The latter were analyzed by light microscopy, as well as immunohistochemically for HPV structural proteins. HPV DNA typing was accomplished by Southern blot, spot, and in situ hybridization techniques using the DNA probes for HPV 6, 11, 16, 18 and 31. Of the 513 lesions, 24.8% regressed, 59.8% persisted, and 14.1% progressed, 11.9% having been coned due to progression to carcinoma in situ (CIS). So far, 1.1% of lesions have recurred after such a treatment. The natural history was significantly associated with the grade of CIN (HPV-NCIN versus HPV-CIN, p less than 0.01). The progression rate was highest (33.3%) and regression lowest (5.6%) in HPV 16 lesions. All the recurrent lesions disclosed HPV 16 DNA, this type being found most frequently in the severest lesions (CIN III). Depending on the hybridization technique used, changes in HPV type were found in 11-17% of the lesions during the follow-up. Noteworthy is the discovery of all the HPV types also in biopsies with no signs of HPV infection on light microscopy, suggesting a latent HPV infection in these 'regressor' lesions. The results confirm that cervical HPV infections show a natural history comparable to that of classical CIN. This report describes a case of NAE from whom a biopsy specimen was obtained and studied immunohistologically using several monoclonal antibodies against lymphocyte-associated antigens. The tumor cells occluding luminal space express both T and B cell markers such as Leu-1, Leu-3, Leu-10, Leu-M3, HLA-DR, and HLA-DQ, whereas they do not express Leu-2, Leu-4, lymphocyte-common antigen, and immunoglobulins. Factor VIII as a marker of vascular endothelial cell is false positive in the present case. Since immunoglobulins as a definitive B lymphocyte marker are negative in this case, there is no direct evidence indicating that NAE is a tumor of B lymphocytic lineage as previously reported. These results suggest the two possibilities that NAE is a unique variant of T cell lymphoma or true vascular endothelial tumor sharing the common histogenesis with malignant lymphoma. Although cytologic, histologic, ultrastructural and molecular biologic data support the concept of HPV as an oncogenic virus, the precise mechanisms linking the infection with cancer are not known at this time. Three categories of histologic features of osteomalacic bone are proposed: (1) increased osteoid, elevated osteoblast and osteoclast numbers and marrow fibrosis; (2) increased osteoid and osteoblast numbers, and (3) increased osteoid associated with few osteoblasts. Clinical conditions for which quantitative bone histomorphometric data are available are presented and related to the three categories. {"title":"Epidemiological data on cervical carcinoma relevant to cytopathology.","authors":"C La Vecchia,&nbsp;A Decarli,&nbsp;G Gallus","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Epidemiological studies based on cervical cytopathology have aroused widespread interest since it was realized that they could be a useful tool for measuring the effectiveness of screening programs and defining practical measures for the prevention of invasive neoplasms and deaths. In the present article, published evidence from screening programs, cohort investigations and case-control studies is reviewed, and possibilities for further analyses and applications are discussed. In particular, when estimates of the relative protections conveyed by Pap smear from various case-control studies conducted on different populations and using different criteria of selection were pooled, a surprisingly close concordance emerged, with overall risk estimates of invasive cervical cancer of 0.42 for women reporting one smear, and of 0.20 for two or more smears in the past. This protection appeared to be long-lasting in a considerable proportion of cases, since the major determinant of invasive cancer risk was the number of previous smears rather than the interval since last smear, and a noticeable residual effect was evident even more than 10 years after the last smear. Personal as well as literature data suggest (a) diffuse glomerular hypercellularity found in most cases of CRYGN is mainly due to polymorphonuclear leukocytes and even more to monocyte exudation; (b) the interposition of the latter cell type in the glomerular capillary wall is the main responsible cause of the frequent occurrence of the membranoproliferative pattern in CRYGN, and (c) peculiar structures found in the electron dense deposits are characteristic of CRYGN and related to cryoglobulin composition. Biopsies of in situ and early, well-differentiated adenocarcinomas were often limited in amount, precluding an accurate assessment of stromal invasion. In such cases, a comprehensive sample, e.g., by conization, was required to establish the diagnosis. Invasive adenocarcinoma could be distinguished from the in situ change by the loss of branching pattern of normal endocervical glands, a marked variation in the size and shape of the glands, and a complex papillary or solid growth pattern. The most important findings were a high degree of osteosclerosis, narrowing and fibrosis of medullary spaces, and the presence of abundant uncalcified osteoid tissue with an osteomalacia-like appearance. The osteoclasts were not numerous. All of them lacked the brush border and many showed wide cytoplasmic vacuoles containing amorphous and filamentous material. This condition results primarily from reduced osteoclast function which produces partial or complete absence of marrow spaces, a variable compensatory hepatosplenomegaly, interferences with tooth eruption and certain neurological complications. Experimental investigations of some mutations have shown that restoration of bone resorption and recovery from the disease follow bone marrow transplantation. These data indicate that in these mutations, including some children, the defect is intrinsic to osteoclasts or their stem cells. In other mutations in animals and children, bone marrow transplants do not restore bone resorption and the defect is believed to be one of local environmental products or signals whose importance in cell differentiation and regulation is becoming increasingly appreciated.
