The aim of the present studies was to evaluate the effect of DOP on LES in purebread male beagles and whether in could be antagonised by the DOP-antagonist MCP. The LESP in beagles is sensitive to dopamine (DOP) and its \"antagonist\", metoclopramide (MCP). MCP raised the inhibited LESP after DOP-infusion in vivo, while it failed to change DOP-induced relaxation of opossum LES in vitro [1]. Data obtained after monotherapie with MCP did not exceed LESP-response to MCP after DOP-pretreatment. DOP applied as background counter-inhibition could be used to correct interdigestive phasic changements of LESP in order to reach a stable starting-point to investigate the action of LESP-stimulating compounds pharmacomanometrically. The effect of a ductus hepatopancreaticus blockade in experimental group A (32 animals) was greatly exacerbated by previous ligation of the ductus thoracicus (experimental group B; 34 animals). Edema of the interstitial pancreatic tissue led to lipolytic necrotizing pancreatitis with a slight hemorrhagic component, increasing ascites after the 12 th hour of the experiment, and numerous abdominal fat necroses after about 19 hours, but only relatively minor necroses of acinar parenchyma cells in the pancreas. Fat tissue necroses were only found in almost 20% of the animals in group A, and these probably resulted from manipulation of the duodenum during the implantation of a shunt to divert the bile, whereas they were found in all animals of group B after the 19th hour of the experiment, usually in large numbers. The sometimes considerable increases in serum amylase and particularly lipase activity were caused by obstruction of the efferent ducts, but did not increase appreciably after disturbance of the lymph drainage systems. But there are adverse effects in 20-30% dependent on the serum-level of the resorbed SASP-metabolite sulphapyridine (SP). Relapses during treatment may have their cause in an under-dosage or patient's non-compliance. In 19 patients with UC the possibility of an individualized most effective treatment and the patient's compliance by the simple practicable analysis of SP-serum-level were checked. The results show that it is necessary to dose the SASP in dependence of the genetically determined type of acetylation. The SASP-dosage must be decreased in patients with slow acetylation and vice versa consequently. After rectal SASP-application - in opposite to the oral treatment - significant lower serum levels of SP were analyzed. The increase of AP is known to be caused by a de novo-protein synthesis in the liver, as several investigators have demonstrated using protein synthesis inhibitors. In this study female Wistar rats were partly sham-operated, partly bile duct-obstructed. One half of each group received the protein synthesis inhibitor ethionine, 24 h after bile duct obstruction in the group with ethionine the following enzyme activities were measured in the serum: AP 216 +/- 80 U/l; gamma-GT 4.5 +/- 1.9 U/l; GlDH 85 +/- 26 U/l; GPT 375 +/- 163 U/l. In the group without ethionine the activities were: AP 459 +/- 69 U/l; gamma-GT 4 +/- 0.9 U/l; GlDH 120 +/- 81 U/l; GPT 417 +/- 191 U/l. This was followed by the simultaneous administration of all these substances as a \"cocktail\". A comparison between the single and the \"cocktail\" dosage did not reveal any significant differences in the pharmacokinetic parameters. So the \"cocktail\" administration is even possible in chronic liver diseases. Requiring relatively little time, it makes statements possible concerning various cytochrome P450 enzymes including the types of hydroxylation and acetylation. Incidence and complications in these patients were evaluated retrospectively. The most frequent eliciting factors were gastrointestinal bleeding, infections and acute alcohol excess, more than 2/3 of them having severe portal hypertension with ascites and esophageal varices at the time. Altogether 55 of the 59 liver coma patients died. The most important complications were, beside bleeding and infections, disturbances of the renal function and of the coagulation. Out of 400 colorectal carcinomas from the periods of 1960 to 1966 and 1981 to 1983 126 (32%) of all carcinomas remained clinically undetected during lifetime of the patients. In 1/3 of these cases the clinician did not suggest a malign neoplasm, in the other 2/3 a malignant neoplasm of unknown or false origin was assigned. 44% of the right-sided colon carcinomas, 36% of the left-sided colon carcinomas and 22% of the rectal carcinomas remained clinically undetected. At the time of autopsy in 10 cases (13%) a second malignant neoplasm far from the colorectal region could be demonstrated. Among 2944 patients, admitted with being suspect of having viral hepatitis, there were 128 patients (adequate to 4.4%) with a drug-induced liver injury diagnosed in the result of all clinical, serological, histological and immunological findings. Inducing noxes were - in accordance to the frequency of ordination - ovulation inhibitors followed by Berlocombin, Ketazon, Depressan and Obsidan. In view of a clinic for infectious diseases it has to be pointed to the necessity of a high degree of security in the differential diagnosis of the drug-induced liver injury in relation to the viral hepatitis and one has to consider the great responsibility of the clinician. The results of bacteriological, histological, enzymatic and chemical methods were compared. Urease activity was determined both in biopsies (CLO-test) and in gastric secretion. Furthermore, the concentration of urea was measured in gastric secretion. 71% of gastritis, 86% of ulcus duodeni and 83% of ulcus ventriculi diagnosed by endoscopy produced positive histological and/or cultural results, while 100% of control persons exhibited negative for both parameters. The sensitivity of the CLO-test (n = 112) was 95% and its specificity 78% when compared with bacteriological and histological results. The majority of false positive results was due to delayed and incomplete reactions. However, when compared with endoscopic-histological results the specificity of the CLO-test was 100%. The results of a modification of CLO-test (without culture medium) were up to 5 hours comparable. There was no positive correlation between the concentration of urea in gastric secretion and the histological or cultural identification of Campylobacter pylori. However, the measurement of ammonia turned out to be promising (sensitivity 80%).
