At the same time, an important aspect of the present-day preoccupation with the need to integrate the reality of violence and its devastations seems to be that of an increase in popular demand to be able to hold on to ever-increasing violence as a source of fantasy and excitement. It is as if we Žnd it so difŽcult to accept it as a reality in our lives and thus part of our shared humanity, but still wish to keep it as part of fantasy life where it can be enjoyed as a source of excitement, as in the cinema or in sado-masochistic sexual practices where it is felt to be still in the person’s control. At the same time in the real world we continue to think of it as something that we are trying to eradicate as if by denial, or to rid ourselves of through splitting and projecting and equating it with evil or the work of the devil. We are not only frightened of violence when it becomes part of our reality and therefore is no longer in our control, but we can become blinded by our fear from understanding the function it plays in human life. But what this book makes clear is how it is in the therapeutic situation, where we offer ourselves as an object to a patient, that we can begin to understand the ways in which the patient uses the therapist in his Žght to survive within a relationship to another and through the experience becomes able to use the object more creatively. The clinical papers in this book show how these experiences have made it possible for analysts to begin to understand the nature of the vital role violence plays in mental functioning and psychic survival. The various case examples demonstrate how what we know about the importance of the role of violence in our physical survival as individuals and as a species can also be felt as necessary by the patient for the survival of his individual ego or the integrity of his identity. The six analysts who have contributed individual chapters to this book all describe the clinical encounters with patients where the issue of physical violence or of suicidal behaviour was central to their pathology. As well as giving brief clinical accounts of their patients they discuss issues of technique and of the developmental difŽculties which have resulted in the pathological outcome. In some ways a sequel to Obholzer and Roberts’ earlier book The Unconscious at Work, this text offers a map to help us understand what takes place within community systems of care. As someone who has been working with these ideas in relative isolation for more than Žfteen years, I know how difŽcult it can be to retain mental clarity about them. Even when I have felt clear, it has often seemed extraordinarily difŽcult to convey them to others. I do not think this entirely due to inarticulateness; there seems to be an active resistance to these ideas. One possible explanation might be the absence of a concept of normative uctuations in mental states in the repertoire of some mental health professionals, at least in the USA. Also, in the professional circles I inhabit, the work of Melanie Klein is only remotely familiar, and often seen as strange. Most of my colleagues have not heard of Menzies Lyth, Bion, Meltzer, and certainly not the more recent writers on this topic. I hoped for a text to help clarify these ideas for my students and colleagues. With this difŽculty in mind, while the book’s introductory summary of theory is quite good, it will still be incomprehensible to most Americans and I wondered if the same might also be true for community workers in areas where Kleinian ideas are not familiar. It does ‘address some of the incoherence in this Želd’ but it also introduces a healthy dose of post-modern incertitude, leaving more questions than answers. Can we speak of just one paradigm of homosexuality? Is it an illness, a deŽcit, or a construction and what other psychological, social, or political issues might cut across this debate? How is homosexuality comparable with other forms of marginalization such as racism or disability? And how many models of psychotherapy are there anyway? These are some of the issues which the various authors in the book introduce and I realized my naïve desire for ‘straight’ answers was unrealistic. Shelley’s introduction presents a brief review of homosexuality and its historical and cultural backdrop. This contextualization leads him to suggest ‘that instead of pursuing monolithic representations of homosexuality, why not abandon the dogma associated with this word of contention and, pursue a concept of homosexualities?’ (p. 1). He argues that this term could then embrace a whole range of sexual behaviour from the culturally sanctioned same-sex rituals of some New Guinea tribes to the practices of the bi-sexual, gay and lesbian communities of the Western world. This review, although interesting and clearly an attempt to contextualize the ‘illness model’ of homosexuality, led me to wonder what the rest of the book was about. This is quickly addressed by Shelley: ‘The purpose of this book is to address some of the incoherence which has resulted from this shift towards afŽrmative therapy, which has been a “reaction to” rather than a “working through” of the preceding illness models’ (p. 8). He argues that the demise of the illness model in most psychotherapies has resulted in a ‘theoretical vacuum’ of understanding about homosexuality that has been ‘sustained’ by the shift to an afŽrmative position. He believes that many schools of psychotherapy are ‘duplicitous’ in espousing such an afŽrmative position without the revision of theory that would support such a shift. This ‘incoherence’ is exacerbated by a profession in which 250 different schools of counselling and psychotherapy have been identiŽed (p. 7, Corsini 1981); although I think Karasu (1986) made it over 400. From these he has chosen BOOK REVIEWS","PeriodicalId":85362,"journal":{"name":"Psychodynamic counselling","volume":"6 1","pages":"421 - 425"},"PeriodicalIF":0.0,"publicationDate":"2000-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/13533330050132215","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"59787331","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This paper gives the background and definition to one aspect of this - the countertransference - and traces the development of the concept from Freud's first use of the term in 1910 to the contemporary view that it is a useful tool of psychotherapy. The first part explains its connection with the Kleinian concept of projective identification and discusses its elaboration by the object relations school. There is general acceptance nowadays that the countertransference contains a great deal of information about the client's psychological world. It is therefore important to understand this process and the authors have identified three main dimensions to countertransference. These are its vector (or direction and force), its variance (the quality it represents), and its valence (its effect on the client). The second part of the article illustrates, through the use of example and metaphor, how these three dimensions are defined and can be recognized. Common themes and paradigms of countertransference are identified and discussed along with some ways in which experience has shown how these might be contained and worked with constructively. Finally, a clinical vignette is presented in which some of the dimensions of countertransference are identified and used to understand the client's psychic world and foster therapeutic change.","PeriodicalId":85362,"journal":{"name":"Psychodynamic counselling","volume":"6 1","pages":"359 - 379"},"PeriodicalIF":0.0,"publicationDate":"2000-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/13533330050132125","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"59787573","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Describing some theoretical views of the origins of the sense of difference and the significance of this for the work of the supervisor and supervisee, I suggest that, for maturational processes to take place, the development of a potential space is essential. This paper looks back on a recent creative writing project undertaken with patients in a hospice day centre, and suggested reasons why the patients found this activity not only enjoyable but also therapeutic. Both the hospice as an institution and the poetic form are defined in terms of a transitional space. The article shows how the act of reading and writing poetry places a value for patients on not knowing at a time when the plain hard facts of terminal illness loom large. The author describes a practical way of engaging with poetry in this context, and uses clinical material to describe how the literary arts can provide a transformational object, and a point of inspiration, that may help patients to negotiate the changes brought about by their illness.","PeriodicalId":85362,"journal":{"name":"Psychodynamic counselling","volume":"6 1","pages":"215 - 234"},"PeriodicalIF":0.0,"publicationDate":"2000-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/135333300407558","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"59786928","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

What is perhaps more novel is the readiness on the part of psychodynamic therapists to take seriously not only their own somatic responses to the patient but their body as such – and to be open enough to talk and think together about this. This is precisely what a rather large audience of psychotherapists, counsellors and trainees reected on at a conference organized by the Freud Museum, which took place on a quiet Sunday in May in a deserted Central London. I approached the event with great curiosity and some anxiety. The title of the conference and the choice of the day gave it a vaguely esoteric, extra-curricular tone. The event turned out to be quite dense, with a lot of interesting material, looking at this topic from different perspectives. I went away full of disparate thoughts that I will try to share with you. The Žrst speaker was Joyce McDougall. Her very entertaining paper, ‘The imprisoned body’, could perhaps have been more appropriately called ‘The fragmented body’. What came across in her speech, much more than the sense of constraint experienced by the therapist ‘imprisoned’ in her chair, was the vast amount of bodily response she allows herself to experience in her work with her patients. It felt as though she has been observing her somatic countertransference for so long and with such accuracy that she was able to draw broad categories for different pathologies: the borderline PSYCHODYNAMIC COUNSELLING 6.4 NOVEMBER 2000","PeriodicalId":85362,"journal":{"name":"Psychodynamic counselling","volume":"6 1","pages":"533 - 537"},"PeriodicalIF":0.0,"publicationDate":"2000-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/13533330050197133","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"59787187","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In our current times when the issue of sexual abuse, particularly against children, and that of false memory as an alleged product of the therapeutic process itself is an increasingly pervasive and disturbing social and legal concern, it may well be a quite necessary addition. While predictably concentrating on the experience of sexual abuse in its relation to false and recovered memory, what the book also succeeds so effectively in doing is to remind the reader of just how profoundly complex and perŽdious a phenomenon memory itself is while remaining the very theoretical foundation on which psychoanalysis was constructed. It is perhaps the essentially elusive, unreliable nature of memory that makes the work of the psychodynamic counsellor and therapist so very difŽcult and, certainly when dealing with matters such as sexual abuse, so full of risk for client and therapist alike. As one of the contributors to this collection, Phil Mollon, puts it ‘Between the extremes of “true” and “false” memory lies a vast area of uncertainty and ambiguity’ (p. 127). The fourteen chapters in this book can be roughly divided into three categories, with considerable overlapping in most, but it will hopefully help to give the reader of this review some sense of the book’s rich and varied content around its deŽning subject. The categories are the historical/research, the societal, and the clinical. The Žrst of the historical/research chapters, with which the book opens, is provided by Gwen Adshead, who gives a background to the concept of false memory and outlines some of the most important Žndings and research methods related to it. She advocates an approach to all enquiry into the subject of false memory syndrome based on compassionate scepticism and intellectual rigour and asks the teasing question as to whether therapists actually believe their patients. Marjorie Orr takes a similar historical perspective in throwing light on the origins of the international false memory societies, and reminds us of the importance of protecting the few who are wrongly accused of perpetrating sexual abuse while bringing to justice the genuine offenders as well as seeking justice for the victims themselves.
