{"title":"Clinical case of peripheral facial palsy as first and single COVID-19 symptom in an immunized patient","authors":"I. B. Peripheral facial palsy is possible as the first and single COVID-19 symptom. The author presents own clinical observation of a 58-year-old patient with peripheral facial palsy. It is known that the patient completed the vaccination 8 weeks ago and also had direct contact with the patient SARS-CoV-2 (the patient's son). Facial nerve palsy was the first and only symptom of coronavirus infection. In the blood there was a moderate lymphocytopenia, increased interleukin-6, C-reactive protein, seromukoids. MRI of the brain and CT of the temporal bones did not reveal focal and diffuse pathology. After treatment, there was a complete recovery on the ninth day of treatment. Treatment included antiviral drugs, vitamins, steroids, kinesiotaping of facial muscles. The author substantiates necessity and demonstrates the results of treatment with endothelioprotective therapy (combination of L-arginine and L-carnitine) , which is aimed at elimination of systemic inflammation, oxidative stress caused by SARS-CoV-2 virus.\u0000\u0000Conclusions. General practitioner need to be alert when examining patients, including those who have been vaccinated and those who have direct contact with patients with coronavirus infection. Pathogenesis of peripheral facial palsy in COVID-19 multidirectional: neuroinvasive and neurotropic effect of the virus on the nerve, hypercoagulation, ischemia of the peripheral nerve on the background of systemic inflammatory reaction, oxidative stress. Timely detection, careful monitoring and adequate treatment of pathology are necessary to prevent complications (persistent facial contractures, synkinesis). It is due to the high comorbidity of given disorders. They can mutually worsen the duration of each other, complicate the process of treatment and increase the risk of relapse further. Besides, sleep disorders can be preserved even after the compensation of the main disease, that can influence on the quality of life and patient`s social functioning. The goal of the actual research is the learning of the influence of the sleep disorders on the quality of life and patients` social functioning with anxiety-depressive disorders of neurotic and organic genesis. For achievement of the set goal it was explored 120 patients with anxiety-depressive disorders, who were divided into 4 groups depending on the genesis of the disease and the method of medical influence. The research was done with the help of clinic- psychopathology method, adding with Pittsburg index of the quality of the (PSQI), integrative index of the quality of life J Mezzich (the dyad of the patient and the doctor), methods of describing and analytical statistic. As a result of made research, the reverse correlation connections were detected between the sleep disorders and the indexes of the quality of the patient`s life. These connections were observed during the treatment. It was determined, that the combined treatment was the most effective, directed to the therapy of the main disease and directly the correction of the sleep of the patients with anxiety-depressive disorders of neurotic and organic genesis, in comparison with the patients who had a treatment of the main disease. {"title":"Level of compliance of patients with alcohol dependence at the stages of active treatment of alcohol misuse","authors":"Natalia M. Lisna","doi":"10.26565/2312-5675-2021-18-05","DOIUrl":"https://doi.org/10.26565/2312-5675-2021-18-05","url":null,"abstract":"The article shows that one of the causes of relapses and re-hospitalizations is low compliance of patients, or non-compliance with their therapeutic regimen, especially at the end of anti-alcohol treatment. The author has developed some definitions, which specify the specifics of the phenomenon of compliance with patients with alcohol dependence; its levels in this group of patients were determined. Social and clinical-psychological factors that contribute to or hinder the formation of compliance are identified. Factors that increase compliance include: the presence of family support; motives for alcohol consumption, which are associated with the desire to neutralize negative emotional experiences (fear, anxiety, insecurity); therapeutic attitudes to understand the hidden causes of the disease and change painful stereotypes of behavior; ergopathic type of attitude to the disease; high level of subjective control in the professional field; emotional stability, social activity, as well as practicality and the ability to adequately assess their condition. Factors that reduce compliance are: young age of the patient; frequent relapses; repeated hospitalizations; addictive motivation to drink alcohol; therapeutic facilities for obtaining a \"secondary gain\" from the disease; apathetic type of attitude to the disease; overall low level of subjective control; conflict, negligence, rigidity of thinking. The selected psychological factors of compliance with alcohol dependence can be considered as targets of psychoprophylactic and psychocorrectional programs aimed at optimizing the process of adherence to the therapeutic regimen of patients with this pathology, which, in turn, will help prevent relapses and maintain remissions. {"title":"Characteristics of cognitive impairment in multiple sclerosis patients depending on different risk factors","authors":"T. The aim of this study was to asses characteristics of CI in separate cognitive domains depending on socio-demographic (age, sex, level of education), disease parameters (severity, course type and disease duration) and external factors (smoking). The current study enrolled 137 MS patients (102 women and 35 men) aged from 22 to 69 years. All participants were divided into two groups depending on the disease course: group А – patients with relapsing-remitting (RR-MS) type (n=106) and group B – participants with progressive forms of the disease (n=31). The following study discovered that disruption of separate cognitive domains was present even without the apparent CI according to MоCA (Montreal Cognitive Assessment): executive functions impairment (p=0,0013) was found most frequently in case of RR-MS, and memory (p=0,0233) decline in case of progressive forms. In the group A moderate CI were associated with decrease of memory (p<0,0001), attention (p=0,0061), executive functions (p=0,0005), language (p=0,0080) and abstract thinking (p=0,0018); severe CI – with disorders of attention (p=0,0055), language (p<0,0001) and abstract thinking (p=0,0144). As for the group B, moderate CI were associated with decline of abstract thinking (p<0,0001), and severe CI – with impairment of memory and executive functions (p=0,0337). Level of physical disability and smoking impact CI independently of MS course, meanwhile, presence of higher education proves to be beneficial for preserving cognitive functions. Nazarchuk","doi":"10.26565/2312-5675-2021-18-02","DOIUrl":"https://doi.org/10.26565/2312-5675-2021-18-02","url":null,"abstract":"Objective - to study the emotional and psychic state at the patients with neurological cervical spine disorders (NCSD) and determine of the constitutional psychic human qualities influence at the it. \u0000\u0000Methods and subjects. 87 patients (18-44 years old), who suffered from subacute and chronic NCSD, were examined by clinical-anamnestic, clinical-functional (vertebral neurological), clinical-neurological methods. Emotional and psychic state was examined by Spielberger-Hanin test and Hamilton Depression Rating Scale. Psychic functions of human constitution were studied by temperament type determining with using Eysenck’s EPQ questionnaire.\u0000\u0000Results and discussion. The course of subacute and chronic cervicalgia, cervicocranialgia, cervicobrachialgia (neck and shoulder pain) was characterized depression symptoms at the melancholic temperament type (at 76% patients, р< 0,05). At the choleric temperament type depression symptoms was present meeted very often too (at 68% patients). At the sanguine temperament type depression symptoms was very rarely (at 29% patients). The peculiarities from reactive and personality anxiety were found too. It were at the melancholic and choleric patient highter significantly, than at the sanguine patients (p <0,05, p <0,01, p < 0,001). Temperament influence on the depression symptoms and anxiety confirmed by reliable correlations.\u0000\u0000Conclusions. The emotional and psychic state at the patients with NCSD characterize as anxiety on the background of neurological symptoms of cervicalgia, cervicocranialgia, cervicobrachialgia (neck and shoulder pain). It this symptoms add with decreased performance, mood and sleep disturbances and all these complaints in the aggregate are summed up as depressive disorders, mainly of mild severity. Constitutional psychic human qualities, which included in temperament, influence on the it formation. The models of influence different temperament types on the emotional and psychic states at the patients with NCSD were represented. It points pathogenic significance of melancholic and choleric temperament types and sanogenetic qualities of sanguine type. Constitutional psychic human qualities and emotional and psychic state should be considere at the diagnostic and treatment patients with NCSD. Significance of the human constitution, emotional and psychic state for NCSD confirm by study results. Je présenterai en l’occurrence, en leur totalité,1 quelques textes courts qui présentent la particularité d’être écrits en pataouète, la langue populaire des Français d’Algérie. Une analyse poussée de ces textes demanderait le concours de la philologie et de la linguistique ou de la dialectologie, qui ne sont pas de mise ici. Cependant, l’intérêt de ces textes est et reste certain ; ils constituent un témoignage et des documents de première main d’une langue (ou d’un dialecte, si l’on préfère) maintenant en voie de disparition : car c’est une langue ou un dialecte qui va disparaître fatalement, avec la disparition progressive de ses locuteurs naturels, les Pieds-Noirs, qui le parlaient normalement, tous les jours, chez eux en Algérie ou en Afrique du Nord, et de leurs descendants de première génération ‘française’. Ceux-là souvent étaient nés là-bas (ainsi qu’on le dit en pataouète) et avaient été rapatriés avec leurs parents en 1962, au moment de l’indépendance de l’Algérie, non plus ‘française’, mais devenue une république indépendante habitée et gouvernée depuis par la composante majoritaire de sa population, jusqu’alors gardée dans une condition de subordination coloniale.
