{"title":"The psychological assessment of patients with chronic pain.","authors":"A H Lebovits","doi":"10.1007/s11916-000-0045-z","DOIUrl":"https://doi.org/10.1007/s11916-000-0045-z","url":null,"abstract":"<p><p>This article reviews the objectives of psychological evaluations, as well as the standard pain center evaluation protocol that uses a pain questionnaire, a structured clinical interview, and pain assessment measures that include pain intensity rating scales and the McGill Pain Questionnaire. The most frequently used measures of psychological status, such as the Beck Depression Inventory and the Minnesota Multiphasic Personality Inventory (MMPI), are reviewed. Psychological predictors of invasive procedures and of disability are also outlined. A search of PubMed, the online site of the National Library of Medicine, for papers on RSD reveals nearly 2200 articles on the topic (using algodystrophy as the search word, wherein RSD references are also included, and more older and European articles are also listed). From 1991 through 1998 inclusive there is an average of nearly 100 articles per year on the topic, which represents more than a third of all the articles referenced since 1965. In the decade of the 1980s, there is an average of 64 articles per year, 74 per year in the last half of the decade and 54 per year in the first half. Prior to the decade of the 1980s, one finds an average of 40 articles per year back to the mid-1960s. The controversy surrounding the disorder centers around the nature of the problem and whether it is a primary organic disorder or a primary psychogenic disorder associated with the accomplishment of some secondary gain. If it is the former, then clearly research should continue to determine the nature and etiology of the malfunctioning organ(s). If, on the other hand, RSD is a psychogenic disorder, then the medical community does well to focus mainly on the peripheral manifestations of the problem. First seen as an alternative to dying in the impersonal, highly technical environment of the hospital, it provides comprehensive palliative, supportive care in patient-oriented settings, usually at home or, in some instances, special hospice units. Now the challenges of hospice care largely go beyond the philosophic to include the organizational, demographic, and pragmatic difficulties of providing such care in various settings. The etiology and pathophysiology of pain is still far from being known, but several important neurobiological aspects have emerged in the last decade. This article reviews the present knowledge on three clinically relevant topics in TMD: referred pain mechanisms, somatosensory changes, and sensory-motor integration in the craniofacial region. The complexity of the clinical problem, the multidimensional nature of chronic pain, and general lack of rigorous study design, however, have obscured outcome assessment and hampered efforts to optimize patient selection criteria. Recent work has focused on refinement of existing therapies for FBSS and identification of factors that influence outcome and improve patient selection criteria.
