{"title":"The Investigation of Pastoral Care Interventions as a Treatment for Depression Among Continuing Care Retirement Community Residents","authors":"D. Baker","doi":"10.1300/J078v12n01_06","DOIUrl":"https://doi.org/10.1300/J078v12n01_06","url":null,"abstract":"ABSTRACT This prospective study provided support for the efficacy of pastoral care with older adults both as a treatment for depression and as a prophylactic to deter the potential negative impact of change and life circumstances. Subjects included forty persons taking anti-depressant medication, forty persons at risk for depression, and forty people recruited by convenience. Treatment subjects were visited by a chaplain for a 30-minute period weekly for six months. Control subjects received only minimal pastoral care. Treatment group post-test scores for depression decreased, while follow-up treatment group scores for depression increased after cessation of the intervention. Frankl's Logotherapy assists in elucidating what a crisis of meaning looks like clinically and Tillich's formulation of the anxiety of non-being is adapted for describing the ontological crisis in MDD. A case study applies the model to one individual's approach to eldership. Scholars and Biblical tradition offer explanations for this. Viktor Frankl has offered specific psychotherapeutic insights into human meaning that can be considered in the special case of nursing home elderly. However, meaning must take into account the several types of nursing home residents which include the terminal (comatose or active), the cognitively impaired, the cognitively intact but physically impaired, and the rehabilitation patient (short term). A list of potential sources of meaning to consider in preparing for ministry in nursing homes is presented. This exploration is offered as a professional tribute by a pastor whose ministry with older adults is greatly enriched by their contributions. It is also offered as a personal testimony by a middle-aged individual whose own life journey is most encouraged and challenged by their thought. It is not every philosopher/psychiatrist or theologian/psychologist who can have such significant influence on both levels of the sojourner-in-ministry. Nor is it every author, whose insights are born of specific disciplines, who can offer such stimulating insights to those who are practitioners in another field. Both Viktor Frankl and Charles Gerkin possess these capabilities in abundance. Logotherapy finds its philosophical roots in existentialism and phenomenology, its psychological roots in psychoanalysis and individual psychology, and its spiritual roots in a profound commitment to the human being as an irreducibly spiritual creature. It is perhaps in the area of the conception of the human being that Frankl’s differences with those schools of thought which constitute the roots of Logotherapy can best be distinguished. The anthropology of a therapeutic system profoundly influences the entire system. An ethnomethodological study of a CBRF (community based residential facility) with two sections, each housing about 10 persons, revealed meaning in residents' actions, feelings, and expressions of values. These individuals showed it is possible to retain what Frankl called “tragic optimism” despite cognitive deterioration. Through active engagement with their environments and with one another, a wide range of expressed emotions including happiness and humor, and caring sensitivity toward others, these elders showed that given a supportive environment, dementia does not destroy meaning. Its hermeneutical phenomenological analysis introduces an understanding of personhood which affirms one's capacity to find meaning in life, indeed, even in suffering and dying. It is this unique spiritual capacity which conveys a renewed awareness of self worth and human dignity and assists persons in understanding themselves as fashioned in the image of God.
