{"title":"[Sensitivity and specificity of alpha-1-antitrypsin and acid alpha-1-glycoprotein in colorectal carcinoma].","authors":"H Putzki,&nbsp;O Hafner","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In a group of 80 patients with colorectal cancer and a control group of 91 persons and in 15 patients with acute diverticulitis of the sigmoid colon acid alpha-1-glycoprotein (AGP) and alpha-1-antitrypsin (AAT) were measured. The median of AAT in the carcinoma group was 3.34 g/l, in the control group 2.39 g/l. AGP was in the carcinoma group 1.19 g/l, in the control group 0.79 g/l. Both differences are significant. The sensitivities of AGP and AAT were compared with the sensitivities of CEA and CA 19/9 using ROC-curves. AAT and AGP have a distinctly lower sensitivity than CEA, but only on the basis of a high specificity of 95%. In the region of lower specificities AAT has the highest sensitivity. Therefore, the sensitivity of CEA for colorectal carcinoma is not reached by AAT and AGP, when a high specificity of the test is required. We investigated the following criteria: a) the gallbladder volumes after overnight fasting on 5 consecutive days, b) the spontaneous motility under fasting over 12 or 24h at hourly intervals, c) the emptying rate and time of maximal contraction after stimulation with sorbitol, and d) the emptying rate and time (like c) after premedication with \"cholagogic\" drugs (Cholecysmon, Divalol), nifedipine, indomethacin or N-butylscopolaminiumbromide. We found a great intrapersonal and interpersonal variability in gallbladder volumes after overnight fasting in consecutive days. In subjects fasting over 24h phases of dilatation with maxima at 12 AM and 4 PM alternated with phases of contraction with maxima at 8 AM and 4 PM. After premedication with nifedipine, indomethacin and N-butylscopolaminiumbromide a significant decrease of the emptying rate was observed. The time of maximal contraction did not differ however. After premedication we found a distinct intra- and interindividual variability too. Pentagastrin, given as a 1-hour intravenous infusion, changed the interdigestive motility to an irregular (postprandial) pattern, which continued up to the end of measurement or was followed by a prolonged phase III with return to cyclic activity. Infusion of somatostatin for 1 hour interrupted the interdigestive motility leading to a prolonged phase I pattern and to partial discoordination. After the end of infusion phase III motility returning to cyclic activity or irregular activity were registered. A standard relaparotomy 10 days after implantation was performed in 3 dogs. The disease dependent mucosa types were classified into three groups according to Shmerling (1970). 5-hydroxytryptamine was demonstrated with an impregnation technique and histochemically, as well, in EC-cells in the crypt epithelium. The EC-cell content in specimens with subtotal villus atrophy (type III) was higher significantly compared to normal mucosa (type I). Furthermore, the elimination of caffeine and metamizol was determined in serum as a parameter of in vivo biotransformation. In a further step the activity of 7-ethoxycoumarin O-deethylation was measured in liver biopsy samples of 30 out of 45 patients (in vitro parameter of biotransformation). The half-life of ICG was compared with the data of biotransformation by means of the calculation of correlations with special consideration of histological findings in liver biopsy samples (monocellular necrosis, intra-acinar fibrosis, structural transformation). Results demonstrate that ICG-elimination is a nonsufficient criterion of liver blood flow in patients with liver disease, because of considerable variation and many influencing factors including the function of the liver cells. Smoker with chronic liver diseases had higher 14CO2-results than nonsmoker. The difference was only significant in fatty liver disease in the first stadium (p less than 0,001) and chronic active hepatitis (p less than 0.05). Patients with chronic liver diseases of fewer severity had higher mean values of aminopyrine breath test, if they had chronic alcohol consumption. The difference between these and abstinent patients could secure only in fatty liver disease in the first stadium (p less than 0.001). Patients with severe chronic liver diseases had lower results of aminopyrine breath test, if they drink alcohol regular. The evaluation of data by cluster analysis, optimized with the aid of multivariate normality testing, was able to recognize correctly a majority of cases. For diagnosis of malignancy, an 89% sensitivity and 82% specificity was obtained. To investigate liver blood flow intravenous indocyanine green (0,5 mg/kg) was given 12 and 1 hours after oral verapamil (80 mg) or nifedipine (20 mg). In the presented study an influence of verapamil and nifedipine treatment on the hepatic drug oxidizing system was not observed. The elimination half-life of intravenous indocyanine green was only 1 hour after oral nifedipine (20 mg) significantly decreased. 12 hours after nifedipine (20 mg) or verapamil (80 mg) as well as 1 hour after verapamil (80 mg) treatment no change in indocyanine green elimination was observed. The estimated increase of liver blood flow after oral nifedipine may cause increased absorption of other oral administered drugs and may accelerate hepatic elimination of drug with high hepatic extraction rate. 6 fast an 6 slow acetylators were found. As a control we used the return of the parameters of electrical and motor activity to their original levels. The reaction was registered with the help of electromyography, using bipolar electrodes and with measurement of intraluminal pressure, using open-tip catheter technique. Spike-activity was graphically presented and statistically performed. It was shown, that Secretin and Glucagon caused relaxation of duodenum, but Pentagastrin, Pancreozymin and Somatostatin stimulated duodenal motility with spastic component and gradual phasic character of a process. After truncular vagotomy the reaction to Pancreozymin and Pentagastrin was slowly, to Glucagon strongly reduced, but the reaction to Secretin was extended. We observed after TV either no reaction or a diminished reaction to Somatostatin. Main principles are known as \"Overfilling\"--and \"Underfilling\"-hypotheses followed by disturbances of different kinds of mediators influencing renal blood flow and natriuresis. The endpoint of a cascade is the development of a hepatorenal syndrome which is connected with therapeutic resistance against diuretics.
