The quantity is difficult to estimate. We did it by measuring the acid output as the aggressive factor and we sized the resulting mucosalesions. In the gastric corpus we could not find any change in mucosaprotectionindex after cimetidine, pirenzepine and vagotomy. Significant correlation with age concerned the normal findings only. In patients with duodenal ulcers we found in 91% gastritis and in 88% campylobacter pylori in antral mucosa. A small group with duodenal ulcers was campylobacter pylori-negative as well as without gastritis signs. Comparing the antrum and corpus gastric mucosa (250 pat.) the antral mucosa was representative for the whole stomach in view of gastritis and campylobacter pylori test, sufficiently for practical use. From September 1987 to June 1989 80 patients (56 women, 24 men; mean age: 66 years) had been selected for ESWL at the Charité-Hospital. Indications were: Impacted stones (30 patients), very large stones (22 patients), stones above bile duct strictures (28 patients). 79 patients have been treated by device \"Lithostar\" (Siemens) during 114 sessions in abdominal position under X-ray visualization. ESWL-treatment was impossible in one patient because of technical reasons. Per treatment session 1000 to 5000 shock waves with an energy of 19 kV, directed on a single or distributed among several stones, have been applied. In 79 cases out of 114 treatments complete or partial fragmentation (69.3%) was attained. Stone fragments were removed endoscopically in 56 patients and in 1 patient percutaneously via biliary drainage. In 5 cases the fragments passed spontaneously. On the total, in 72.2% of our patients stones clearance was achieved. Immediately, 2 and 7 days after TAA cessation it was investigated by means of in vivo (caffeine and metamizol elimination) and vitro methods (cytochrome P-450, 7-ethoxycoumarin and 7-ethoxyresorufin O-deethylation), whether this animal model represents the restricted cytochrome P-450-dependent biotransformation comparable to human liver cirrhosis. The total capacity of the liver was diminished immediately and 2 days after TAA cessation. After 7 days the capacity was unchanged compared to the controls or partly even enhanced. We performed a study in 3 groups of 12 compensated and 7 decompensated cirrhotic patients and 11 controls to determine the renal, hemodynamic and hormonal effect of repeated water immersion (6 immersions in 3 weeks). In all groups immersion resulted in a marked acute natriuresis and diuresis combined with acute reduction of blood pressure. In both cirrhotic groups we found decreased salt excretion except the acute effect of immersion in comparison with controls. The natriuresis of the immersion day were in decompensated patients significantly elevated in comparison with a control day. This was not due to the acute, but the prolonged immersion effect on renal excretion. During the study the immersion effects increased in both groups of cirrhotic subjects. The controls showed decreased responses. The immersion response shall be caused at least, in part by the Atrial Natriuretic Peptide (ANP): all groups had a significant elevation of ANP during the immersion which was like the renal natriuresis prolonged in the 6th immersion in cirrhotics. The cutting biopsy cannula represents a compromise between the fine and the Menghini needle. In this way, small tissue cylinders can be attained. The aim of the present study was to investigate to what extent the sampled specimen could be assessed by cytology or histology. Fine-needle puncture resulted in 22 (69%) out of 32 cases in material suitable for cytology. Cutting biopsy led in 34 out of 36 cases (94%) to specimen valid for histology. Thus, cutting biopsy clearly extends our diagnostic tools. The most frequent changes of the hepatocytes are those of the organelles, which are adaptive at the beginning, but in later stages they can develop to degenerative alterations leading to necrosis. Cases of drug-induced hepatitis simulating all types of non-suppurative hepatitis are a major problem of diagnosis. Bile duct lesions can include pure cholestasis, cholangiolitis and destruction of intrahepatic ducts. Drug-induced vascular lesions including tumours can be found as isolated phenomenon or in association with other signs of drug-induced liver damage.
