A model is presented for the development of a crisis team without additional funding, utilizing half-day shifts contributed by clinical social workers who hold full-time positions elsewhere in the hospital. Effective and efficient emergency clinical work required that the social workers be taught about biomedical issues pertinent to presentations, along with instruction as to why and when to seek consultation from backup psychiatrists. {"title":"The psychiatric emergency/crisis disposition and community networks.","authors":"D T Campos, M T Gieser","doi":"10.1300/j261v03n01_09","DOIUrl":"https://doi.org/10.1300/j261v03n01_09","url":null,"abstract":"<p><p>Recent advances in techniques of rapid neuroleptization have enabled psychiatric emergency/crisis intervention staff to stabilize many acutely ill patients in a brief time period, frequently less than 24 hours. This has resulted in less need for hospital admissions and has challenged the mental health field to develop alternative disposition options. These options can be classified through a \"hierarchy of crisis placements.\" From least to most restrictive in dispositional context these are: (1) the patient's family, (2) emergency housing, (3) a foster home, (4) the crisis hostel, (5) the 24-hour holding bed facility or intensive observation apartment, (6) the crisis bed unit, and (7) inpatient hospitalization. The psychiatric emergency/crisis intervention unit serves as a gateway for these dispositions following emergency treatment. Thus, a primary function of the unit is the advocacy for, and coordination of, dispositional services, including those provided by a variety of community resources. {"title":"An aid for diagnosing acute ischemic heart disease in the emergency room.","authors":"R B D'Agostino, P A Sytkowski, M W Pozen, H Selker","doi":"10.1300/j261v02n02_03","DOIUrl":"https://doi.org/10.1300/j261v02n02_03","url":null,"abstract":"<p><p>A five-year study was undertaken to develop a valid mathematical model that could aid in diagnosing acute ischemic heart disease in the emergency room, thus reducing inappropriate admissions to the coronary care unit. The study was divided into two substudies. In the first, variables significantly predictive of ischemic heart disease were identified and a logistic function was developed and tested. In the second, a six-hospital study, the variables of the first substudy were validated and a final logistic regression was developed and tested prospectively.
