{"title":"Small bowel imaging.","authors":"J C Lappas","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Radiologic imaging continues to play an integral role in the diagnosis and management of diseases of the small bowel. The mesenteric small intestine remains the only gastrointestinal tract segment for which diagnostic study is not principally dependent on endoscopic viewing. Because radiologists assume primary responsibility in the diagnostic evaluation of the small bowel, it is essential that methods capable of accurately demonstrating small bowel morphology are appropriately applied. Barium contrast studies and enteroclysis, in particular, remain the primary diagnostic methods in the small bowel for most clinical indications. Many recent reports focus on the deep abdominal applications of these modalities. Although duplex tracing remains the basis of most diagnoses, the use of color Doppler imaging has facilitated the performance of these examinations. Endosonography performed with high-resolution transducers in the lumen of the gut is another exciting and expanding area. Staging of gastrointestinal tumors is the major application of this modality. The question for the radiologist, therefore, is the definition of selected populations and situations for the use of low-osmolality agents to achieve the greatest patient benefit at the least cost. Fat-suppressed T1 weighting and dynamic-enhanced imaging are two new methods for evaluating renal and adrenal masses. MR angiography techniques are beginning to depict anatomy with sufficient detail to be considered clinical tools. Bladder cancers can be better staged with enhanced high-resolution images, especially when combined with surface coils. Endorectal surface coils continue to provide dramatic images of the prostate and have significantly improved local staging with MR imaging. {"title":"Neonatal and pediatric sonography.","authors":"D K Yousefzadeh","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>This paper reviews the contribution of sonography in assessing common and some less common neonatal and pediatric disorders. The samples reflect the strength or currently perceived weaknesses of sonography and duplex Doppler studies in both neonates and children. Controversies, when present, are highlighted to provoke thought and remind us that many of the premises on which we base our research are, themselves, still shaky. Comments are provided to encourage complementary studies by other investigators or by the original authors. Because of their significance for the practice of pediatric radiology and sonography, two studies, one adult and one experimental, are incorporated. Since there is still plenty of room for improving sonographic techniques and technology and because new developments, such as the use of contrast agents, are in the horizon, the true limitations of sonography and duplex and color Doppler is yet unknown. Therefore, because of the well-known advantages of using sonography in children, we must exhaust all our efforts, \"tricks,\" and resources as a sonographer before we subject the child to more invasive, stressful, and expensive studies. {"title":"Radiology and treatment of urinary tract stone disease.","authors":"W H Bush","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Renal and ureteral calculi are treated primarily using extracorporeal lithotripsy, with percutaneous nephrostolithotomy retaining an important role for the treatment of large stones and complex situations. Aspects of pretherapy evaluation are reviewed. The vast majority of calculi 5 mm or less in diameter in the mid and lower ureter will pass spontaneously; in patients with acute ureteral obstruction, lower-osmolar nonionic contrast for urography caused as much discomfort as conventional high-osmolar contrast. Treatment planning for extracorporeal lithotripsy has changed in that internal stenting is no longer routinely recommended. Milk-of-calcium and calyceal diverticular stones respond poorly to extracorporeal lithotripsy. The access route used for percutaneous stone removal varies among investigators. Some advocate an intercostal approach for up to one third of patients; substantial complications occur with placement of a track above the 11th rib. Sepsis develops after percutaneous nephrostomy in up to 21% of patients, but the risk of sepsis can be decreased significantly by the administration of antibiotics during and after the procedure. Complications of extracorporeal lithotripsy include renal hematoma (especially if the patient is hypertensive or is taking aspirin), regional organ injury, and bacteremia. Although originally feared to occur frequently, hypertension occurring after or caused by extracorporeal lithotripsy was not confirmed to be a major problem. This paper discusses the role of endovaginal sonography in common first trimester diagnostic problems, including pregnancy dating, first trimester pregnancy loss, ectopic pregnancy, and early diagnosis of fetal anomalies, and reviews the use of endovaginal color flow Doppler in the diagnosis of ectopic pregnancy. Clinical methods of assessing fetal growth are disappointing and necessitate that a high-risk group be identified for more intensive surveillance. This review discusses the role of ultrasound biometry in establishing whether a fetus is small. Behavioral characteristics of the fetus may also be assessed using real-time ultrasound. The realization that not all small fetuses are a result of growth failure has led to the development of Doppler ultrasound to identify the fetus at risk.
