{"title":"Treatment research priorities for human immunodeficiency virus infection.","authors":"C B Pettinelli,&nbsp;S M Schnittman","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Viral infections such as with the human immunodeficiency virus (HIV) present difficult challenges for the development of effective antiviral therapies. These viruses depend on the host cell machinery for their existence, and interference with these processes typically interferes with other important host physiology. HIV presents other challenges as well because of its inherent pathogenic destruction of the immune system. It is the goal of HIV therapeutics to attempt to cure HIV infection, or if that is not possible, to stop HIV disease progression while preserving a high quality of life for HIV-infected individuals. This may be achieved through an effective combination of interference with the viral life cycle and the pathogenic processes, and by slowing or reversing the immunologic dysfunction that leads to the complications of HIV infection. Unprecedented progress has been made in understanding the virus and HIV disease pathogenesis. This knowledge has led to the identification of viral features that have become targets for therapeutic intervention. This article reviews the most important priorities of HIV treatment research for adult HIV-infected patients for the immediate future. Although introduction of Hib polysaccharide vaccines had little impact on disease incidence, development and use of Hib polysaccharide-protein conjugate vaccines dramatically reduced Hib meningitis rates. With widespread use of the new Hib conjugate vaccine, elimination of Hib meningitis in the United States may be achieved. These events within the cell have been studied extensively using the model system of infection with Listeria monocytogenes. This bacteria resides primarily intracellularly; immunity to it is mediated by cellular responses. Upon phagocytosis by a macrophage, engulfed Listeria express a hemolytic molecule, listeriolysin O (LLO), and can disrupt the membrane of the endosome and escape into the cellular cytoplasm. This allows the organism to escape the hostile environment of the endosome and also gives it access to the processing machinery of the cytoplasm. Therefore, Listeria monocytogenes can be processed within an endosome and be presented by class II MHC, or can escape into the cytoplasm and be processed there and presented by class I MHC. This LLO molecule is not only an important virulence factor, it is also a dominant antigen in the cellular immune response to Listeria.
