Occasionally, orthodontic appliances or small orthodontic components are accidentally swallowed and have caused problems with either the airway or the gastrointestinal tract, especially where the patient is supine or semi-recumbent. Despite their rare occurrence, the morbidity from a single incident and the level of specialized medical care that may be needed on emergency basis to manage such incidents is too high to ignore. Moreover, there is also the related risk of malpractice litigation given the fact that these incidents are preventable and increasing awareness among people. This article attempts to review potential risks and complications of ingestion/aspiration episodes based on relevant literature and describe the type of appliances and their parts most likely to cause problems. A patient 12 years, class III in permanent canine teeth, with multiple agenesis of upper posterior teeth was treated with the aid of four mini-implants in the palate and Haas modified type appliance. During the period of expansion, an interincisal diastema was observed as the first clinical sign of disjunction of the sutures. After correction of cross bite, the expansion of the palate was confirmed by upper occlusal radiographs. Thus, the evaluations showed that the technique was effective and that new scientific studies should be conducted to further develop this subject. {"title":"Effects of Lower Third Molar Angulation and Position on Lower Arch Crowding.","authors":"Mimoza E Selmani,&nbsp;Julijana Gjorgova,&nbsp;Manushaqe E Selmani,&nbsp;Mirsad Shkreta,&nbsp;Shkelzen B Duci","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Unlabelled: </strong>The role of the third molars in lower arch crowding has been debated for more than a century.</p><p><strong>Objectives: </strong>The aim of this study was to determine the relationship between lower arch crowding and the presence of angulation and position of lower third molar.</p><p><strong>Methods and materials: </strong>The measurements of the dental arch were made in 120 subjects aged 16 to 21 years, with average age to 18 years. The subjects were divided into two groups: Class I normal occlusion comprised 35 male and 25 female with mean age 18.87years, whereas Class I crowding comprised 27 males and 33 females with mean age 18.5 years. The dental pantomogram (DPT) were used to calculate the ratio of retromolar space (Ganss ratio), angulation of third molar to second molar and third molar to the base of the mandible.</p><p><strong>Results: </strong>The results showed that measurements of Ganss ratio, third molar angulation to the base of the mandible, and third molar to second molar inclination, was statistically significant between crowded and normal groups.</p><p><strong>Conclusion: </strong>It can be concluded that there was a strong relationship between angulation and position of third molars and lower arch crowding. This treatment alternative is indicated in clinical situations like mild to moderate class III malocclusion, mild anterior mandibular tooth size excess, periodontally compromised teeth, ectopic eruption of mandibular incisor and minimal openbite tendencies. Unlike in premolar extraction cases, space closure in mandibular incisor extraction cases is unique in which the extraction space will be in the middle of the arch. The end result of space closure in these cases should be well aligned, upright, anterior teeth with parallel roots and the goal can be achieved with the bodily tooth movement through proper application of biomechanics. Even though several factors causing tooth malformations have been identified, many are still partially understood, thus requiring a more thorough study. I present here a newly designed customized self active HIV-Spring (Hari Inverted 'V' Spring) for closing a large diastema with minimum orthodontic materials and less activation visits. This author propose, a simplified, non-surgical technique to correct skeletal anterior open bite in adults using non-ligating, frictionless brackets and vertical elastics. Treatment was performed with a mini-implant and a temporary crown.
