Quality, as defined by Donabedian, includes assessment of structure, process, and outcomes. However, the definition of quality in health care must be expanded to include the expectations and opinions of patients, their representatives, and society. The purpose of this article is to examine the outcome variable of costs in a specific nursing practice setting. Cost is frequently defined as the judicious use of resources consumed by structures and processes of care. This article presents activity-based costing methodology and results of a cost study of primary care and mental health services provided by advanced practice nurses (APNs) in a school-based health center. The application of the methods and outcomes of this pilot study has significant implications for the delivery of health care by APNs in a variety of settings, including community nursing centers, freestanding birthing centers, and rural and urban neighborhood centers. As part of the survival process, institutions must demonstrate--to the public and to regulatory agencies--that they are measuring the effect of their care structures and processes. Institutions must be willing to share these measurements with the public as well as to use them internally to identify performance problems and document the impact of process improvements or other changes. The report card is a simple, easily understood method for reporting quality data and comparing varying aspects of patient care processes and outcomes. However, as measurements, report cards are not robust enough to serve as the sole source of quality-related data, and interinstitutional comparisons may be misleading. It is essential for health care providers to determine the effectiveness of their care and to institute changes in practice to affect patient outcomes in a positive fashion. Determining the appropriate outcomes to measure and formatting the report in an easy-to-read manner with meaningful information is important for presentation to a variety of audiences. This article examines the concept of patient outcomes and relates them to an actual example of one condition and related medical and nursing diagnoses. The importance of measurements in the overall care delivery process and the documentation of discharge outcomes are also discussed. {"title":"Measuring the effectiveness of nursing care.","authors":"K D Marek","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Identification and measurement of the outcomes of nursing care have received great attention in the past decade. With advances in information technology, large clinical data bases are being developed for use in outcomes evaluation. It is important that nurses be thoughtful in designing the data elements that will be included in these data bases. Key data elements include nursing diagnoses, interventions, and nursing sensitive outcomes. Multiple variables influence the outcome of health care. It is important in evaluation that system, patient, and provider characteristics be considered, especially when developing benchmarks for comparison of outcome achievement. Data related to the process of care are needed beyond provider encounter, and nurses must be cautious that the care they give is not credited to other providers. This article reports on two preliminary studies and one major study of the Patient Satisfaction with Nursing Care Questionnaire.
