One possible means of enhancing digital radiographic image is pseudocoloring (i.e., color conversion of gray-scale images). Recently, this method has become widely used in digital radiology. The human eye is more sensitive to differences in color than to differences in shades of gray. Theoretically, converting a gray scale intensity level of a digital image into colors could enhance the radiographic information. There have been some studies that applied pseudocoloring of digital radiographic images for the detection of caries and periodontal defects. However, thus far, this method failed to show a significantly improved ability for the detection of these lesions. Further investigations are necessary in order to develop specific algorithms that will increase the validity of pseudocoloring in two-dimensional imaging in dentistry.
","PeriodicalId":82453,"journal":{"name":"Refu'at ha-peh veha-shinayim (1993)","volume":"31 4","pages":"13-5, 59"},"PeriodicalIF":0.0,"publicationDate":"2014-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"32941351","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It has the potential to safely and effectively reduce pain associated with RAS. We present two patients who used ORALMEDIC® to reduce pain caused by RAS lesions. Both patients developed side effects presenting as deep ulcerations with a necrotic base at the location where ORALMEDIC® has been repeatedly applied as pain did not diminish after the first application. One patient used ORALMEDIC® several times on the same lesion. However the other patient followed the manufacturer instructions when he successively used ORALMEDIC® stick twice over the same lesion. Patients were treated with systemic antibiotics and local antiseptic agents. The healing process of the ulcers was slow and has completed its course within six weeks. It is suggested that when dental surgeons recommend ORALMEDIC®, they should emphasize the importance of using this agent according to the manufacturer's instructions in order to minimize the risk for side effects.
","PeriodicalId":82453,"journal":{"name":"Refu'at ha-peh veha-shinayim (1993)","volume":"31 4","pages":"10-2, 59"},"PeriodicalIF":0.0,"publicationDate":"2014-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"32941350","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is a bright white dot or area that appears on the surface of the maxillary central incisors and/or the gingiva of the front teeth, visible in intra-oral and smile extra-oral photographs. Recently it was found that the general population defines a nicer smile when the LRZ is located in the gingival area of the upper central incisors' surface rather than the incisal third. The aim of this study was to find out in two populations that are not aware of this new parameter, laypersons and orthodontists, whether there is a difference in the identification of the LRZ and the importance of the LRZ in determining the quality of the smile. Twelve pairs of facial photographs, including 10 of ordinary smiling persons and 2 of smiling models, were presented to 108 laypersons and 30 orthodontists. The only difference between each pair was the location of the LRZ that was moved, compared to the original photograph, gingivally or incisally respectively, using Photoshop (Adobe). Each participant was asked to mark whether he/she noticed a difference between the 2 pictures, and if so, to score the nicer smile. Data analysis was carried out using Chi square test and Fisher's exact test (SPSS v17). The results showed that both populations had difficulty in recognizing the differences between the pictures that were based solely on the LRZ parameter. However, when differences were recognized, most of the participants in both populations pointed on the smile where the LRZ was located gingivally as the nicer one. The orthodontic population recognized the difference between the two pictures, in which only the LRZ was changed, to a lesser extent compared to the laypersons populations. This result was with statistical significance (p = 0.007).
","PeriodicalId":82453,"journal":{"name":"Refu'at ha-peh veha-shinayim (1993)","volume":"31 3","pages":"26-30, 60-1"},"PeriodicalIF":0.0,"publicationDate":"2014-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"32666639","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Physicians and dentists who treat aircrews and divers are occasionally challenged by those manifestations, though their uncommon appearance. This article reviews the two main barometric-related phenomena in the oral cavity: dental barotrauma and barodontalgia. Dental barotrauma includes all barometric-related dental mechanical phenomena. Tooth fracture or failure of dental restoration usually appears in a tooth with a leaking restoration or secondary caries lesion. In addition, changes in barometric pressure can cause a reduction in the retention of dental restoration and appliance. Barodontalgia is the oral pain which evoked during changes of the atmospheric pressure. This manifestation can be classified as a direct or non-direct pain. In most cases, the direct pain is caused by deterioration of pre-existed oral disease, whereas the source of the nondirect pain is an extra-oral facial barotrauma. These two barometric-related manifestations can cause a decrease in life quality and jeopardize the safety of flight or diving.
","PeriodicalId":82453,"journal":{"name":"Refu'at ha-peh veha-shinayim (1993)","volume":"31 3","pages":"19-25, 60"},"PeriodicalIF":0.0,"publicationDate":"2014-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"32666638","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"[NUG--necrotizing ulcerative gingivitis: a review].","authors":"Y Mizrahi","doi":"","DOIUrl":"","url":null,"abstract":"

Necrotizing Ulcerative Gingivitis (NUG) is an acute and rare (0.5-11% of the population) infectious disease of the gum tissue, which is characterized by ulceration and inflammation of the inter-dental gum tissue. NUG was documented by historians since the fourth century BC, most of the reports from the ancient world were in the context of illness among fighting troops, present studies of NUG in the modern world are still common among soldiers. NUG is associated with poor oral hygiene and weakening of the host, especially in immunocompromised patients, malnutrition and poor living conditions, as well as in the context of mental stress. NUG is more common in young adults, but reports of morbidity in young children with malnutrition in the background are not uncommon. NUG diagnosis is based on three essential symptoms: sore gums, bleeding gums and the most diagnostic characteristic, ulceration and necrosis of the interdental papillae. The disease is considered to have a clear initial infectious etiology, when the main bacteria, associated with the disease, include: Bacteroides intermedius and Fusobacterium sp. The infection involves anaerobic aerobic bacteria with a majority of Gram-negative bacteria. The treatment of NUG is based on combining mechanical removal of tartar with local and systemic delivery of antimicrobial agents. Adequate treatment usually prevent the progression of the disease and ulcer healing is expected in a few days. Nevertheless, lack of treatment can lead to deterioration in the form NUP to Noma.
","PeriodicalId":82453,"journal":{"name":"Refu'at ha-peh veha-shinayim (1993)","volume":"31 3","pages":"41-7, 62"},"PeriodicalIF":0.0,"publicationDate":"2014-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"32666641","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In recent years, the role of OSAS in a vast range of morbidities has been recognized, namely metabolic, neuro-cognitive and cardio-vascular disorders, thus emphasizing the importance of early detection. The recommended initial treatment for pediatric OSAS consists of surgical removal of adenoids and tonsils. However, today it is clear that there are other factors involved in the etiology of the syndrome. An association is believed to exist between the airway and craniofacial development and vice versa. The structure of the face and jaws has been shown to play an important role in the pathophysiology of OSAS, thus, the orthodontist plays a substantial role in the diagnosis and treatment of these children. Moreover, OSAS is believed to have a direct and indirect effect on oral health. Obstructive Sleep apnea syndrome is also common in adults, but is defined as a separate entity being different in terms of pathophysiology, clinical presentation and treatment. This paper reviews the pathophysiology, diagnosis, clinical implications and available treatment options for pediatric OSAS. Moreover, we will review the impact of the condition on the structures of the face, jaws, mouth and teeth and discuss the dentist's critical role in identifying and treating these problems.
","PeriodicalId":82453,"journal":{"name":"Refu'at ha-peh veha-shinayim (1993)","volume":"31 3","pages":"48-58, 63"},"PeriodicalIF":0.0,"publicationDate":"2014-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"32666560","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"[A tooth or an implant--literature based decision making].","authors":"H Bar On, E Sharon, M Lipovezky-Adler, O Haramaty, A Smidt","doi":"","DOIUrl":"","url":null,"abstract":"

The common use of dental implants in the daily practice led to a profound change in the available treatment strategies. The option of replacing a diagnosed doubtful tooth with an implant has become widely accepted and often used. The prognosis systems in use today are based on the three major disciplines: endodontics, periodontics and prosthodontics. Combining these three may impair and bias the decision making process and increase the tendency to base it on subjective clinical experience and personal preference. Reading and reviewing the relevant literature gives no clear tool for use. Root canal treatment is considered a highly predictable treatment procedure and a treated tooth is affected mainly by the quality and type of the fabricated restoration and the risk of caries. Periodontal treatment followed by a suitable maintenance regimen will likely allow long term tooth survival. When comparing the success rates of natural teeth rehabilitation versus implant supported restorations, it appears that with implants an additional treatment is demanded along the years. This coincides with the fact that to date there is no consensus regarding the extent of perimplantitis and perimucositis that is to be expected around a restored implant. In addition, a peri implant tissue problem or a failure of a dental implant may prove to be more challenging than a failure of a tooth. It is important to remember that a dental implant is made to substitute a missing tooth and it is a treatment modality with known and clear indications for rehabilitation of an edentulous space. The aim of this paper is to review and discuss the various aspects of whether to maintain a compromised or a doubtful tooth or to prefer a treatment modality using dental implants. In conclusion it is advised here, to incorporate the discussed issues in the decision making process towards the most suitable treatment plan.
","PeriodicalId":82453,"journal":{"name":"Refu'at ha-peh veha-shinayim (1993)","volume":"31 3","pages":"7-13, 59"},"PeriodicalIF":0.0,"publicationDate":"2014-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"32666637","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Among many important properties, A FPD should be able to resist bite forces, the harsh oral environment and to remain intact for a long period of time. When coming back from the dental laboratory, the mechanical properties of the FPD are optimal. But it is not uncommon for the dentist to perform a machining procedures on the restoration in order to achieve good fitting to the prepared teeth. In most cases these modifications are made using high speed dental hand piece, and diamond burs. The surface integrity of the restoration is an important parameter that influences on the restoration strength and durability. The more smooth the restoration surface is, it is less prone to fracture, and is less prone to coloring and plaque retention. Therefore, every modification the dentist makes on the restoration is not recommended. In some cases, the adjustments are made on a FPD which is already cemented to the teeth. If the teeth are vital, the implications of the machining of the restoration are even more destructive. As a result of the friction between the bur and the restoration, the temperature rises. There is a risk of pulp necrosis in temperature above 42 degrees C.
