We report a case of asymptomatic advanced snowflake opacification presenting 13 years after implantation who maintained a visual acuity of 6/6. Primary tumor was found in the lung and choroidal masses in both eyes. Anterior segment neovascularization causing intraocular pressure elevation was the only presenting sign of metastatic choroidal tumor. Six eyes of six patients with severe limbal stem cell deficiency were included. All patients were started 0.1 mg x kg 1 x day(-1) tacrolimus orally 3 days before the surgery. Acute vitreomacular traction was observed in two eyes (3.3%) at the first POD. Spontaneous resolution occurred in both eyes within one week.
