{"title":"Myocardial revascularization for acute myocardial infarction.","authors":"Janusz Stazka, Krzysztof Olszewski, Krawczyk Elzbieta, Janusz Rybak","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Unlabelled: </strong>Coronary artery bypass grafting (CABG) for acute myocardial infarction (AMI) is associated with increased morbidity and mortality compared with CABG in non-AMI patients. We describe the surgical results of these high-risk patients. Sixteen patients (nine male and seven female) underwent CABG after recent onset of acute myocardial infarction. The mean age was 64.7 (range: 51 to 78). Seven patients (43.8%) had at least one myocardial infarction in the past, three (18.8%)--diabetes mellitus, two (12.5%)--chronic renal failure after nephrectomy. Five patients (31.3%) were preoperatively in cardiogenic shock, and six patients (37.5%) required preoperative intraaortic balloon pump (IABP) for stabilization. Two patients (12.5%) had postinfarction ventricular septum perforation (VSD) and three patients (18.8%) hed left main artery trunk stenosis. Mean ejection fraction was 46%. During the first 24 hours six patients were operated, during the second day two, up to the seventh day five more and last three between 8th and 21st day after AMI. The operations were performed with extracorporal circulation in middle hypothermia (34 degrees C). The mean number of grafts per patient was 2.9, and the left internal thoracic artery was used in 11 patients (68.8%). In two patients VSD was closed with Dacron patch. All the patients needed longer time of reperfusion and inotropic drugs and eight (50%) of them mechanical support (IABP) during weaning from cardiopulmonary bypass. Three patients (18.8%) died (both with VSD) because of low output syndrome and multiorgan failure, all were over 70 years old (72, 73, 78).</p><p><strong>Conclusion: </strong>emergency coronary artery bypass grafting for acute myocardial infarction is associated with increased operative mortality and morbidity especially in the first 48 hours. The only risk factors for postoperative mortality in this group of patients are age over 70 years, cardiogenic shock, left main artery stenosis and the shortness of the interval between acute myocardial infarction onset and surgery.</p>","PeriodicalId":8245,"journal":{"name":"Annales Universitatis Mariae Curie-Sklodowska. {"title":"Psychosomatic aspects of arterial hypertension in women.","authors":"Monika Kowalik","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Arterial hypertension is one of the most widespread diseases in the world, both in women and men. In Poland, about 45% of the adult population suffers from arterial hypertension--it afflicts 45% of men and 36-38% of women. Arterial hypertension is a set of regulatory disorders of a complicated, multifactor etiology encompassing genetic conditions, the influence of the environment and some individual factors. Being also psychosomatic disorder it is strongly linked with improper adaptation to stress, personality traits and behavioural conditions esspecially with the type A behaviour pattern. This type of behaviour includes a strong need of achievement a tendency to dominate, and aggressiveness. The type A women--impatient, aggressive, hostile to their environment, committed to work and competitive--have a higher CVR (cardiovascular reactivity) coefficient and higher arterial blood pressure than type B. Some authors suggest that the lowering of estrogen levels in women over the age of 50 increases the frequency of occurrence of arterial hypertension. In spite of the fact that we do not have a full knowledge of the mechanisms starting the reaction chain, the awareness of these factors can help us decrease the risk of the development of the disease or hinder its progression.</p>","PeriodicalId":8245,"journal":{"name":"Annales Universitatis Mariae Curie-Sklodowska. The pathophysiology of uremic pruritus is poorly understood but believed to result from metabolic disequilibrium, increased release of pruritogenic mediators and altered balance between the opioid receptors. Ultrastructural examinations of the liver were performed. {"title":"Investigations of ultrastructure of damaged and regenerated skeletal muscle fibers.","authors":"Mirosław Lańcut, Piotr Godlewski, Marta Lis-Sochocka, Józef Visconti, Krystyna Czerny","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Our investigations concerned the head of the parietal part of quadriceps femoris, and we based our investigation on observations of the ultrastructure of muscle fibers using an electron microscope. We observed tissue samples taken from patients (10 men) 25-35 years old, who had old damage of knee joint ligament (after about 6 week's immobilization). In the first group, segments of tissue of parietal head of quadriceps femoris were taken inter-operationally from patients in whom there was found old damage of knee joint ligament. The second group was of tissue segments of this muscle after surgical repair of knee and rehabilitation, which consisted in power training using resistance machines. The muscle fiber samples of quadriceps femoris which were taken from patients during the first operation, showed big changes in their ultrastructure. These changes included: myofibrils disintegration; disturbance of regularly arranged striation in sarcomers; dissappearance of Z line. In the sarcoplasm, we observed large vacuolisation, and in the interfibrillar spaces--an accumulation of exudate and morphotic elements of blood outside the capillary vessels. Observations of muscle tissue after regeneration, showed a big improvement in the muscle cell's ultrastructure--the myofibrils were regularly arranged, and the sarcomers striations showed no deviations from normal structure. {"title":"Heavy metal pollution and children morbidity rate in the Rejowiec Fabryczny area.","authors":"Elzbieta Królak, Marzena Piłat","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The concentrations of heavy metals (Pb, Cd, Cr, Cu, Mn and Zn) were analyzed in the soils of the Rejowiec Fabryczny area. The levels of these metals were also measured in the raw materials used in cement production, in cement itself, and in dust removed by electrofilters mounted over the clinker kilns. Morbidity rates were also analyzed of children up to 6 years of age examined and treated in the local ambulatory medical center in Rejowiec Fabryczny in 1982 and 2002. The soils of the Rejowiec Fabryczny area showed elevated levels of Pb, Cd, Zn and Cu, comparing to the reference values. Installation of electrofilters in the cement plant resulted in a considerable reduction of Pb and Cd discharge. It was observed that the frequency of respiratory disorders in children from the Rejowiec Fabryczny area was higher comparing to other regions. The changes in homeostasis of this important antioxidant may be induced by the effect of sex hormones for example. Based on this, the aim of our work was to estimate the influence of sex hormones administration and vitamin E supplementation on tissue ascorbic acid concentration in female and male rats. It has been showed that estrogen administration to females resulted in vitamin C loss, while testosterone administration to males elevated this level in all the tested tissues.
