Our first experience confirm the excellent cosmetic and functional results after laser surgery. Furthermore a decrease of recurrence rate was found due to laser vaporization of the adjacent tumor area. Especially in idiopathic fibrosis the initial placement of percutaneous nephrostomy and long-term immunosuppression is the treatment of choice. {"title":"[ESWL with the Lithostar--experience with 3,000 treatments].","authors":"G Brien, C Bick, P Kirschner, P Althaus, E Tutschku, S Christoph","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The experiences with 3,000 extracorporeal shock wave lithotripsies of kidney and ureteral calculi using the Lithostar are reported. Indications, contraindications and therapeutic approach are reviewed and the importance of out-patient follow-up is stressed. The success rate of 75% (including further spontaneous passage of residual desintegrated calculi) is good in the view of an unselected and unfavourable patient population. In the other patients a second ESWL treatment may be necessary. {"title":"[Laboratory diagnosis of cystinuria].","authors":"W Berg","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Delayed diagnosis, apparently due to insufficient knowledge of partly simple methods of cystine determination and their practical application, is often a particular problem in the treatment of cystine lithiasis. The present paper gives a survey of a variety of laboratory methods to determine cystine in the urine both qualitatively and quantitatively. Their applicability in various diagnostic situations is discussed. In clinico-chemical routine, colorimetric cystine determinations using Brands test, but partibularly the nickel/dithionite reagent, is an established and reliable laboratory method. In the untreated control group no changes in the values of Ca, Na and Zn were found. In larger stones the ultrasonic lithotripsy has been used. Due to our experience and a review of the literature it was found that the majority of urinary calculi in children could be removed endoscopically. The open operation should be used in exceptional cases. Braun, Melsungen, FRG) Hemosorbent SKN-2K (USSR) and the own product Wofatit UH 91. As adsorptives the hypnotics Metaqualon, Pyrithyldion, Crotylbarbital and Phenobarbital were used. The investigation have been performed in a single-pass system in a relation of 1:25 to the clinical practice conditions. The concentration measurements to the estimation of adsorbed amounts were made by UV-VIS spectrometry. It was found that Wofatit Y 88 is superior to Hämoresin with regard to adsorbed amounts and adsorption speed.
