The work and education domain was left out of the analysis due to its low applicability to our subjects. A convenience sample of 100 patients with Parkinson's disease (PD) (41 male, 59 female; mean age 72+/-7 years) were consecutively observed at our Rehabilitation Clinic.

Results: According to Rasch analysis, two response categories of IPA-I were collapsed into one and 1 item (''intimate relationship'') was deleted. The remaining 24 items fitted the unidimensional construct that the scale was intended to measure. IPA-I demonstrated good reliability (person separation = 0.93, item separation = 0.97) and internal construct validity. All six IPA-II items proved to belong to the same construct. For IPA-II, item separation = 0.97, person separation = 0.71. The targeting and spread of item difficulty and the quite low person separation reliability of IPA-II made it possible only to differentiate people with low vs high level of problems in participation. Both scales showed the expected correlation with PD-specific clinical and quality of life measures.

Conclusions: IPA-I shows promise as a tool for measuring participation in people with PD. IPA-II has acceptable psychometric characteristics for measuring perceived problems in participation. Additional steps to improve their metric properties and further studies in people with different kinds of health conditions need to be carried out.
","PeriodicalId":87180,"journal":{"name":"Europa medicophysica","volume":"43 4","pages":"451-61"},"PeriodicalIF":0.0,"publicationDate":"2007-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"27121811","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In the present paper, we report 2 cases of low voltage electrical injuries with frozen shoulder development secondary to proximal humerus fracture and supraspinatus tendon tear. Magnetic resonance imaging, taken due to persistence of shoulder pain and reduction of shoulder joint mobility in cases monitored because of proximal humerus fracture associated with electrical injury, revealed a tear of supraspinatus tendon. Moreover, frozen shoulder status was clinically established in both of the cases. The cases were treated with an appropriate physical therapy and rehabilitation program. Supraspinatus tendon tears accompanying proximal humerus fractures should not be overlooked in low voltage electrical injuries and development of frozen shoulder should be avoided by applying the proper physical therapy and rehabilitation program.
","PeriodicalId":87180,"journal":{"name":"Europa medicophysica","volume":"43 4","pages":"469-73"},"PeriodicalIF":0.0,"publicationDate":"2007-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"27121813","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"Functional assessment in rehabilitative medicine: principles and methods.","authors":"L Tesio","doi":"","DOIUrl":"","url":null,"abstract":"

In rehabilitative medicine, functional assessment means a decision process that results from the interaction between classifications (for example, diagnostic) and measures, and that aims to recognize, anticipate or modify the interaction between the disabled person and his environment. In this context, the measure is the intersection of a person along the conceptual gradient and continuum ''from less to more'' attributed to variability in the ''total person,'' such as independence, pain, cognitive capacity, or fatigability. The principal instrument is the additive questionnaire (functional scale). This consists of various items believed to represent a common variable. These kinds of variables are observable only partially across various aspects of the whole person, and are not completely predictable. Their measure, consequently, can only be derived from an estimated statistic. The additive questionnaire is the principal measurement instrument. Several items all representing the same variable receive scores whose sum is assumed to be proportional to the quantity of the underlying variable. For various reasons the sum of the scores does not represent a true linear and continuous measure like those seen in chemistry and physics. Recently developed methods, in particular Rasch analysis, can extract true measures from raw scores. Thanks to better measurement of variability in persons within rehabilitative medicine, functional assessment can become increasingly more valid and informative.
","PeriodicalId":87180,"journal":{"name":"Europa medicophysica","volume":"43 4","pages":"515-23"},"PeriodicalIF":0.0,"publicationDate":"2007-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"27121819","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We offer a biopsycho-ecological model of illness and disability referred to as health environmental integration (HEI) as a framework intended to encourage a more complete understanding of disability as arising at the interface between the person's internal and external worlds. By this model, person-level functional abilities as well as the subjective meaning of those functions result in part from interactions created by the exchange of energy and information between the person and his environment. Three components are essential in measuring function: severity of deficit, type of deficit, and meaning. We present functional status staging as an approach to measuring severity and type of deficit, and recovery preference exploration as a way to measure meaning. Rehabilitation medicine can come closer to bridging the gap between biological and the subjectively-based aspects of human function, by measuring all three concepts.
","PeriodicalId":87180,"journal":{"name":"Europa medicophysica","volume":"43 4","pages":"543-9"},"PeriodicalIF":0.0,"publicationDate":"2007-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"27121683","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Similar to a rolling pin, it consists of two handles at either side of a connecting rod, the handles are bilaterally moved forward and backward, sideways, and rotated, the base plate is inclinable. A computer mouse attached to the rod enables playing games offering computer-biofeedback. Two patients, 6 and 5 weeks after a first-time supratentorial stroke, suffering from a flaccid non-functional upper extremity have been studied. Interventions performed were additional 30 min of RS-training every workday for 6 weeks; one session included 400 repetitions evenly distributed between the forward backward movement and drawing a circle clock- and counter clockwise. Afterwards the patients could play games. Upper extremity portion of the Fugl-Meyer Motor Assessment Score (FM, 0-66), and muscle strength by a Medical Research Council (MRC) sum score (0-45), the FM assessment was blinded. In the 2 patients, the FM (0-66) improved from 7 to 37, and from 17 to 43, their initial (terminal) MRC sum scores were 6 (36) and 13 (31).
