These defects have a vast etiology, such as traumatic, thermal or electrical burns, benign and malignant or congenital tumor resections, radiotherapy treatments sequelae, and infections. Noting that injuries such as scalping and burns (thermal or electrical), generate significant repercussions such as severe tissue loss, chronic osteomyelitis or minor sequelae such as scar alopecia. This study aims to report a case of late scalp reconstruction with a tissue expander and posterior advancement flap, due to cicatricial alopecia, in an 11-year-old female, victim of scalding by hot water in the right frontotemporal region. Methods: It was performed a retrospective analysis of the patient’s medical record. The present work follows the standards of the Helsinki ethics committee. Conclusion: The scalp tissue expansion technique by stages and subsequent scalp advancement flap performing proved to be effective in restoring the patient’s hair structure and hairline with minimal local distortion, restoring the scalp’s shape and aesthetics of the patient. ■ ABSTRACT","PeriodicalId":79099,"journal":{"name":"Revista brasileira de cirurgia","volume":"36 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"71174958","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The current parameters (power and energy) in the devices allow only an indirect inference of skin temperature change. Methods: Eighty-three patients, between 17 and 75 years old, were submitted to laser lipolysis with 980nm diode, using a Flir® T540 thermocamera for temperature monitoring. Lipolysis was obtained by applying the laser to the deep subcutaneous layer and skin retraction in the deep dermis, with a velocity of 5 centimeters per second. The laser was used in the deep dermis of the abdominal flaps of patients undergoing dermolipectomy to determine the temperature that causes skin burns, totaling 27 cases. Results: A power of 15W was used on the medial side of the arm and thigh, and 4000J average energy was applied per region. In the dorsal area and abdomen, 20W was adopted, and 6000J were applied in each quadrant. On average, the studied tissue’s initial temperature was 31°C, and at the end of the laser application was 37°C. The temperature that caused the burn was 45°C. Conclusion: The use of the thermocamera allowed a more homogeneous distribution of the laser and provided the surgeon with effective monitoring so that an adequate dose of energy is applied to the subcutaneous and deep dermis, obtaining the desired effect. ■ ABSTRACT","PeriodicalId":79099,"journal":{"name":"Revista brasileira de cirurgia","volume":"36 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"71174480","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This reconstruction becomes more complex when the upper and lower eyelids are resected to total thickness, and the eyeball is preserved. The objective is to report the techniques used for periorbital reconstruction in bipalpebral resection cases (upper and lower) with preservation of the eyeball by the Group of Orthopalpebral Surgery of the Division of Plastic Surgery and Burns of HCFMUSP. Methods: All cases that met the inclusion criteria from 2000 to 2019 were reviewed, and epidemiological, surgical, and postoperative follow-up data were described. Results: Only two cases were submitted to total exeresis of the periorbital tissue and remained susceptible to eyeball preservation. Both were reconstructed with frontal flaps with complete occlusion of the eyeball at the first moment, followed by the release in stages, keeping the globe viable after reconstruction completion. Discussion: Similar reports are scarce in the literature, and, in these cases, surgical results were functionally acceptable but with significant aesthetic limitations. Conclusion: We suggest a new option for total reconstruction of the upper and lower eyelid (with a single pedicular flap, devoid of connective graft and in multiple stages) that protects and conserves the eyeball during the various stages of surgery. The results were functionally favorable, considering the severity of the cases. ■ ABSTRACT","PeriodicalId":79099,"journal":{"name":"Revista brasileira de cirurgia","volume":"36 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"71174612","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

O objetivo é analisara evolução técnica da abdominoplastia circunferencial simples e composta e suas complicações. Métodos: Foram avaliados 29 pacientes, sendo 28 do sexo feminino, com média etária de 41,17 anos, submetidos à abdominoplastia circunferencial, entre 2002 e 2012. Este estudo retrospectivo, realizado através de dados colhidos dos prontuários médicos, avaliou: tempo de cirurgia, peso da peça cirúrgica ressecada, tempo de internação hospitalar, antibioticoterapia utilizada, complicações associadas e alterações ocorridas na técnica operatória neste período. Resultados: A abdominoplastia circunferencial composta foi realizada em 23 pacientes (79,3%) e a simples em seis (20,7%). O tempo cirúrgico médio foi de 346 minutos e o peso médio da peça operatória foi 4323 gramas. Individuals with different needs and sociodemographic profiles have used similar means to search for medical service providers, modifying consumer relations. Thus, is necessary a more detailed analysis of consumers’ characteristics of health services related to plastic surgery and the factors that impact their decision-making, since patients and their families need to feel safe when seeking medical treatment, and professionals should be the facilitators of this process. Methods: This is a real-life, prospective, observational, and descriptive study conducted in a single hospital center. We included in the sample 501 patients submitted to plastic surgeries, from June to November 2017, performed exclusively by specialist surgeons by the Sociedade Brasileira de Cirurgia Plástica (SBCP). Sociodemographic characteristics, life habits, and criteria for the surgeon’s choice were evaluated. Results: The sample consisted predominantly of young, brown, married adult women with average body mass index (BMI), higher education level, and belonging to the upper-middle class, most of whom were physically active and did not reveal any bias. The most used criterion for choosing the plastic surgeon was the indication of friends or family. Conclusion: This study produced reliable results for describing patients’ sociodemographic characteristics and life habits, allowing the identification of the most relevant factor in the plastic surgeon’s choice. ■ ABSTRACT","PeriodicalId":79099,"journal":{"name":"Revista brasileira de cirurgia","volume":"36 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"71174700","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Importantly, control of exsanguinating haemorrhage is now addressed at the first stage of the primary survey by applying direct pressure to the source of major bleeding or application of a tourniquet. Only hard signs (lack of palpable pulses, continuing blood loss, or expanding haematoma) should be used to diagnose vascular injury. A devascularised limb is a surgical emergency and vascularity should be restored as soon as possible, and definitively within 3–4 hours, using vascular shunts.
