Her chief complaint was effort angina. The coronary angiography showed obstruction of left coronary ostium with intact main stem and its branches, perfused from right coronary artery and the aortogram revealed aortic regurgitation of grade 3-4. Left Valsalva sinus was obstructed by adhesion of left coronary cusp. Each coronary cusps were resected, and aortic valve replacement with 23 mm SJM valve and coronary arterial bypass by means of in-situ graft of left internal thoracic artery underwent successfully. Nonbronchial systemic collateral arteries and pulmonary arteries may contribute significantly to pulmonary hemorrhage. In recent literature, the former embolization is widely accepted, but embolization of the latter has not been stressed, excluding the patients of AV malformation or pulmonary aneurysm. The recognitions of the precice bronchial artery anatomy and numerous collateral vessels are essential for successful embolotherapy. This distal end is tapered, therefore its outer diameter decreased to 1.1 mm. It has an inflatable balloon at the distal tip and four circular channels. Through one of the lumina, 0.014 inch PTCA guide wire can be used. The steerable guide wire enable the angioscope to be inserted to the target lesion safely and accurately. Recently we investigated the appearance of coronary artery in acute coronary syndromes. The results have indicated that thrombi, intimal irregularities, and xanthomatous atheromas were observed more frequently in patients with acute myocardial infarction, recent myocardial infarction and unstable angina. It is concluded that a thrombus overlying a rupture in the lining of plaque plays a major role in an acute coronary disorders, and that the fragile, lipid-rich gruel atheroma may procede its rupture. Coaxial alignments of the coronary artery were obtained in more than 80% of attempted patients. However, a finer controllable distal tip to allow good coaxial alignment and a larger balloon to reduce the coronary good flow and make the angioscopic catheter easier to track, are necessary for more complete visualization.</p>","PeriodicalId":79253,"journal":{"name":"Rinsho kyobu geka = Japanese annals of thoracic surgery","volume":"14 2","pages":"105-9"},"PeriodicalIF":0.0,"publicationDate":"1994-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"20350742","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The rate of initial success in PTCA elevated up to more than 90% recently, in accordance with the progress in technology and the operator's skill. The limitations of PTCA are acute coronary occlusion which occurs in a few percent during or just after the procedure, and restenosis which occurs in thirty to fourty percent within six months. Some new devices have been invented to add better results for the patients. Directional coronary atherectomy (DCA) is one of the devices to remove atheroma in narrow segment, instead of splitting it. DCA can create a larger and smoother lumen than that created with PTCA, and it may be expected to reduce the restenosis rate. The therapeutic choice in IHD is still controversial. At present, several clinical trials are being conducted in order to evaluate and compare medical therapy, CABG, and PTCA. These results will provide informations to help the decision making in the management of the patients with IHD.</p>","PeriodicalId":79253,"journal":{"name":"Rinsho kyobu geka = Japanese annals of thoracic surgery","volume":"14 2","pages":"115-20"},"PeriodicalIF":0.0,"publicationDate":"1994-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"20352042","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
