Considering the borderline tumor status, we made an attempt to preserve the right ovary and thus preserve her fertility using minimal invasive surgical procedure (laparoscopic unilateral salpingo-oophorectomy). The patient subsequently became pregnant spontaneously within 2 months and had a full-term delivery. {"title":"Chronic Progressive Disseminated Histoplasmosis in an Immunocompetent Host - A Case Report","authors":"A. This disease is till now, sporadic in India.[1] Histoplasmosis has three main types of clinical presentations-acute primary, chronic cavitatory, and progressive disseminated.[1] Immunocompetent middle aged to older adults typically present with chronic progressive course of disseminated histoplasmosis. This form of histoplasmosis is more common in males than females. Usual presentation is low-grade fever, malaise, and oropharyngeal ulcers. {"title":"A Case of Aberrant Regeneration of Oculomotor Nerve Following Vasculitic Infarct in A Case of Tubercular Meningitis in A Young Female","authors":"H. She also complained of binocular horizontal diplopia in up and down gaze. She was diagnosed with tubercular meningitis (for her symptoms of fever, headache, vomiting, backache, lower limb paresthesia with a history of inability to walk, and seizures) with cerebellar involvement with the left eye pupil involving complete third nerve palsy 8 months ago. She was receiving antitubercular therapy (Category 2) planned for 1 year. Contrast-enhanced CT scan at that time showed few disc and ring T2 hypointense enhancing lesions with surrounding vasculogenic edema in the right parietal post central cortex, bilateral cerebellar hemisphere, and inferior vermis with subtle blooming on GRE seen in inferior vermis lesion possibly calcification suggestive of infective etiology and an acute non-hemorrhagic infarct most likely vasculitis in pontine region, Figure 1. On examination, her unaided visual acuity recorded 20/20, N6 in both eyes. Ocular motility examination was normal in the right eye whereas evaluation revealed –3 limitation of elevation and –2 limitation of depression in the left eye Figure 2. Color vision testing with Ishihara chart was normal in both eyes. Signs of aberrant regeneration such as pseudo von Graefe’s signs (adduction due to contraction of levator palpebrae muscle) and pseudo Duane’s sign (adduction of the eye on attempted upward or downward gaze) were present. Pupil in the left eye was mid-dilated sluggish reaction to light. Pupillary constriction was noted and measured with a pupil scale when eye adducted and converged, that is, pseudo Argyll Robertson sign present, Figure 3. Bhandari","doi":"10.15713/ins.bhj.60","DOIUrl":"https://doi.org/10.15713/ins.bhj.60","url":null,"abstract":"Proximal tibial plateau fractures constitute 1% of all fractures and 8% of the fractures in the elderly.[1] Isolated injuries to the lateral tibial condyle constitute 55–70% of tibial plateau fractures. Of the total number of proximal tibial plateau fractures, 1–3% are open injuries, resulting in a loss of osteoarticular fragment with a possibility of consequent development of an intra-articular gap nonunion. Loss of osteochondral articular surface along with metaphyseal bone loss results in a severe defect with further reduction of the joint function compounded due to infection. Nanda","doi":"10.15713/ins.bhj.63","DOIUrl":"https://doi.org/10.15713/ins.bhj.63","url":null,"abstract":"Introduction: Uterine rupture, being an uncommon and severe obstetrical event, is associated with high perinatal and maternal morbidity and mortality. For an unscarred uterus, incidence of rupture is approximately 1 in 17,000–20,000 deliveries or, as reported by the World Health Organization, 0.006%. Pregnancy in the presence of uterine anomalies is unusual and rare in clinical practice, and very few cases have been reported. Most of the uterine anomalies are asymptomatic and hence never noticed. An increase in the incidence of rupture in Mullerian anomalies has been noted. We present a case series of uterine rupture seen in two cases at our center. Materials and Methods: The first case was of a primigravida with a unicornuate uterus and rupture involving the lateral wall and right rudimentary horn. She underwent an obstetric hysterectomy. The second case was of a multiparous woman with failed attempt at medical termination of pregnancy with fundal uterine rupture which was repaired. Conclusion: Uterine rupture involving the lateral wall and the rudimentary horn of a unicornuate uterus is a rare clinical condition and very few cases of rupture of uterine anomalies have been reported.
