Koren-Karie","doi":"10.1002/IMHJ.20010","DOIUrl":"https://doi.org/10.1002/IMHJ.20010","url":null,"abstract":"Mothers' insightfulness, which involves understanding the motives underlying the child's behavior in a complete, open, and accepting way, was examined both at the beginning and at the end of a therapeutic preschool program for preschoolers using the Insightfulness Assessment (Oppenheim & Koren-Karie, 2002). The goal of the study was to examine the links between improvement in mothers' insightfulness and reduction in children's behavior problems. Thirty-two children referred to a therapeutic preschool program for a range of behavioral and emotional problems and their mothers participated in the study. Results showed that mothers who shifted from noninsightfulness before treatment to insightfulness after treatment had children whose behavior problems decreased whereas mothers who did not make such gains had children whose behavior problems increased. The social skills of all children improved during treatment. The organizing theme is that a dual attachment and trauma lens must be used in the assessment and treatment of infants and toddlers with mental health and relationship problems. The quality of attachment is an important factor in children's capacity to process and resolve traumatic experiences. At the same time, traumatic events often have a damaging effect on the quality of existing attachments by introducing unmanageable stress in the infant- parent relationship. It is argued that trauma in the first years of life needs to be assessed and treated in the context of the child's primary attachments. Reciprocally, the etiology of attachment disturbances should include an assessment of possible exposure to trauma in the child and in the parents. Les bebes classifies comme etant secures utilisant la SSS avaient des scores AQS bien plus eleves qu'en particulier les enfants insecures et desorganises. Au niveau item AQS, les bebes desorganises etaient decrits comme etant bien plus non-cooperatifs, difficiles, et en colere par rapport aux bebes secures. Lorsque la securite, evaluee en utilisant la SSS, fut controlee, la qualite observee du comportement interactif parental, l'ego-resilience parentale, les hauts niveaux d'orientation dans l'exercice du bebe et de plaisir, et des niveaux bas de tendance a la colere du bebe ont chacun ete trouves pour expliquer les portions importantes et uniques de la variance dans les indices de securite AQS. Infants were classified as easily frustrated or less frustrated and observed in several types of interactions with their mothers in the laboratory. Mothers completed several measures that indicated their level of parenting stress, psychological functioning, and marital adjustment. Maternal behavior with infants was coded along the dimensions of sensitivity, intrusiveness, and physical stimulation. Results indicated that maternal intrusiveness was related to infant temperament and that maternal physical stimulation was predicted by an interaction of infant temperament and mothers' perceived parenting stress. Implications of these findings for mother- infant interaction and subsequent child adjustment are discussed. RESUMEN: Un grupo muestra de 162infantes de seis meses de nacidos fue seleccionado de una muestra aun mayor de 346 infantes, sobre la base de lo que las madres reportaron acerca del temperamento de sus infantes y una evaluacion de laboratorio del temperamento. La force et l'utilite clinique de notre modele resultent de ses trois composantes principales: 1) une perspective fondee sur la relation, qui reconnait le role important que les sentiments et les emotions jouent tout au long de la vie; 2) un modele de supervision entre collegues qui permet d'assurer qu'aucun individu ni aucune discipline ne soit l'expert; et 3) une experience de collaboration d'education continue, basee sur le cas par cas. Les traits essentiels de la formation et du maintien d'un groupe CPSG sont discutes, y compris l'acces a un site internet contenant des documents cles pour commencer un CPSG et des documents d'evaluation. Les barrieres existantes qui empechent une communication efficace entre les disciplines sont discutees, tout comme le sont des methodes specifiques pour structurer les donnees de cas. Cette etude essaie de determiner dans quelle mesure cette association est influencee par (a) le risque en sante mentale de la dyade; et (b) les differentes definitions et mesures operationnelles d'a la fois la relation dyadique et le concept de communication. Quatre vingt dix-neuf bebes (de 10-30 mois) et leurs meres ont ete recrutes: 46 etaient des dyades a risque appelees a consulter dans une clinique de sante mentale pour des difficultes emotionnelles et/ou de comportement liees a la relation et 50 etaient des dyades n'etant pas a risque et ne desirant pas consulter pour des raisons de sante mentale et servant de reference normative ou de groupe de comparaison. Plusieurs facteurs ont ete evalues: (a) la competence developpementale, (b) la psychopathologie maternelle, (c) la qualite de l'interaction mere-bebe pendant le jeu, (d) la classification de securite de l'attachement, (e) la communication pre-linguistique et socio-affective, et (f) la communication linguistique. Chez tous les bebes etudies, la qualite de l'interaction mere-bebe durant le jeu, plutot que la classification de securite de l'attachement, etait liee aux competences de communication pre-linguistique et socio-affective des bebes mais noh a la communication linguistique. Differents aspects de l'interaction mere-bebe ont predit la communication pre-linguistique pour les bebes cliniques et les bebes de comparaison, alors que seul l'âge du bebe a predit la communication linguistique. Tous les bebes ont fait etat de competences de communication dans l'eventail normal, mais la performance statistiquement plus faible dont les bebes cliniques ont fait etat pourrait s'averer cliniquement significative durant l'enfance. L'engagement des peres a ete etudie en ce qui concerne un nombre de variables, y compris la personnalite, les attitudes et la structure familiale. Le concept de comprehension des etats mentaux et de la mentalisation et la notion/definition de travail de la Fonction Auto-Reflexive (Reflexive Self Function, an anglais), telle qu'elle a ete concue par Fonagy et ses collegues, semblent avoir d'importantes implications pour l'engagement paternel et parental, et pour le developpement normal de la relation bebe-mode de soin. Le but de ces recherches etait delargir les connaissances et reflexions actuelles sur le paternage durant la grossesse, en enquetant sur la maniere dont 112 futurs peres participaient a la transition a la paternite et la maniere dont il etaient capables de reflechir a leur experience. Notre but consistait a identifier des styles parentaux selon les concepts de May et de classer la Fonction Auto-Reflexive (Reflexive Self Function, en anglais) dans The Clinical Interview for Parents During Pregnancy (CGG-Colloquio per Genitori in Gravidanza). Le CGG est un entretien semi-structure, concu en gardant a l'esprit les vues psychanalytiques sur les representations mentales les plus recentes, et a pour but d'enqueter sur les sentiments et les attitudes envers le bebe, le monde interieur et subjectif du couple. Les symptomes depressifs et le style d'attachement insecure chez les meres et/ou les peres etait lie aux perceptions du nourrisson comme etant de temperament plus enclin au negativisme qu'au positivisme. Des analyses multivariates de symptomes depressifs et des styles d'attachement avec un temperament percu suggerent que les symptomes depressifs et le temperament percu demeurent tres lies, alors que les associations entre les styles d'attachement et le temperament percu etaient, dans la plupart des cas, reduites a presque rien. Nous avons aussi teste si l'attachement secure chez les parents empechait les perceptions de nature negative ou positive liees a la depression, sans pouvoir trouver de preuve etayant cette theorie. Carter","doi":"10.1002/IMHJ.10090","DOIUrl":"https://doi.org/10.1002/IMHJ.10090","url":null,"abstract":"Objectif: Cet article presente les proprietes psychometriques de l'adaptation francaise de l'Infant Toddler Social and Emotional Assessment (ITSEA; Carter & Briggs-Gowan, 2000), appelee l'Evaluation Sociale et Emotionnelle de Jeunes Enfants (ESEJE; Bracha, Nicolazo & Bedjhet, 2000). Methodes: L'adaptation francaise a necessite une traduction et une retrotraduction et a ete administree a 179 parents dans des centres de PMI et a 115 parents dans des creches. Des analyses factorielles confirmatoires ont ete utilisees pour evaluer la structure factorielle et le model fit a ete evalue en utilisant trois indices standards. Une analyse de la variance a examine les effets de l'âge et du sexe sur les moyennes des domaines et des echelles. Resultats: Les analyses factorielles confirmatoires ont soutenu une structure factorielle comparable a celle rapportee pour l'ITSEA. Tous les domaines et la plupart des echelles font preuve d'une fiabilite intra-echelle adequate.
