A 29-year-old male with communicating hydrocephalus and ventriculoperitoneal shunt in-situ, presented with tense ascites. Extensive investigations in the form of complete blood count (CBC), renal function test (RFT), liver function test (LFT), Computerised Tomography (CT) of brain, Contrast Enhanced CT (CECT) of Abdomen, paracentesis was done. Ascitic fluid β2-tranferrin test was positive indicating that the ascitic fluid was indeed CSF, therefore a diagnosis of CSF ascites was made. The patient’s ventriculoperitoneal shunt was converted into a ventriculoatrial shunt. Following the procedure, there has been no refilling of ascites, thus, further confirming the diagnosis of CSF ascites. COVID-19 has been implicated in occurrences of cardiovascular and thromboembolic complications due to systemic inflammation and coagulopathy (2-4). On the other hand, some published papers have argued that the signs and symptoms of severe COVID-19 infection are more similar to the pathophysiology and phenotype of complement-mediated thrombotic microangiopathy (TMA), rather than to sepsis-induced coagulopathy or diffuse intravascular coagulation (DIC) (5). Six years ago she had undergone a total hysterectomy with bilateral salpingo-oophorectomy, omentectomy and pelvic lymph node dissection for a serous papillary carcinoma of the ovary (FIGO stage IIA), followed by adjuvant chemotherapy. On regular follow up she remained disease free, until presentation. Her hemoglobin was 9.1g/dl and CA-125 level was elevated-128u/ml. Gastroscopy revealed an ulcerated submucosal tumor in the antrum measuring 5x4cms with no e/o active bleeding & minimal altered blood in stomach. Biopsy showed scanty metastatic tumor deposits from ovarian cancer. CT scan of chest, abdomen & pelvis showed a large, well-defined heterogeneously enhancing serosal mass lesion in the antrum of the stomach with soft calcification within, with no ascites. Few paraortic lymph nodes were enlarged. No other organ involvement was observed. She received chemotherapy and is currently on close follow up. Isolated gastric metastasis from an ovarian cancer is extremely rare and most often occur in advanced stage with synchronous lesions in the peritoneum. Histologically serous carcinoma is the most common variety. Prabhudesai","doi":"10.15713/ins.bhj.93","DOIUrl":"https://doi.org/10.15713/ins.bhj.93","url":null,"abstract":"Aim: We report this case to acknowledge the presence of multi drug resistant Pseudomonas aeruginosa in the community setting, which transpired in a young female, without any known comorbidity or exposure risk. \u0000Background: Pseudomonas aeruginosa is an extremely unusual pathogen in community acquired pneumonia (CAP), especially in previously healthy young adults. Immune status, underlying lung disease and possible exposures like hot tub use, should always be ruled out. There have been reports of bacterial aetiology being next common after Tuberculosis for Pyopneumothorax, but ours is the first case to report P.aeruginosa as a cause of CAP and pyopneumothorax in an absolutely healthy young adult. \u0000Case description: A young female, developed Pneumonia from the community setting which grew a nosocomial organism. Complications involved cavity formation and Pyopneumothorax, which required Intercostal Drainage (ICD) and extensive intravenous antibiotics. Immediate and appropriate management is life saving. \u0000Conclusion: CAP due to heinous organisms like Pseudomonas aeruginosa should be suspected in any patient who presents with a rapidly progressive course, even in the absence of risk factors and exposures. In cases of Pleural effusion or Pneumothorax or both, ICD drain should be inserted. Broad spectrum antibiotic coverage especially in hemodynamically unstable patients should be initiated immediately, followed by antibiotic stewardship protocols. \u0000Clinical significance: We would like to emphasize the importance of acquiring cultures from respiratory site, for early initiation/switching of appropriate antibiotics. When managed well, with surgery and antibiotics, patients usually recover without significant residual deficits. The route of spread of infection and the organism involved needs to be investigated for complete eradication of the disease. \u0000We present the case of a young girl, without any co-morbidities, initially diagnosed as having Cerebral Venous Sinus Thrombosis, and treated accordingly, who later suffered a cascade of complications that proved almost fatal. She was managed successfully at our institution, and recovered completely in the end, and is currently leading a normal life. A pathophysiological analysis of her disease process was made retrospectively, and indicated how a seemingly innocuous dental infection spread over time to produce a life-threatening subdural empyema, involving a fastidious opportunistic commensal organism. Radical surgery is the best treatment.Case ReportWe herein report a case of a large ACC of the left adrenal gland extending into the IVC through the left renal vein in a 68-year-old male patient. He presented with pain in the left side of the abdomen for one month and significant weight loss (7 kilograms). On examination, a hard irregular non-ballotable lump was palpable in the left lumbar region. Ultrasonography, computedtomography, and whole-body PET CT all revealed a large mass over the upper pole of the left kidney with tumour thrombus in the IVC. Laboratory tests showed creatinine levels of 0.9 mg/dl (normal range: 0.5–1.2 mg/dl) while plasma levels of cortisol, free metanephrines were normal. A midline laparotomy was performed, and the tumour was completely excised with regionallymphadenectomy, ipsilateral radical nephrectomy and IVC thrombectomy.Results And DiscussionWe emphasize that adrenal cortical carcinoma is a rare tumour and can have tumour thrombi invading the IVC. Core biopsy showed a metastatic high-grade neuroendocrine carcinoma. She responded poorly to chemotherapy. An exploratory laparotomy revealed bilateral large ovarian masses. Total hysterectomy with bilateral salpingo-oophorectomy was performed. Histopathology showed a metastatic high grade neuroendocrine carcinoma of both ovaries, consistent with metastasis from small cell neuroendocrine carcinoma of the lung. Post adjuvant chemotherapy she remained controlled over four years. Many cases of foreign body granuloma with various foreign bodies have been reported. However, mesh acting as a foreign body is quite a rare occurrence. \u0000Summary: Here we report a case of a 52 yrs old lady who presented to us with complaints of swelling over abdomen in supraumbilical area since 6 months. Patient had a history of umbilical hernioplasty done 4 yrs back. On clinical evaluation, the provisional diagnosis of an ?abdominal wall tumor ?recurrent hernia was made. Ultrasound and computed tomography of the abdomen was done which was suggestive of a midline abdominal wall abscess. Uncontrolled diabetes and overzealous use of steroid seems to be two main aggravating factors. Aggressive medical and surgical management of mucormycosis decreases mortality rate from 88% to 21%. \u0000Aim: This study is aimed to evaluate anaesthetic challenges in the post COVID patients coming for surgical clearance of mucormycosis. \u0000Demographic characteristics, associated comorbidities, procedural data, surgical details, hospital stay and mortality rates were reviewed in 32 patients. The mean age of our patients was 52.66 yrs. and 96.9% had associated comorbidities. Two (6.3%) patients had mallampati classification (MPC) IV and 12 had III (40.6%). Average anaesthesia duration was 282.5 min with mean blood loss 792 ml. Twenty-one (65.6%) patients were shifted to ICU (intensive care unit) out of which 15 (46.87%) were electively ventilated. Mortality was seen in 2 patients with mortality rate of 6.25%. \u0000Conclusion: Surgical resection of mucormycosis in post covid patients presents unique challenges – associated comorbidities, difficult airway, prolonged surgical duration, intraoperative hemodynamic instability massive blood loss, Amphotericin B related side effects mainly nephrotoxicity and hypokalemia, need for postoperative ventilation and prolonged hospital stay.
