In the initial quantitative phase of the study, a clinical audit of oral health services provided for adults with intellectual disabilities was carried out for individuals who underwent a general anaesthetic (GA) for dental treatment at Christchurch Hospital during a 5-year period. In a qualitative second phase, fifteen semi-structured interviews were carried out for 13 individuals with an intellectual disability to investigate the experience and perceptions of individuals with an intellectual disability (and caregivers and guardians) relating to their oral health care.</p><p><strong>Results: </strong>The majority of the treated patients lived in care, and many had severe medical problems. Most had both restorative treatment and tooth extractions carried out under GA. Their major concern was anxiety associated with the dental visits. Support people/ guardians shared this concern, and played a vital role in helping to manage this anxiety. Many of the latter had difficulty identifying dental problems in their charges, and they often relied on detection of changes in the individual's behaviour or demeanour. The time spent waiting in waiting rooms for treatment was a frequently reported cause of stress.</p><p><strong>Conclusions: </strong>As a group, these individuals present challenges for the provision of oral health care, given their severe disability and medical conditions, and many require a high level of support for daily activities. When dental treatment is required, a GA is often necessary in order to carry it out. The management of anxiety was a key issue for the patient group and support people play a vital role in helping to manage this anxiety, and achieve a successful visit. {"title":"Post-operative complications following dental extractions at the School of Dentistry, University of Otago.","authors":"Darryl C Tong,&nbsp;Haidar H Al-Hassiny,&nbsp;Adrian B Ain,&nbsp;Jonathan M Broadbent","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objectives: </strong>To determine the frequency and correlates associations of post-extraction complications at a dental school.</p><p><strong>Design: </strong>Retrospective review of patient records.</p><p><strong>Setting: </strong>Exodontia clinic at the School of Dentistry, University of Otago, Dunedin.</p><p><strong>Main outcome measures: </strong>Provider characteristics, patient demographic characteristics, patient medical history, teeth extracted and occurrence of postoperative complications.</p><p><strong>Results: </strong>Of the 598 extractions (540 routine and 58 surgical) which were undertaken in the audit period, 74 (12.4%) resulted in post-operative complications. Dry socket and post-operative pain were the major complications. A higher complication rate was found among patients treated by fourth-year undergraduate students than among those treated by more senior students or staff. {"title":"Dental technology services and industry trends in New Zealand from 2010 to 2012.","authors":"S S Alameri,&nbsp;J M Aarts,&nbsp;M Smith,&nbsp;J N Waddell","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To provide a snapshot of the New Zealand dental technology industry and influencing factors.</p><p><strong>Background: </strong>Developing an understanding of the commercial dental laboratory environment in New Zealand can provide insight into the entire dental industry.</p><p><strong>Methods: </strong>A web-based survey was the primary method for data collection, with separate questionnaires used for dental laboratory owners and dental technician employees.</p><p><strong>Results: </strong>The mean net income for dental laboratory owners in New Zealand was similar to that of the United Kingdom, at $40.50 per hour. Clinical dental technicians are the highest paid employees, with a mean of $33.49 per hour. The mean technical charge for complete dentures was $632.59; including clinical services, it was $1907.00. The mean charge for a porcelain-fused-to-metal (PFM) crown was $290.27. Dental laboratory owners expressed fear about the possibility of losing dental clients to overseas laboratories due to the availability and cheap charge of offshore work. Only 25.4% of dental laboratories surveyed had computer-aided design (CAD) facilities, and even fewer (7.9%) had computer-aided manufacturing (CAM) systems.</p><p><strong>Conclusion: </strong>Clinical dental technology appears to be prospering. {"title":"Peri-implantitis diagnosis and treatment by New Zealand periodontists and oral maxillofacial surgeons.","authors":"Assil A Russell,&nbsp;Andrew Tawse-Smith,&nbsp;Jonathan M Broadbent,&nbsp;Jonathan W Leichter","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objectives: </strong>This study aimed to investigate the understanding, diagnosis and management of peri-implantitis by New Zealand periodontists and oral maxillofacial surgeons (OMFS).</p><p><strong>Design: </strong>Telephone interviews (in combination with a postal and electronic survey) were conducted of all 25 periodontists and 32 OMFS listed as specialists on the New Zealand Dental Register. A seven item multi-choice and short answer questionnaire was used to investigate: their definition of peri-implantitis; the number of annual referrals received in their practice for this condition; their diagnostic, preventive and treatment strategies for peri-implantitis; and their perception of the role of general dental practitioners in its management.</p><p><strong>Results: </strong>The participation rate was 84.6%. Most respondents defined peri-implantitis as a disease of multifactorial aetiology that leads to destruction of the bone supporting an implant. The average number of cases seen annually differed between periodontists (11 cases/year) and OMFS (4 cases/year). The criteria used by the respondents to diagnose peri-implantitis included increased probing depths and radiographic evidence of bone loss. Each type of specialist used mechanical debridement for treatment, but a higher proportion of OMFS performed surgical procedures as treatment. The prevention strategies used smoking cessation advice and ensuring good plaque control. All respondents agreed that peri-implantitis is an important disease that can lead to implant failure, and all acknowledged the role of general dental practitioners in diagnosis, referral for treatment and long-term implant maintenance.</p><p><strong>Conclusion: </strong>The definition, diagnostic criteria and management strategies used by New Zealand specialists are generally consistent with those found in the literature. {"title":"Ukaipō niho: the place of nurturing for oral health.","authors":"J R Broughton, M Person, J Te H Maipi, Koi R Cooper-Te, A Smith-Wilkinson, S Tiakiwai, J Kilgour, K Berryman, K C Morgaine, L M Jamieson, H P Lawrence, W M Thomson","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objectives: </strong>To report on oral-health-related characteristics, beliefs, and behaviours among participants in a randomised control trial of an intervention to prevent early childhood caries (ECC) among Māori children, and to determine whether there were any systematic differences between the intervention and control groups at baseline.</p><p><strong>Design: </strong>Baseline measurements from a randomised control trial (involving 222 pregnant Māori women allocated randomly to either Intervention or Delayed groups) which is currently underway.</p><p><strong>Setting: </strong>The rohe (tribal area) of Waikato-Tainui.</p><p><strong>Methods: </strong>Self-report information collected on sociodemographic characteristics, pregnancy details, self-reported general and oral health and health-related behaviours, and oral health beliefs.</p><p><strong>Results: </strong>Other than those in the Delayed group being slightly older, on average, there were no significant differences between the two groups. Some 37.0% were expecting their first child. Most reported good health; 43.6% were current smokers, and 26.4% had never smoked. Only 8.2% were current users of alcohol. Almost all were dentate, and 57.7% described their oral health as fair or poor. One in six had had toothache in the previous year; 33.8% reported being uncomfortable about the appearance of their teeth, and 27.7% reported difficulty in eating. Dental service-use was relatively low and symptom-related; 78.9% needed to see a dentist. Overall, most of the sample believed that it was important to avoid sweet foods, visit dentists and to brush the teeth, while about half thought that using fluoride toothpaste and using floss were important. Some 38.2% felt that drinking fluoridated water was important. Oral-health-related fatalism was apparent, with 74.2% believing that most people usually get dental problems, 58.6% believing that most people will need extractions at some stage, and that most children eventually get dental caries.</p><p><strong>Conclusions: </strong>Mothers' important role in nurturing the well-being of the young child includes the protection and maintenance of the growing child's oral health (or ukaipo niho).
