We observed the predictive value of sonological parameters for early detection of postoperative obstruction at the new pelvi-ureteric junction in children following unilateral A-H pyeloplasty.\u0000Methods: This cross-sectional study included 12 children who underwent unilateral A-H pyeloplasty. Post-operative follow-up was done after D-J stent removal and completed within six months. Maximum antero-posterior pelvic diameter (APPD), cortical thickness (CT), and pelvi-cortical (P/C) ratio were compared.\u0000Results: Mean APPD was 32.7 mm at the day after removal of the stent, 24.4 mm at one month, and 19.7 mm at four months. Mean CT was 5.2 mm at the day after removal of the stent, 6.1 mm at one month and 8.0 mm at four months. P/C ratio was 8.3 mm at the day after removal of the stent, 5.2 mm at one month, and 3.4 mm at four months. Khan","doi":"10.3329/bsmmuj.v16i1.65660","DOIUrl":"https://doi.org/10.3329/bsmmuj.v16i1.65660","url":null,"abstract":"Background: The surgical management of symptomatic adult Chiari malformation type 1 (CM1) with or without syringomyelia (SM) continues to be a dilemma considering the outcomes.\u0000Objectives: The study aimed to compare the clinical outcomes between stealth cranioplasty (SC) and one of the most commonly practiced standard procedures, posterior fossa decompression with duraplasty (PFDD). Clinical outcomes between SC and another procedure posterior fossa decompression (PFD) were also compared.\u0000Methods: This comparative cross-sectional study was carried out on 37 males and 16 females symptomatic adult CM1 patients, ranging from 18 to 47 years of age from June 2019 to May 2021. Clinical outcomes were assessed, compared, and analyzed in terms of changes in clinical symptoms and signs, chicago chiari outcome scale (CCOS) score, and occurrence of complications. \u0000Results: Of the 53 patients, 23, 19 and 11 underwent SC, PFDD, and PFD, respectively. There were no significant post-operative changes in symptoms and signs among groups except changes in limb weakness between SC and PFDD (P=0.004). Considering average CCOS score, SC performed better only than PFDD (P=0.003), while category-wise SC was better than both PFDD (P=0.004) and PFD (P=0.010). Ruhul Amin Miah","doi":"10.3329/bsmmuj.v16i1.65662","DOIUrl":"https://doi.org/10.3329/bsmmuj.v16i1.65662","url":null,"abstract":"Background: This study was carried out to evaluate the utility of multiplex real-time polymerase chain reaction (PCR) to identify the common bacterial agents of community acquired pneumonia (CAP).\u0000Methods: Sputum and blood samples were collected from 80 clinically suspected CAP patients in three tertiary-level hospitals in Dhaka city. Multiplex real-time PCR assay was carried out to simultaneously detect five common bacterial agents of CAP; Streptococcus pneumoniae, Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydophila pneumoniae, and Legionella pneumophila. Routine microbiological methods and serology were carried out. The results of PCR were compared with culture, Gram stain and serology.\u0000Results: Among the 80 patients, sputum samples of 35 (43.7%) patients were positive by PCR, of which the most commonly detected bacteria were S. pneumoniae (25/35, 71.4%), followed by H. influenzae (9/35, 25.7%) and L. pneumophila (1/35, 2.9%). All 80 sputum samples were negative for both M. pneumoniae and C. pneumoniae by PCR. Out of the 26 culture positive sputum samples, 8 (30.7%) were positive for S. pneumoniae and 1 (3.8%) was positive for H. influenzae. Among the 52 Gram stain valid sputum samples, 24 (46.1%) were S. pneumoniae and 7 (13.5%) were H. influenzae. By serology, out of the 80 cases, M. pneumoniae was detected in 32 (40%) and C. pneumoniae in 24 (30%) of cases. Angiogenesis is a significant driving force in the development of fibrogenesis in the bone marrow, which leads to myelofibrosis. The microvascular density (MVD) with immunomarker CD34 can be used to assess the degree of angiogenesis. The objective of this study was to examine the association between degree of myelofibrosis and angiogenesis in hematological malignancies.\u0000Methods: Forty-six trephine biopsy specimens of various hematological malignancies with myelofibrosis were studied at the Department of Pathology of Bangabandhu Sheikh Mujib Medical University. Extent of myelofibrosis in each case was assessed by examining the reticulin and Masson’s trichrome stained sections using a semiquantitative grading system of bone marrow fibrosis (MF) within a scale of MF-0 to MF-3. Angiogenesis was measured by counting MVD in the ‘hotspots’ after immunostaining with CD34 antibody.\u0000Results: The trephine biopsy cases were grouped into early fibrotic (MF-1) and advanced fibrotic (MF-2,3) consisting of 16 (34.8%) and 30 (65.2%) patients, respectively. The patient was 75 years old farmer, normotensive, diabetic, and presented with complaints of pain, watering, photophobia, redness and dimness of vision in the left eye. The left eye's visual acuity was perception of light and projection of rays in all four quadrants. Slit lamp examination found swollen eyelids, matted eyelashes, congested conjunctiva, seidel test positive, an inferonasal corneal perforation, shallow anterior chamber, irregular pupil, and prolapse of the iris through the perforated cornea. The patient was diagnosed with a left-sided perforated corneal ulcer (recurrent viral keratitis with stromal necrotizing variety) having iris prolapse. The perforation was sealed by cyanoacrylate glue and a soft bandage contact lens. This improved the patient’s condition. Injuries are one of the major causes of disabilities. A cross-sectional survey was conducted in 2009 to explore the epidemiology of injury-related disability in a district of Bangladesh. A total of 37,039 populations were interviewed in 8905 households both in the urban and rural area. Overall prevalence of injury-related disability was found 6.8 per 1000 population. Significantly higher rate was observed in the rural area compared to urban. Compare to female the rate was found significantly higher among male (8.5; 95% confidence interval: 7.3-9.9), compared to female (5.0; 95% CI: 4.1-6.2). Rates were found to increase with age, the highest prevalence was found among 60 years and above age group where the rate was 24.3 (95% CI: 19.0-30.6) per 1000 population. Compare to literate people illiterate people were found more vulnerable (odds ratio 3.1; 95% CI 1.7-5.9). The prevalence among illiterate was 13.4 (95% CI: 11.1-16.1) per 1000 population, and it was 3.8 (95% CI: 2.0-6.5) among higher secondary & above education level population. Falls caused more than 32% disabilities. Road traffic injuries, cuts, machine injuries, burns, and violence were the other common causes of injury disability. About 30% of the people identified with an injury-related disability mentioned that they don’t use public transports, and 65% of them mentioned this is because of their disability. About 37% people reported that they had lost their job because of the disability. Injury share the significant cause of disability in Bangladesh. Akhter","doi":"10.3329/bsmmuj.v15i4.64152","DOIUrl":"https://doi.org/10.3329/bsmmuj.v15i4.64152","url":null,"abstract":"This cross-sectional study was carried out at the department of Pediatric Neurology, Institute of Pediatric Neurodisorder and Autism, Bangabandhu Sheikh Mujib Medical University, Dhaka with the aim to assess the relationship between gross motor function and MRI findings in children with cerebral palsy. The duration of the study was 12 months. All the cerebral palsy cases who met the selection criteria were enrolled. Detailed history taking and physical examination was done. The gross motor function of all cases was evaluated according to Gross Motor Function Classification System- Expanded and Revised (GMECS-E and R). Magnetic resonance imaging of brain was done in all enrolled patients and reports were evaluated. MRI findings were classified as maldevelopment, cortical and subcortical gray matter lesion, periventricular white matter injury, basal ganglia lesion, normal and other findings. A total of 45 children with cerebral palsy aged between 2 to 12 years were included in this study. Mean age of the children was 4.73 (+3.17) years. Male were 31 and female were 14 in number. Functionally majority patients were in GMFCS level IV (26.7%) and level V (20%). MRI findings were abnormal in 35 (77.78%) cases. Most common abnormal MRI finding was cortical and subcortical grey matter lesions 22 (48.9%). This study revealed that cortical and subcortical gray matter lesions were significantly associated with higher level of GMFCS (IV-V) (P=0.038) and maldevelopment of brain were also significantly associated with higher level of GMFCS (P=0.01). Normal MRI findings had significant association with mild to moderate GMFCS level (I-III) (P=0.012). Rahman","doi":"10.3329/bsmmuj.v15i4.64151","DOIUrl":"https://doi.org/10.3329/bsmmuj.v15i4.64151","url":null,"abstract":"This cross-sectional study was conducted to determine the serum lipid profiles in chronic obstructive pulmonary disease (COPD) patients and its correlation with the severity of COPD in the Department of Respiratory Medicine at Bangabandhu Sheikh Mujib Medical University. A total of 100 spirometric-confirmed cases of COPD were included. Fasting blood samples for lipid profiles were collected. To identify the association between severity of COPD with lipid profiles Pearson’s correlation was used. Further multiple linear regression was done to identify the relation. The mean (standard deviation) age of the patients was 59.0 (10.7)years. The ratio of males and females was 19:1. The mean forced expiratory volume (liters) in 1 second (FEV1) was 55.1 (18.1). Most of the patients had stage II (48%) and stage III (36%) airflow obstruction. Plasma level of total cholesterol and triglyceride tend to increase, statistically non-significant, with stages of COPD. However, the frequency of its association in Bangladeshi people has not been widely studied. The aim of this study was to determine the association of Epstein Barr virus association in Hodgkin Lymphoma through detection of Latent Membrane Protein 1 by immunohistochemistry and serum Epstein Barr virus viral capsid antigen IgG antibody titer by serology. This was a cross-sectional study in purposively selected 45 histologically diagnosed cases of Hodgkin lymphoma at Bangabandhu Sheikh Mujib Medical University, Dhaka, during the period of March 2017 to December 2018. Latent membrane protein 1 was positive in 71.1% of cases of Hodgkin lymphoma. Among these positive cases, 96% of cases had significantly raised titer of serum Epstein Barr virus viral capsid antigen IgG antibody (P<0.0001 obtained by Fisher’s Exact test), which had a male predominance.
