However, nurses, as enlightened patient advocates, also have an entrusted interest in fully understanding the legal and ethical considerations of the informed consent process. Some of the ethical principles impacting informed consent are existing cornerstones of professional nursing practice. Nevertheless, surgeons bear the ultimate responsibility for the informed consent process before surgery. Nevertheless, few studies have explored nurses' attitudes toward PDD. This pilot project sets out to fill the gap regarding nurses' attitudes and values regarding PDD and perceptions of consistency or inconsistency with the American Nurses Association (2001) Code of Ethics for Nurses With Interpretive Statements (The Code). Thirteen subjects self-selected from a population of registrants attending a national ethics conference by completing an anonymous descriptive survey. The majority of subjects noted that their personal and professional values related to PDD are in agreement (regardless of their specific position). The subjects were divided on whether PDD is ethically consistent with The Code. Despite being unsure if PDD is ethically consistent with The Code, these nurses found relief of suffering to be a more compelling rationale for PDD over patient autonomy. If not addressed, it may create job dissatisfaction, withdrawal from the moral dimensions of patient care, or even leaving the profession. Using the 21-Moral Distress Scale-Revised to assess moral distress, 323 surveys were received from 5 healthcare disciplines. {"title":"Advanced practice registered nurse intended actions toward patient-directed dying.","authors":"Jessica Jannette,&nbsp;Marcia Sue DeWolf Bosek,&nbsp;Betty Rambur","doi":"10.1097/NHL.0b013e3182919e81","DOIUrl":"https://doi.org/10.1097/NHL.0b013e3182919e81","url":null,"abstract":"<p><strong>Unlabelled: </strong>Patient-directed dying (PDD) (also known as physician-assisted suicide) has been a heavily debated issue in the United States since the passing of the Death With Dignity Act in Oregon in 1997. Previous research surrounding PDD has been limited to assessing the attitudes of physicians, nurses, and patients. The purpose of this study was to describe the intended actions of advanced practice registered nurses (APRNs) toward initiating PDD discussions and prescribing a lethal dose of medication under PDD legislation. A survey design was used to guide this pilot study. An investigator-developed questionnaire was disseminated electronically to APRNs on a free professional Listserve in a rural northeastern state. The final sample was composed of 16 APRNs whose area of certification were identified as 63% family, 31% adult, 6% psychiatric/mental health, and 13% other. Hospice and palliative care employment experience was reported by 75% of the sample.</p><p><strong>Results: </strong>The APRN subjects were more willing to engage in PDD discussions than to prescribe under PDD legislation. The APRNs reported an increased willingness to initiate discussions and prescribe when patient pain and suffering were explicitly stated. Seventy-five percent of APRNs viewed their personal and professional opinions regarding PDD to be synonymous even though 50% were unsure as to whether PDD was consistent with the American Nurses Association (2001) Code of Ethics for Nurses.</p><p><strong>Conclusions: </strong>Advanced practice registered nurses reported increased intent to discuss than actively participate through means of prescriptive authority under PDD legislation. Whitney, EdD, RN, CNS At the monthly interdisciplinary team conference, nurse practitioner Amy Jones discussed with the team: I have been approached by a woman who read the article in The New York Times about brides using 500 kcal/d tube feedings as a way to lose weight before their wedding. She asked whether our clinic provided this service and was asking questions about whether tube feedings would be a viable weight reduction option for her since her wedding is a just over 6 weeks away, and she is still unable to fit into her grandmother’s wedding dress. She described herself as being 5 ft 4 in and 168 lb. According to the weight chart, her ideal weight would be 108 to 132 lb, and a healthy weight would be up to 145 lb. I was really caught off guard. I had no clue that other clinics were engaging in this practice. I told her I would get back to her after I learned more about this practice. I would like to propose that we seek some consultation to help us better understand the ethical and clinical aspects of this clinical issue before we decide what our clinic’s policy should be. {"title":"Horizontal hostility: a threat to patient safety.","authors":"Barbara L Wilson,&nbsp;Connie Phelps","doi":"10.1097/NHL.0b013e3182861503","DOIUrl":"https://doi.org/10.1097/NHL.0b013e3182861503","url":null,"abstract":"<p><strong>Objective: </strong>The objective of this study was to determine the perceived level of horizontal hostility (HH) in a 220-bed acute care community hospital and whether the threat of or experience with HH influenced nurse behaviors directly related to patient safety.</p><p><strong>Background: </strong>While the acknowledgement and presence of HH in nursing are gaining prominence, little is known about how a nurse's experience with HH directly influences his/her actions with patients under their care, even when the nurse realizes these actions may not be in the patient's best interest.</p><p><strong>Methods: </strong>We used a 28-item survey tool aimed at determining the level of perceived HH in an acute care Magnet-aspiring hospital in the Southwest and then asked about nurses' actions as a result of that experience. Almost 500 nurses were surveyed over a 2-month period in 2011.</p><p><strong>Results: </strong>Of the nurses who had personally experienced HH, a high number reported performing interventions or actions that could compromise patient care and/or safety, including (a) failing to clarify an unreadable order, (b) lifting or ambulating heavy or debilitated patients without assistance rather than asking for help, (c) using an unfamiliar piece of equipment without asking for clarification, and (d) carrying out an order that the nurse did not believe was in the best interest of the patient, among other behaviors.</p><p><strong>Conclusion: </strong>The presence of HH has clear implications for patient safety.
