MATERIALS AND METHODS Some 242 solar-damaged anatomical sites on 61 human subjects were treated with three laser treatments at two study centers. The treatment involved applying a carbon suspension to the skin surface and irradiating the exogenous chromophore with a Q-switched Nd:YAG laser. Cosmetic skin-resurfacing was accomplished using a wavelength of 1064 nm, a fluence of 2.5 J/cm2, a pulse duration of 6-20 ns, a pulse repetition rate of 1-10 Hz and a fixed beam size of 7 mm. The treatment sites were evaluated at baseline, 4, 8, 14, 20 and 32 weeks for skin texture, skin elasticity and rhytid reduction. All sites were treated at baseline, 4 and 8 weeks. Skin replicas taken prior to treatment and at the conclusion of the study were analyzed for wrinkle and cosmetic improvement using the Magiscan digital image process system. Adverse events were recorded throughout the study. RESULTS At 8 months the investigators reported improvement in both skin texture and skin elasticity, as well as rhytid reduction compared to baseline. In self-assessments, subjects also reported noticeable skin texture and cosmetic improvement, but assessed wrinkle reduction less favorably than seen by the investigators. The majority of adverse events were limited to mild, brief erythema. CONCLUSIONS Topical carbon suspension assisted Q-switched Nd:YAG laser treatment offers a safe cosmetic method of resurfacing solar-damaged skin. Post-treatment erythema is the most common side effect of this method of skin resurfacing. Sublethal thermal damage to the dermis has been proposed as an etiology for post laser erythema. Recent developments in laser resurfacing technology have aimed at minimizing thermal damage to the dermis. OBJECTIVE To determine depth of skin ablation, extent of thermal injury, and ideal laser parameters for the FeatherTouch laser system. To assess the safety and efficacy of laser resurfacing at high energy fluences with a single pass. DESIGN Laser resurfacing was performed in the preauricular skin of five patients undergoing rhytidectomy. A total of 60 sites were tested with fluences of 7 to 17 Joules/cm2. Histologic evaluation of excised skin showed maximal thermal injury to be restricted to 60 microns in the papillary dermis. The reticular dermis showed no evidence of injury. Based on these findings, laser resurfacing at 17 J/cm2 (70 watts) was performed on 30 patients (in the periorbital area, a maximum of 9 J/cm2 or 36 watts was used). Follow up ranged between 12 and 18 months. RESULTS Based on histologic comparison of average and high fluence laser resurfacing, high fluence laser resurfacing did not cause added thermal damage to the reticular dermis. In the clinical group, no major complications such as scarring, scleral show, infection or ectropion were encountered. Transient hyperpigmentation was noted in three patients. Overall patient satisfaction was good to excellent. Post-treatment erythema lasted an average of 4 weeks. CONCLUSION We conclude that CO2 laser resurfacing of the face (excluding the periorbital region) can be performed safely and effectively, with the FeatherTouch laser, at 17 J/cm2 with one pass. It is cosmetically disfiguring and can be the cause of significant morbidity. In particular, the affected skin is often red or purple due to increased vasculature. It is particularly resistant to both surgical and medical therapy. We describe a patient with lupus pernio affecting her nose, which showed a dramatic improvement following treatment with the pulse dye laser (PDL). A biopsy taken after treatment showed the continued presence of sarcoidal granulomas. It is broadly divided into congenital and acquired forms based upon the time of presentation. To date no effective treatment has been reported for this condition. In view of the vascular component of this eruption, it was felt that the pulsed dye laser (PDL), 585 nm, may have a role to play in the selective photothermolysis of these lesions. The treatment of five patients with acquired UNTS using the PDL is described. Based upon the findings it is concluded that the PDL is an effective treatment for UNTS; however, the clinical response is short lived with a 100% recurrence noted in all cases. Selective absorption of hair chromophores from lasers and broad band light sources results in destruction of hair follicles while leaving the skin undamaged. A discussion of the basic principles of selective photothermolysis as it applies to hair removal by lasers and light sources is presented, followed by a comparative review of three melanin target based systems: Ruby laser, Alexandrite laser, and a broad band intense pulsed light. These systems are efficient and safe with proper patient selection. Peer-reviewed scientific data for many of the hair removal laser systems is lacking. OBJECTIVE This paper provides a chronicle of 24 of the 31 patients who participated in the original 3-month trial for hair removal utilizing an intense pulsed light source. METHODS A total of 24 of the original 31 patients who took part in the one treatment, 3-month, intense pulsed light trial were examined again at 1 year following the treatment. RESULTS Long-term epilation of 75% hair removal was found in this group of patients after 1 year with a single treatment.
