{"title":"Perspectives of significant others in dialysis modality decision-making: a qualitative study.","authors":"Alexis de Rosenroll, Kathryn Smith Higuchi, Katherine Standish Dutton, Mary Ann Murray, Dawn Stacey","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To explore the experience of the dialysis modality decision-making process from the perspective of the significant other.</p><p><strong>Methods: </strong>A qualitative interpretive description study was conducted using the Interprofessional Shared Decision Making Model (Légaré et al., 2011). Data collection included one-on-one, semi-structured interviews, the Decisional Regret Scale, and the SURE tool.</p><p><strong>Results: </strong>Ten significant others were interviewed. They included wives, husbands, and daughters of dialysis patients. Their roles involved providing a positive outlook, \"being with\", advocating, caregiving, learning together, sharing opinions, and communicating values, preferences and treatment feasibility. Broader factors influencing significant others included choosing life, unanticipated life change, and personal health problems. Implementation of the chosen modality resulted in unanticipated events, relationship changes and challenges to travelling.</p><p><strong>Conclusion: </strong>Significant others play supportive roles for dialysis patients and are involved in the decision-making process associated with treatment decisions. {"title":"The connection between neurosciences and dialysis: a quick neurological assessment for hemodialysis nurses.","authors":"Charlotte McCallum, Margaret Leonard","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Stroke is a significant medical emergency and occurs frequently in patients receiving renal replacement therapy. In 2005, the Registered Nurses Association of Ontario (RNAO) published Nursing Best Practice Guidelines for Stroke, highlighting the necessity and expectation that all nurses, regardless of specialty, are able to perform an abbreviated neurological assessment, identify the symptoms of stroke, and respond to these as a medical emergency. This article highlights the significance of neurovascular disease in persons who receive renal replacement therapy, and serves as an educational review for dialysis nurses to increase their neurological functional assessment skills. Our centre has had a long tradition of working with a professional practice model with proven nursing outcomes such as job satisfaction, empowerment and perceptions of improved patient care. Our model, in place since 1999, has provided an opportunity to discuss and articulate a vision for nursing practice based on the values of accountability, evidence-informed care and empowerment. In order for the model to effectively guide nursing practice, a revision was necessary to keep pace with the changes in the renal program and the health care environment. The revised model needed to address the enhancements in nursing roles, practice environment, corporate requirements and patient care needs. {"title":"Implementation of a foot assessment program in a regional satellite hemodialysis setting.","authors":"Barbara Wilson, JulieAnn Lawrence","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Individuals with chronic kidney disease are at higher risk for foot problems (i.e., ulcers, deformities, amputations) than the general population. Research demonstrates that assessment and active monitoring, teaching, and timely interventions can reduce the number and severity of lower limb amputations in the hemodialysis population. This paper reports on a quality improvement project aimed at implementation of a foot assessment program in a regional satellite hemodialysis setting based on the Registered Nurses' Association of Ontario's (2005) best practice guideline (BPG), The Assessment and Management of Foot Ulcers for People with Diabetes. Elements of the program include a one-time full assessment of risk for all patients transferring to the satellite program followed by monthly foot checks for those deemed high risk (i.e., people with diabetes). Evaluation of the program has been positive from both patients and hemodialysis nursing staff. There has been a greater emphasis on self-management around the care and management of patients and their high-risk feet.
