These plans are necessary today to drive improved performance and overall success in the increasingly difficult health-care environment. Thus, effective scheduling of nursing personnel is important in controlling health-care costs. Scheduling nurses in hospitals is a complex problem due to variability in demand and a variety of conflicting interests and objectives. This article reviews the models that have been developed for nurse scheduling, including heuristic-, optimization- and artificial intelligence-based models. Given the especially crucial role played by employees in the health-care field, and the relatively high level of education and sophistication that characterizes this group, it is also recognized that initiating and sustaining meaningful employee involvement programs in a health-care setting represent unique challenges and opportunities. This article presents the operational evolution of a successful employee involvement program in a large, diversified academic medical center. The developed program includes quality-improving and revenue-enhancing suggestions as well as cost-containment ideas. Specific operational issues and program changes are examined within the context of the total quality process at the University of Michigan Medical Center. The first issue is the process for developing a range of alternatives at various levels of sophistication. A second key issue is the development of a full cost analysis which includes such costs as cost of parallel operation, cost of back up systems, training costs and initial system modification costs. The article describes the components of the clinical data system as well as its functionality. The advantages of automated encoders for coding, editing and grouping of clinical data are outlined, as well as the advantages of relational database software for clinical data management. Clinical data support a myriad of activities within the health care facility. The microcomputer system provides the departmental user with flexibility and customization to meet and manage the ever-changing clinical data requirements.
