{"title":"Urostomy and urinary pH.","authors":"B A Walsh","doi":"10.1097/00152192-199207000-00004","DOIUrl":"https://doi.org/10.1097/00152192-199207000-00004","url":null,"abstract":"<p><p>Significant variations of urinary pH can cause problems for all human beings, but these problems are magnified when an individual has a urostomy. Most significant stomal and peristomal complications are related to an alkaline urine including hyperkeratosis; stoma bleeding, incrustation, and ulceration; stoma stenosis; urinary tract infection; odor; and urinary calculi. Treatment of these conditions includes both external and internal measures. External methods of treatment involve keeping urine away from the stoma and the peristomal skin by use of a correctly fitting clean appliance and a night drainage system. Vinegar solution compresses can help to restore the acid mantle of the skin. Internal methods of treatment that are advocated in the literature include ingestion of cranberry juice and ascorbic acid to promote urine acidity. The beds are designed to reduce body surface pressures for patients with potential for or actual pressure injury related to mechanical forces (pressure, friction, and/or shear). A monitoring system for therapeutic bed usage was initiated by the Enterostomal Therapy Department in March 1990 because of concerns about prolonged and sometimes unnecessary use of the beds. {"title":"Chronic parastomal ulcers: spectrum of dermatoses.","authors":"C S Ng, H C Wolfsen, R A Kozarek, L L Brubacher, A L Kayne","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Parastomal ulcers that develop after stoma surgery have reportedly been associated with recurrent inflammatory bowel disease and chronic infection. We report 13 patients with refractory parastomal ulcers, which occurred at a mean of 11 years after surgery. Parastomal ulcers in eight patients were the result of dermatologic conditions (e.g., contact dermatitis, bullous pemphigoid, lichen sclerosus et atrophicus, eczema, or psoriasis) or contact ulcers from dermatitis of the skin around the stoma and faceplate pressure. These ulcers healed after treatment with topical medications at a mean of 4 weeks. Five patients with inflammatory bowel disease had pyoderma gangrenosum ulcerations, which healed with systemic treatment at a mean of 25 weeks. {"title":"Quality monitoring for maintenance of skin integrity.","authors":"S S Ward","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In today's cost-conscious health care environment, the ET nurse is in a unique position to be instrumental in improving the quality of patient care. Continuous quality improvement in a cost-effective manner is the goal. Through collaborative efforts, the ET nurse and staff nurses have developed quality indicators for the maintenance of skin integrity in the belief that process, when provided effectively, will increase the probability of a desired outcome. Process indicators measure discrete nursing activities that contribute directly or indirectly to patient care. Closely linked process and outcome indicators were selected. Outcome refers to the effects of treatment on patients.
