{"title":"Clinical pathways: a tool to evaluate clinical learning.","authors":"M J Bradshaw","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Clinical pathways are a means by which an instructor can objectively and effectively evaluate student learning and progress toward clinical outcomes. An advantage to use of pathways in one-time experiences is that the pathway serves as a criterion-based frame of reference for both student and instructor, since the criteria are the same as for other clinical experiences in that course. The faculty member thus has an objective measure of student learning and performance, and the student always knows the measure on which she or he will be evaluated. Clinical pathways are limited to brief experiences and are not designed to show professional growth and progress in learning over time. A pathway could be designed, however, to appraise critical thinking and professional behaviors associated with spontaneous incidents, such as a problem patient. {"title":"The effectiveness of a standardized educational program for children using patient-controlled analgesia.","authors":"A M Kotzer,&nbsp;J Coy,&nbsp;A D LeClaire","doi":"10.1111/j.1744-6155.1998.tb00217.x","DOIUrl":"https://doi.org/10.1111/j.1744-6155.1998.tb00217.x","url":null,"abstract":"<p><strong>Purpose: </strong>To examine the effectiveness of two types of preoperative education (routine education and a standardized educational program) for children undergoing spinal fusion.</p><p><strong>Design: </strong>Two group, phase-lag design.</p><p><strong>Setting: </strong>Tertiary pediatric hospital.</p><p><strong>Participants: </strong>Children ages 8-18 years (N = 93).</p><p><strong>Main outcome measures: </strong>Adolescent Pediatric Pain Tool, Child Pain Scale, Post-PCA Satisfaction Interview, and PCA infusion pump data.</p><p><strong>Results: </strong>No statistically significant differences between the groups on any of the main outcome variables. Children and parents reported, however, that the SEP provided them with invaluable information regarding the use of PCA and alleviated their concerns about getting \"hooked on drugs,\" overdosing, side effects, and being able to get pain relief when needed.</p><p><strong>Conclusion: </strong>Children having spine fusion surgery experienced severe postoperative pain that was not ameliorated by optimizing use of PCA through standardized education. {"title":"Stress and coping behaviors of substance-abusing mothers.","authors":"S J Kelley","doi":"10.1111/j.1744-6155.1998.tb00215.x","DOIUrl":"https://doi.org/10.1111/j.1744-6155.1998.tb00215.x","url":null,"abstract":"<p><strong>Purpose: </strong>To examine parenting stress and coping behaviors in substance-abusing and non-substance-abusing mothers.</p><p><strong>Design: </strong>A comparative descriptive design.</p><p><strong>Setting: </strong>Pediatric primary care clinic.</p><p><strong>Participants: </strong>Low-income, predominantly African-American mothers (N = 60) of young children recruited from a pediatric primary care clinic. Thirty mothers were known substance abusers and 30 had no known history of substance abuse.</p><p><strong>Main outcome measures: </strong>Parenting Stress Index/Short Form (PSI/SF) and Child Protective Service (CPS) validation of abuse or neglect.</p><p><strong>Results: </strong>Substance-abusing mothers scored significantly higher than comparison mothers on total stress and the three subscales of the PSI/SF: parent-child dysfunctional interaction, difficult child, and parental distress. Forty-seven percent of substance-abusing mothers scored in the clinical range on total stress compared with only 3.3% of non-substance-abusing mothers. Proportionately more substance-abusing mothers than comparisons demonstrated maladaptive parenting behaviours as evidenced by CPS-confirmed abuse or neglect of their youngest child.</p><p><strong>Conclusions: </strong>Substance-abusing mothers of young children are at increased risk for increased levels of stress and maladaptive coping behaviors.
