The type of bacteria classified according to Runyon's criteria was Group 3 (M. avium complex). The bacilli had the resistance to most antituberculotics. One of them had the history of pulmonary tuberculosis complicated with giant cavity in left upper lobe and extensive hard pleural adhesion associated with restrictive ventilatory impairment. Another one had the history or right upper lobe lobectomy for tuberculosis complicated extensive pleural adhesion. A combination of thoracoplasty and cavernoplasty was applied to these two cases whose operation were once to the former and twice to the latter respectively, with satisfactory results. The patients show complete closure of the cavity and bulla and have no symptoms of recurrence after surgery. The application of thoracoplasty and cavernoplasty is useful and safe to the treatment of patients suffering from atypical mycobacteriosis of the lung in cases of prospected tough resection, with expecting one stage cure.</p>","PeriodicalId":79253,"journal":{"name":"Rinsho kyobu geka = Japanese annals of thoracic surgery","volume":"14 5","pages":"413-6"},"PeriodicalIF":0.0,"publicationDate":"1994-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"20353010","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Dealing with perioperative complications in lung-resection surgery, it must be taken into account that lung resection always results in, more or less, loss of cardiopulmonary function and that most perioperative complications are caused by accidental circumstances. Perioperative risk must be assessed based upon the predicted postoperative cardio-pulmonary function, but not the frequency of perioperative complications. The role of unilateral pulmonary artery occlusion test and exercise test in lung-resection surgery, and their pathophysiological implications are discussed.</p>","PeriodicalId":79253,"journal":{"name":"Rinsho kyobu geka = Japanese annals of thoracic surgery","volume":"14 5","pages":"367-70"},"PeriodicalIF":0.0,"publicationDate":"1994-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"20351128","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Five patients had lung cancer, and 2 had malignant thymoma. During operation, total clamping of the superior vena cava was required in 2 patients, and partial clamping of the superior vena cava or clamping of the innominate vein in 5. The reconstruction procedures were as follows; the superior vena cava or bilateral innominate veins with a graft in 4 patients, bilateral innominate veins with 2 grafts in 1, and one of the innominate veins with a graft in 2. There was no operative death. Edema of face, neck and arms appeared in 6 patients postoperatively, but it disappeared in 3-10 days. Phlebogram which was obtained in 5 patients demonstrated patency of the grafts in 4 and well-developed collateral drainage in 1. Postoperative survival time was 1-27 months, and 3 patients are alive. No superior vena cava syndrome was recognized in long term observation of any patient.</p>","PeriodicalId":79253,"journal":{"name":"Rinsho kyobu geka = Japanese annals of thoracic surgery","volume":"14 5","pages":"409-12"},"PeriodicalIF":0.0,"publicationDate":"1994-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"20353009","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Patients who are older, more debilitated, and in unstable condition are now considered acceptable surgical candidates. 355 cases of ischemic heart disease, which the author experienced at Haut-Lèvêque hospital (University of Bordeaux, France), were reported in this article. 349 cases underwent coronary artery bypass grafting, and 6 underwent other surgical procedures containing heart transplantation (4 patients). Multiple revascularization and associated procedures are performed for scheduled patients, while resuscitative interventions for emergency cases with life-threatening complications secondary to acute myocardial infarction. Today, heart transplantation is performed with well-established techniques in the world, but not yet in Japan. Actually, however, we have many patients with deteriorated ventricular function secondary to ischemic heart disease, who have no other solutions but heart transplantation, and the number of the patients is increasing year after year. Therefore, the resumption of heart transplantation should contribute to survival in the majority of patients now suffering from severely compromised cardiac function.</p>","PeriodicalId":79253,"journal":{"name":"Rinsho kyobu geka = Japanese annals of thoracic surgery","volume":"14 5","pages":"403-7"},"PeriodicalIF":0.0,"publicationDate":"1994-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"20353008","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The indications for re-surgery were malfunction of mechanical valve in the aortic position, primary tissue failure of biological valve in the mitral position, deterioration of valvular disease and prosthetic valve endocarditis. Re-surgery was performed ranging from 4 to 15 years, with a mean period of 9.5 years, following initial operation. Early operative mortality was 5.5% (2 cases) and late mortality 8.3% (3 cases). There were no cardiac deaths during the late postoperative period.
