The site of origin was found to be occupied by an abundance of adipose cells. In the marginal zone, lipid droplets were obviously increased in count in the cytoplasm of both myocytes and blood capillary-endothelium, rather more than in distant areas. Lipid droplets in the endothelial cells also occurred at a higher frequency in the ARVD group than in any other myocardial disease showing VT. A mechanism which increases lipid droplets in the cytoplasm of cardiomyocytes and endothelial cells operates at the VT origin site in ARVD. Intraendothelial lipids may be an important diagnostic finding in the histology of ARVD.</p>","PeriodicalId":77157,"journal":{"name":"Heart and vessels. Thirty five patients (64%) were found to have calcification of the ventricular walls. Calcific deposits were seen in the right ventricle in 26(59%) patients and in the left ventricle in 16(44%) patients. All patients who showed calcification were in severe cardiac failure indicating late stage of the disease. When image intensifier fluoroscopy was taken as the gold standard, echocardiography had a sensitivity of 100% and a specificity of 98% in detecting cardiac calcification. The high incidence of calcification in the ventricular walls in the present study is apparently due to the systematic use of two-dimensional echocardiography and image intensifier fluoroscopy.</p>","PeriodicalId":77157,"journal":{"name":"Heart and vessels. LV volume and instantaneous rate of change of volume were derived from frame-by-frame analyses of angiograms. LV relaxation was assessed by the time constant of isovolumic pressure decay. LV compliance was evaluated by the pressure-volume relationship. From these analyses, we conclude as follows: (1) In restrictive cardiomyopathy (RCM), LV diastolic dysfunction may be characterized by impaired relaxation and decreased compliance; (2) LV diastolic filling was minimally impaired or normal in both conditions; (3) The ventricular diastolic pressure wave form of \"dip and plateau\" seemed not to be a characteristic sign in RCM.</p>","PeriodicalId":77157,"journal":{"name":"Heart and vessels. ECG abnormalities consisted of atrioventricular block (AVB) in 11, ventricular arrhythmia including premature ventricular contractions (PVC) and ventricular tachycardia in 7, sick sinus syndrome (SSS) in 3, and bundle branch block in 2 cases. Biopsy specimens were obtained from the right ventricle in all cases and, additionally, from the right atrium in SSS cases. Biopsy revealed significant pathology in 19 cases (83%). Advanced histologic changes, including myocyte hypertrophy, disarrangement of muscle bundles, and interstitial fibrosis with or without myocyte degeneration, were observed in 7/11 AVB cases and 1/6 PVC cases. SSS cases showed interstitial fibrosis with disarrangement of muscle bundles in the right atrium, but no significant pathologic changes were seen in the right ventricle.
