The diagnosis was obtained from antenatal ultrasonography in 17 cases, investigation of an abdominal mass at routine palpation in 10, UTI investigation in 1 and uraemia investigation in 1. Eleven patients had associated malformations. Further investigations included ultrasonography, intravenous pyelography, micturition uretrocystography and in a few cases computed tomography and Dimercapto-succinic acid (DMSA) scan. Percutaneous pyelography was performed in 9 cases. Twenty-six children were operated on and 3 were not operated, of which 1 with severe cardiac malformations, died. The age at operation was under 6 months in 14 cases and between 6 and 12 months in 8. Four children developed hypertension preoperatively but the hypertension disappeared within 1 week postoperatively in all cases. The indication for surgery in unilateral multicystic kidney seems to be clear if the patient is symptomatic. Their mean age was 9.8 +/- 2.3 years. Two of them were boys. They comprise 1.6% of the surgical pathology of the spleen in our hospital, which is a Reference Centre for congenital haemolytic diseases. Two of the children had been suffering from intermittent colic pain in the left hypochondrium with a tender palpable mass for a short while to the present day. The third patient was asymptomatic; a splenic cyst with calcification of its wall was accidentally revealed by a plain x-ray of the abdomen. Contrast gastrointestinal studies and intravenous pyelography helped us with the diagnosis; they indicated a splenic mass that caused visceral displacement adjacent to the spleen. Ultrasonography proved the cystic nature of the enlargement of the spleen in all the cases. Marsupialization was performed in one patient and removal of the cysts in the other two. All three patients were followed up and re-checked 1-5 years postoperatively (May-June 1988). In one boy who underwent removal of the cyst four years ago, recurrence of the splenic cyst was revealed by ultrasonography, CT scan and radionuclide scan. Subtotal splenectomy was performed. The remaining upper pole is supplied with blood by the short gastric arteries. The recurrence was attributed to coexistence of invisible tiny cysts that had remained in the splenic tissue after the dissection of the major cyst during the previous operation. The cream (10-16 g) was applied to a total surface area of 100-160 cm2. Each area was covered by a Tegaderm dressing (3M) in order to obtain occlusion. An application time of 2 hours was used. Two age groups of ten children were studied: 2-3 years and 6-8 years. In both groups venous blood samples were drawn prior to application and at 2, 3, 4 and 5 hours. In addition, in one of the studies, the cream was used during the surgical removal of mollusca in order to study the analgesic effect of the cream. The absorption of the local anaesthetics as indicated by plasma concentrations was far below toxic levels. The highest individual concentration of lidocaine was 315 ng/ml and of prilocaine 215 ng/ml. The analgesic effect was sufficient--eight out of ten patients experienced no or slight pain. Transient local reactions (paleness, redness) were frequently observed. In one patient with eczema present at the application site prior to the application, a reaction persisted for one week. This was compared to normal arterioles in healthy newborn lungs. The wall musculature of pulmonary arterioles was 2.8 times thicker than in normal arterioles of newborn lungs. The main cause of this enormous wall thickness is a genuine growth of substance, i.e. myohyperplasia. This study includes retrospective analysis of these cases. The patients were investigated with respect to associated anomalies, radiological diagnosis, surgical treatment and prognosis. The occurrence of major trauma was 3.16% or 441 injuries in 437 children. The most usual injury was fracture of the clavicle, which occurred in 3.0%, after that brachial plexus injury with occurrence of 0.11%, and fracture of long bones with 0.03%. In addition to that there was one child with facial nerve palsy. Four children had two birth injuries each. The most important causes of brachial plexus injury were macrosomia and shoulder dystocia. Eight of 16 children with brachial plexus injury had shoulder dystocia and six of 16 macrosomia. The study material comprised 124 consecutive infants and children referred to uroradiological workup, usually because of a relapsing febrile urinary tract infection. Both examinations were done during the same session. Consistent findings were obtained on 232 ureters, of which 37 displayed II. and 19 III. + reflux. II. reflux was found in 12 ureters by cystography only; thus one-fourth of low-grade refluxes escaped the diagnosis by ultrasound. The false negative findings of ultrasound examination included also two ureters showing III. reflux in cystography. As calculated for therapy-requiring (grade III or higher) reflux, the sensitivity of ultrasound was 90 per cent and its specificity 100 per cent. The results of animal experiments and clinical experience are somewhat controversial. In this study, sixty-six boys, aged from 5 to 15, mean 10.1 years, were routinely operated on for a unilateral inguinal hernia, after which their WH was studied by the Cellstic device. Based on differential count of wound cells and their mutual transformed cell to cell ratios it was found that the optimum level of haematocrit (Hct) for WH was 37-39% with haemoglobin (Hb) values of 12.0-12.4 g/100 ml. Hct values under and over these limits began to retard WH significantly (p less than 0.02) without any visual signs of delay in WH. Both values are below the age adjusted means but just above the lower limits of normal.
