{"title":"New York's Medicare Marketplace: examining new York's Medicare advantage plan landscape in light of payment reform.","authors":"Doug Goggin-Callahaan, Joe Baker, Rachel Bennett, Michell Clerk, Eric Hersey, Fred Riccardi, May Torbattejad, Denise Xu","doi":"10.1891/1521-0987.14.3.187","DOIUrl":"https://doi.org/10.1891/1521-0987.14.3.187","url":null,"abstract":"<p><p>The Patient Protection and Affordable Care Act (ACA) provided for cost savings in the Medicare program, in part to underwrite coverage expansion to Medicare beneficiaries, to finance new coverage for those not eligible for Medicare, and to strengthen Medicare's financial outlook. One cost-saving measure, a reformulation and reduction in payments to private health insurance plans that provide Medicare benefits through the Medicare Advantage (MA) program, had a sound policy basis but was criticized, particularly by opponents o fthe ACA, as a measure that would lead to increased costs, reductions in benefits, and diminished plan choices to Medicare beneficiaries enrolled in MA plans. Despite dire predictions to this effect, a review of a sample of MA plan offerings in New York State in 2012 shows that Medicare beneficiaries enrolled in such plans did not experience significant benefit reductions or increased costs. While the number of plan offerings decreased, the reduction was mostly caused by the elimination of duplicative plan choices in 2011. Although the MA plan executives we interviewed indicated that further reductions in plan reimbursement in future years-tempered by potential bonus payments for meeting quality and performance metrics-could impact plan costs and benefits, they believed plans will employ a number of strategies to remain in the market and maintain benefciary benefits and cost structures. This is because of the lack of classification of variables related to the care receiver and to the demographic and organizational context in which it is inserted.</p><p><strong>Methods: </strong>The 126 organizations in the central region of Portugal were categorized into four groups depending on whether they were located in a rural or urban environment and on whether they were large or small organizations. To obtain information, the In-Home Care Protocol (ProSAD), Elderly Assessment System (EASYcare), and the Center for Epidemiologic Studies-Depression (CES-D) scale were applied to 48 customers (6 randomly chosen customers of in-home care services of each of the 8 randomly selected organizations, 2 per group of variables).</p><p><strong>Results: </strong>The rural context denoted a lack of diversity of services and the number of organizations available is reduced which implies less time spent with the customers. The more dependent customers at the time of registration (Kruskal-Wallis test [KW] = 12.79; p < .05) in large organizations (Mann-Whitney [U] = 190.5; p < .05) benefit more from the services.</p><p><strong>Conclusions: </strong>In-home care services are underused and are oriented to treat those that have a family caregiver. {"title":"Case management: the allocation of hours and influencing factors in the management of survivors of traumatic brain injury.","authors":"Susan Arnold, Meagan Elder","doi":"10.1891/1521-0987.14.1.2","DOIUrl":"https://doi.org/10.1891/1521-0987.14.1.2","url":null,"abstract":"<p><p>Case management is a service frequently provided to clients following a traumatic brain injury (TBI); however, the time required for this service is uncertain. To identify factors that influence case management for clients with TBI, a descriptive analysis of 64 client files were provided with case management services of a rehabilitation provider based in Sydney, Australia. These files were then further examined by using qualitative measures to determine the relationship among case management factors, time required and the severity of the injury. This information was statistically analyzed to identify what factors predicted the greatest use of case management time. The analysis identified inappropriate behavior, family dynamics, accommodation, and formal relationships as primary affectors on case management time. Other issues identified as less demanding of case management time were sustaining employment and provision of equipment. In terms of case management hours, the mean was 2.5 hr/ month. Where there was a combination of three factors, the time for case management increased to a mean of 3.97 hr, and up to 4.34 hr/month (range: 1.5-14.0 hr) of case management time where 4 factors were identified.
