The initial diagnosis was made after the detection of a continuous cardiac murmur at the age of six years. Transthoracic echocardiography showed the right coronaric ostium dilatation, the site of drainage in the left atrium and left ventricle volume overload. Catheterization confirmed the diagnosis. The patient underwent percutaneous closure by PDA occluder device. Immediate post-closure angiograms showed complete occlusion of the fistula. Balloons, stents, and debulking devices have been the mainstay tools of the interventionalist. However, as engineering skills have developed and clinical techniques have improved, new clinical avenues for the interventionalist beyond the coronary vessels are starting to evolve. One of the most opportunistic areas in cardiology for the interventionalist is the left atrium where treatment of clinically important issues such as mitral regurgitation, left atrial thrombus, atrial fibrillation, patent foramen ovale, and atrial septal defect may be approached. Additionally, the left atrium is a source of important hemodynamic information that can be captured with new implantable devices. While in the past the trans-septal puncture has been infrequently used, the future may hold this technique as one of the most common. The following manuscript describes the status of some of the newest opportunities for interventionalists that lie beyond atherosclerotic disease and within the left atrium.
","PeriodicalId":83747,"journal":{"name":"International journal of cardiovascular interventions","volume":"6 3-4","pages":"101-6"},"PeriodicalIF":0.0,"publicationDate":"2004-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1080/14628840410030432","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"24997028","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The objective of our prospective study was to compare sensitivity and specificity of Dobutamine Stress Echocardiography (DSE) and Single Photon Emission Computer tomography (SPECT) with the pressure derived Fractional Flow Reserve (FFR) for the identification of hemodynamic relevant coronary lesions in patients with predominately coronary multivessel disease and angiographically intermediate stenoses.

Methods: Inclusion criteria were a coronary lesion of 50-75% diameter stenosis by visual assessment in patients with known or suspected CAD. Oral administration of numerous pharmacological agents has failed to reduce restenosis after coronary stenting in humans, possibly owing to insufficient local drug concentration. Therefore, drug-eluting stents were developed as a vehicle for local drug administration. The authors developed a new drug-eluting polymer stent that is made of poly-l-lactic acid polymer mixed with tranilast, an anti-allergic drug that inhibits the migration and proliferation of vascular smooth muscle cells induced by platelet-derived growth factor and transforming growth factor->1. Polymer stents might be superior to polymer-coated metallic stents as local drug delivery stents in terms of biodegradation and the amount of loaded drug.
