Recently the inhalation of nitric oxide (NO) has been revealed to produce selective pulmonary vasodilatation, and it has come to be applied in the clinical setting. We experienced a case of 6-year-old boy with complex cardiac anomaly in whom the inhalation of nitric oxide was remarkably effective for the LOS caused by increased pulmonary vascular resistance after Fontan type operation. In the case various conventional treatments failed to improve his LOS during the weaning from a respirator on the second postoperative day. After the initiation of the inhalation of nitric oxide at a concentration of 6ppm, a rapid decreasing of pulmonary artery pressure concomitant with an improvement of hemodynamics were obtained. We concluded that the inhalation of nitric oxide is an effective strategy for the increased pulmonary resistance after Fontan type operation.</p>","PeriodicalId":79253,"journal":{"name":"Rinsho kyobu geka = Japanese annals of thoracic surgery","volume":"14 4","pages":"323-6"},"PeriodicalIF":0.0,"publicationDate":"1994-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"20351126","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Valve replacement was performed in 59 pts (mechanical valve replacement; 42 pts, porcine valve replacement; 12 pts), and 2 pts received OMC due to restenosis after CMC. Preoperative diagnosis included restenosis after CMC, OMC or valvuloplasty in 43 pts, bioprosthetic dysfunction after MVR in 10 pts, perivalvular leakage after AVR in 1 pt, valve detachment after AVR in 4 pts (Beçhet 3 pts, infective endocarditis 1 pts). Valve replacement or valvuloplasty was done for another valves in addition to the previously operated valve in 44 pts (72.1%), especially for tricuspid valve. Restenosis after CMC, occurred at 20.6 years after surgery, and restenosis after OMC and valvuloplasty at 13.1 years. Bioprosthetic dysfunction after MVR occurred at 8.5 years. Valve dysfunction of the mechanical valve was not observed. The early operative mortality after reoperations was 11.5% (7 pts) in this series. And 3 pts (4.9%) died in the late follow-up due to cancer in 1 pt and congestive heart failure in 2 pts. A first choice of mechanical valve for redo valve replacement for aortic/mitral valve, and modified AVR using composite graft and proximal double fixation to the fragile annulus seemed to be satisfactory to accomplish better operative results.</p>","PeriodicalId":79253,"journal":{"name":"Rinsho kyobu geka = Japanese annals of thoracic surgery","volume":"14 4","pages":"287-91"},"PeriodicalIF":0.0,"publicationDate":"1994-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"20350550","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Two murine anti-ID MoAbs (2H1 and 2B12) were established by hybridoma technology. They showed specific reactivity with 4G12 but not with 3H12 human MoAb, human IgM and human IgG. These two MoAbs demonstrated more than 90% of binding inhibition of 4G12 to lung squamous cell cancer cell line (PC10). Moreover, cross inhibition test showed that 2H1 and 2B12 detect different idiotypes of 4G12 each other. Two groups and another control group-receiving no medication-(N = 10, Group III) were compared with respect to the changes in hemodynamic parameters, especially systemic vascular resistance (SVR). After propofol, SVR decreased from 2489 +/- 302 to 1594 +/- 286 dyn sec cm-5 and remained significantly less than the control values until 16.6 +/- 4 min. after the administration of propofol. Following thiopentone administration, SVR decreased from 2653 +/- 298 to 2162 +/- 279 dyn sec cm-5 and remained so for the following 9.4 +/- 3 min. There were more significant decreases in especially cardiac index, perfusion pressure and SVR in the propofol group compared with thiopentone or control groups.
