unaffected step length and decrease of step width and foot angle was noted in gastrocnemius group (P=0.04, P=0.003, P=0.002) respectively, as well as in gastrocnemius/adductors group (P=0.006, P=0.002, P=0.003), respectively. However, the cadence and speed of walking showed significant increase in the gastrocnemius/adductors group; only P=0.03, P=0.004 respectively. Comparing between BT-A groups, there was significant improvement in all parameters between adductors and gastrocnemius/adductors groups in favor of the last group. Moreover, there was significant improvement in muscle tone (P=0, cadence P=0.04), and speed of walking P=0.01 between gastrocnemius and gastrocnemius/adductors groups in favor of the last group; however, there was no significant difference between these 2 groups in respect to the step width P=0.96 and the foot angle P=0.92.

Conclusions: Our findings indicate that, in spastic hemiplegic CP, botulinum toxin has a better effect on tone reduction and improvement of gait parameters when it is given at the gastrocnemius and adductors muscles collectively.
","PeriodicalId":87180,"journal":{"name":"Europa medicophysica","volume":"43 1","pages":"13-20"},"PeriodicalIF":0.0,"publicationDate":"2007-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26292894","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

While there are many clinical studies concerning the efficacy of surgical treatment, there are few regarding conservative treatment. It has not yet been established whether or not conservative treatment is effective and what the best treatment is. The aims of this study were to evaluate the efficacy of conservative treatment in TOS with particular reference to physiotherapy, orthotics, and taping and to make general recommendations for conservative treatment. The literature was reviewed. Medical databases consulted: Medline, Embase, CINAHL, Current Awareness, Pedro, Cochrane Library, Medscape. We used the following key words: thoracic outlet syndrome, double crush syndrome, entrapment, conservative, treatment, rehabilitation, and management. Languages of the articles reviewed: English, French, German, Spanish, Italian, and Portuguese. This analysis focussed on 10 studies of conservative treatment and 3 studies comparing the outcomes of conservative and surgical treatment, published from 1983 to 2001. This review found no randomised controlled trials, systematic reviews, or meta-analyses. Conservative treatment seems to be effective at reducing symptoms, improving function, and facilitating return to work, also when compared to surgery. We could not establish whether or not conservative treatment was better than no treatment or placebo, or what type of conservative treatment was the best.
","PeriodicalId":87180,"journal":{"name":"Europa medicophysica","volume":"43 1","pages":"55-70"},"PeriodicalIF":0.0,"publicationDate":"2007-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26239179","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Analysis was based on independent samples t-test and chi squared test for evaluating statistically significant differences between the 2 gait rehabilitation modes.

Results: The SmartStep could repeat the same results with 0.53 kg error of mean. Barthel Index (BI) was given and stump condition was assessed at admission and at discharge. Influence of age, comorbidities and stump condition on effectiveness of BI was evaluated. Influence of stump problems (pain, flexion, pain with flexion) on LCI was evaluated.

Results: At discharge, 25 patients were able to ambulate. Age and pathological conditions of stumps correlated negatively with BI effectiveness. LCI values were higher for patients with ideal stumps and lower for patients with combined stump pain and flexion deformities. Post hoc analysis showed that the principal factor negatively influencing the LCI score was the presence of stump flexion deformities.

Conclusions: In our homogeneous group of bilateral amputees, age reduced the possibility of improving the level of autonomy. The evidence base for manual therapies for neck pain consists of a reasonably large body of clinical trials, an even greater number of systematic reviews and, more recently, a number of practice guidelines. We have conducted several systematic reviews pertaining to the evidence base for both acute and chronic neck pain as well as for the outcome of control groups of chronic neck pain subjects in clinical trials of conservative therapies. In this review, we first provide background material on the definition and characterization of manual therapies as well as on the epidemiology of neck pain. We then review our recent systematic reviews on manual therapies for acute and chronic neck pain without whiplash. Finally, we provide brief, original reviews of, first, the literature on the treatment of whiplash injury by manual therapies followed by the current practice guidelines pertaining to manual therapies for neck pain. While there are several publications, especially those registered with the Cochrane Collaboration, that are currently the authoritative evaluations of the use of manual therapies for neck pain, the present review is designed to present a broad overview of the topic with a distinctive approach emphasizing the analysis of change scores in the clinical trials. It is hoped that this will benefit researchers and clinicians alike in their management of neck pain patients.
","PeriodicalId":87180,"journal":{"name":"Europa medicophysica","volume":"43 1","pages":"91-118"},"PeriodicalIF":0.0,"publicationDate":"2007-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26612157","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
