High-intensity interval training (HIIT) has emerged as a promising strategy to improve walking in people with chronic stroke, but broadly integrating HIIT into stroke recovery remains limited. One key barrier is the reliance on maximal exercise testing to prescribe HIIT, which is often not feasible in clinical settings. To address this gap, the Fitness After Stroke Trial (FAST) evaluates the preliminary efficacy of HIIT compared to moderate-intensity continuous training (MICT), using a validated submaximal exercise testing protocol to guide individualized exercise prescription in people post-stroke.

Methods: FAST will enroll 50 individuals with chronic stroke into a double-blind, two-arm, parallel-group preliminary efficacy trial. Participants will be stratified by lower extremity motor function and randomized to HIIT or MICT. Exercise sessions will occur three times per week for four weeks using a total body recumbent stepper. The primary outcome is estimated peak oxygen consumption from the Total Body Recumbent Stepper (TBRS) submaximal exercise test. Secondary outcomes include middle cerebral artery velocity at rest and during exercise, flow-mediated dilation and pulse wave velocity. Tertiary outcomes include walking speed and distance.

Discussion: FAST represents one of the earliest double-blind randomized trials to directly compare HIIT and MICT on a recumbent stepper in people post-stroke, using a validated submaximal exercise testing protocol to individualize exercise intensity. This study will generate preliminary effect sizes for key physiological and functional outcomes obtained with this modality, providing the critical data needed to optimize dosing strategies and power a future definitive trial.

Trial registration: NCT05936008. Cardiovascular risk factors track from childhood into adulthood, and vascular abnormalities detected in youth predict future cardiovascular outcomes. Despite compelling evidence, vascular assessment in youth has been impeded by a lack of reference values, standardised measurement protocols, and consensus on distinguishing physiological from pathological vascular ageing. The Youth Vascular Consortium (YVC) was established in 2020 to address these gaps through international collaboration between leading experts in the field.

Results: The YVC comprises 33 research centers from 27 countries across five continents, including 29,704 participants aged 2 to 40 years. All centers assessed at least one validated vascular measure including pulse wave velocity, central blood pressure, augmentation index, intima-media thickness, carotid distensibility, or flow-mediated dilatation. The YVC has generated major outputs advancing vascular health assessment in youth, including international expert consensus on standardised definitions of early vascular ageing from birth through young adulthood and evidence-based recommendations for vascular assessment protocols. In addition, device-specific reference values for pulse wave velocity were established with age and sex specific percentile curves, enabling identification of youth with elevated arterial stiffness.

Conclusions: The YVC provides an international platform for investigating vascular health from early life. The method builds on a nonlinear autoregressive model with exogenous inputs (NARX), implemented through artificial neural networks trained on subject‑specific electrocardiography (ECG) and photoplethysmography (PPG) signals. Three model configurations are evaluated: (1) a reference NARX model trained on 30 min of data, (2) a reduced‑data NARX model trained on 15 min, and (3) a physiology‑informed model (NARXphysio), also trained on 15 min, which incorporates a personalized four‑parameter BP-PPG sigmoidal layer into the training process through a dual‑output loss function that simultaneously estimates the BP and PPG waveforms. By embedding BP-PPG physiology directly into training, the architecture ensures internal physiological consistency-something not achieved in existing PPG‑based or purely data‑driven machine learning approaches. All three models were assessed on a small (n = 5), but rich dataset, covering over six hours of activities of daily living. Despite using only half the training duration of the baseline NARX model, the physiology‑informed approach maintains accurate BP estimation. Beyond improved accuracy, the sigmoidal layer provides physiologically interpretable parameters, including estimated arterial‑compliance curve width with an average of 36.5 mmHg and an estimated PPG contact pressure with an average of 59 mmHg in this study, offering insights unavailable in standard neural network models. Together, these preliminary results suggest that the NARXphysio model enables effective personalization of cuffless BP waveform estimation with limited training data, highlighting its potential to improve both the reliability and the clinical relevance of continuous cuffless BP monitoring.
","PeriodicalId":8466,"journal":{"name":"Artery Research","volume":"32 1","pages":"21"},"PeriodicalIF":1.6,"publicationDate":"2026-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13428692/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148663373","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Purpose of the Study To explore the effects of ischemia on EPCs. Methods We established the EPCs in vitro ischemia model induced by OGD and detected the expression of Sonic Hedgehog (Shh). Results The results show that the expression level of Shh was reduced obviously in EPCs cultured under the OGD conditions. Invasion, migration, and tube formation abilities were significantly decreased in OGD-EPCs, as well as the ratio between the level of serine/threonine kinase B (AKT) and phosphorylated AKT (p-AKT), the ratio between the level of glycogen synthase kinase-3β (GSK-3β) and phosphorylated GSK-3β (p-GSK-3β), and the expression of mammalian target of rapamycin (mTOR) in OGD-EPCs, while they were recovered remarkably after adding smoothened agonist (SAG), insulin-like growth factor-1 (IGF-1), or IGF-1 + SAG.
