{"title":"Extracorporeal shock wave treatment for chronic plantar fasciitis (heel pain).","authors":"C Ho","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>(1) Electrohydraulic, electromagnetic, or piezoelectric devices are used to translate energy into acoustic waves during extracorporeal shock wave treatment (ESWT) for chronic plantar fasciitis (or heel pain). These waves may help to accelerate the healing process via an unknown mechanism. (2) ESWT, which is performed as an outpatient procedure, is intended to alleviate the pain due to chronic plantar fasciitis. (3) Results from randomized controlled trials have been conflicting. Six trials reported data that favour ESWT over placebo or conservative treatment for efficacy outcomes, while three trials showed no significant difference between the ESWT group and the placebo group. (4) The lack of convergent findings from randomized trials of ESWT for chronic plantar fasciitis suggests uncertainty about its effectiveness. These waves may help to accelerate the healing process via an unknown mechanism. (2) Results from randomized controlled trials have been conflicting. Half of the studies showed statistically significant improvement in pain in the treatment group, and half of the studies had data showing no benefit over placebo for any measured outcomes. (3) Limited evidence shows that ESWT is cheaper than arthroscopic surgery, open surgery, and other conservative therapies, such as steroid infiltrations and physiotherapy, that continue for more than six weeks. (2) Evidence suggests modest short-term reductions in pain and improvements in function, and no superiority among HA products. (3) Adverse events are rare, benign, temporary, and likely associated with the IA injection. (4) Economic analyses of mixed quality suggest that HA may be cost effective compared with usual care. Large patients cannot be accommodated, and some persons experience claustrophobic reactions. Open MRI systems, in which the patient is placed between two plates, overcome these disadvantages. (2) Open MRI scanners are widely used in health care. High-field closed MRI systems are preferred for many examinations. (3) Early versions of open MRI scanners had low magnetic field strength, gave poorer image quality than most closed systems, and required longer examination times. Newer open scanners include machines with higher magnetic field strengths and improved image quality. (4) Closed high magnetic field scanners with short magnets and wide bore tubes offer improved comfort to patients, and may be an alternative to open scanners. (3) Recent evidence suggests that FFDM is more accurate than FSM for diagnosing cancer in women younger than 50 years, those with dense breasts, and pre- or peri-menopausal women. (4) The costs of FFDM are higher than those for FSM. (2) Evidence from a small, randomized controlled trial indicates that, compared with placebo, tacrolimus is associated with higher rates of improvement and similar rates of remission in those with fistulizing patterns of disease. (3) Nephrotoxicity, which has been reported with the use of tacrolimus in clinical trials, seems to improve with dose reduction, but may be associated with irreversible histologic changes. (2) The use of RTX with methotrexate (MTX) results in statistically significant clinical improvements among RA patients who have an inadequate response to standard therapies, when compared to the use of MTX alone. (3) The optimal dose, duration of treatment or retreatment, long-term efficacy and safety, and placement of RTX in RA treatment algorithms need to be further investigated. The major cause is hepatitis C, which affects 240,000 Canadians. (2) Assessing the degree of liver fibrosis is critical to its management. Liver biopsy, an invasive procedure, is considered to be the diagnostic gold standard. (3) FibroScan is a rapid, non-invasive technology that uses low frequency vibration and ultrasound to assess the stiffness of liver tissue. (4) The diagnostic performance of FibroScan is good for identifying severe fibrosis or cirrhosis, but it is less accurate for milder presentations.
